Vesico-ureteric reflux in the neuropathic bladder with particular reference to the development of renal scarring.

96 unselected and consecutive patients with myeloceles (n = 75), meningomyeloceles (n = 17) and lipomatous meningomyeloceles (n = 4) were admitted to the Wessex Regional Paediatric Surgical Centre between 1st July, 1969, and 31st December, 1971, for early closure of the spinal defect. Intravenous pyelography and cystography were performed in the first month following closure of the back and repeated at 1, 3 and 5 years of age and at 4 months in patients with upper tract dilatation or vesico-ureteric reflux at the initial examination. The mortality was 30.2% in the first year of life. Primary associated renal anomalies were found in 10.4%. Vesico-ureteric reflux was present in 18 patients by 1 month of age (28%) and in 29 patients by 5 years of age (45%). New renal cortical scars developed in 9 patients. Urinary infection, intrarenal reflux and a bladder with a high urethral resistance were factors of aetiological significance.

The bovine genes for phosphoglycerate kinase, glucose-6-phosphate dehydrogenase, alpha-galactosidase, and hypoxanthine phosphoribosyltransferase are linked to the X chromosome in cattle-mouse cell hybrids.

Bovine embryonic trachea cells were hybridized with mouse A9 cells deficient in hypoxanthine phosphoribosyltransferase, and cattle-mouse hybrid cells clones were isolated after HAT/ouabain selection. In these interspecific cell hybrids, bovine glucose-6-phosphate dehydrogenase, alpha-galactosidase, and phosphoglycerate kinase were expressed concordantly with bovine HPRT. Their expression depended on the presence of bovine X chromosome. These data indicated that the genes for G6PD, PGK, and HPRT are linked and can be assigned to the bovine X chromosome.

Lumbar epidural analgesia provded by an obstetrician.

A survey of 500 patients who were administered lumbar epidural analgesia by an obstetrician as opposed to an anaesthetist is described. The procedure is safe, patients prefer it, and it has numerous advantages over alternative methods of analgesia in labour. A plea is made for more obstetricians to be trained to use this method, so that it may become more widely available.

Patello-femoral joint mechanics and pathology. 2. Chondromalacia patellae.

Two distinct lesions affect the articular cartilage of the patella. Surface degeneration occurs particularly on the odd facet; it is age dependent, often present in youth and it becomes more frequent with increasing age. It probably does not occasion patello-femoral pain in youth, but may predispose to degenerative arthritis in that joint in later years and is regarded as a consequence of habitual disuse. The term "basal degeneration" is used to describe a lesion in which there is a fasciculation of collagen in the middle and deep zones of cartilage without, at first, affecting the surface. It was found astride the ridge separating the medial from the odd facet in twenty-three adolescents who had complained of prolonged patello-femoral pain. They were treated by excision of the disc of affected cartilage, with relief of pain in most cases. The pathogenesis of basal degeneration is related to the functional anatomy of the patella.
