Membranous lipodystrophy of bone.

A 23-year old man with a fracture of the right lateral malleolus was found to have osteolytic lesions of the lower end of both fibulas and of the right talus. He was mentally defective from birth, with convulsions from the age of 4 years and a limp since the age of 15. Biopsy tissue from the right fibula that was removed one year after the fracture consisted of marrow adipose tissue that had been transformed by the development of thick, convuluted PAS positive membranes around the cells. Electron microscopy indicated that each membrane had a complex, microvillous substructure. At the time of biopsy, lytic lesions were found in both femoral medial condyles and, one year later, in the distal shaft of the left radius. Membranous lipodystrophy is a rare, generalized, skeletal disease that was previously reported in Finland and Japan, and is usually followed by progressive presenile dementia.

The association of psycho-social factors and gynaecological symptoms.

This paper analyzes the results of a Community Health Survey conducted in Melbourne in 1971. The Survey finds that gynaecological disorders are found more often in the married and working; in those smoking more than 15 cigarettes; and in those women with family problems. The lowest incidence of gynaecological disorders was found in women from households where the main income earner was either semi-skilled, or unskilled.

Microvascular transplantation of the human fallopian tube.

An unsuccessful human fallopian tube transplant is reported. A microsurgical technique is described which initially secured a viable transplant. However, the allograft subsequently died, probably as a result of rejection. Immunosuppression of the patient caused no complications or difficulties, the donor sharing one HL-A haplotype with the patient. The operative procedure, the risks of immunosuppression, and the ethical aspects of the case are discussed in order to present the problems associated with fallopian tube transplantation.

Microsurgical tubal anastomosis for sterilization reversal.

A series of patients requesting sterilization reversal has been reviewed. Microsurgical tubal anastomosis has been successfully employed and offers improved surgical results. Minimization of the length of tube damaged at sterilization procedures is probably a major factor in determining pregnancy rates should subsequent reversal be desired.

The reduction of the intrapartum still birth rate at the Queen Victoria Memorial Hospital.

A significant reduction in intrapartum anoxic fetal deaths has occurred at the Queen Victoria Memorial Hospital over the last ten years. The possible explanations for this include a variety of factors, which may either improve the health of the population presenting to the hospital, or improve obstetric care. Epidemiological evidence suggests that the use of fetal diagnostic techniques has contributed to the reduction of intrapartum anoxic still births.

Trial of intravenous therapy in women with low urinary estriol excretion.

Estriol excretion in pregnancy is favourably improved following administration of 25% dextrose to patients with persistently low estriol excretion. A double-blind controlled trial was undertaken in 60 patients to assess the efficacy of other regimens of infusion therapy with Hartmann's solution, aminofusin, 10% dextrose, or ritodrine in Hartmann's solution. Estriol excretion rose above the lower limit of normal in 69% of the patients treated. There was no significant difference in success rates between the four solutions studied when subjected to analyses of variance and covariance. Fetal and placental weights were directly related to estriol excretion. Influences of the various therapeutic regimens on metabolic acidosis have been considered and possible reasons for therapeutic success discussed.

A trial of oral ritodrine for the prevention of premature labour.

A double blind clinical trial of oral ritodrine for prevention of premature labour was conducted in 38 primigravidae in whom the internal os of the cervix was one or more fingerbreadths dilated at 28 to 32 weeks gestation. The results suggest that oral ritodrine may not be effective in preventing premature labour.

Methods of recording fetal movement.

Patient, observer and instrumental methods for recording fetal movements are described. Significant correlations between both patient and observer, and patient and instrumental fetal movement counts, were found. Significant diurnal, daily and weekly variation in fetal movement counts occurred which need to be considered in the interpretation of the frequency of fetal movements as an indication of fetal welfare.

The development of a scale for the assessment of sexual behaviour in Australian women.

The aim of this study was to develop a simple method of assessing female sexual response, suitable for use in clinical investigations. Following a review of interview, physiological and psychological methods, a Scale of Sexual Response was developed. Sexually dysfunctional women and women who stated they had no sexual problems completed the Scale. The results demonstrated that 11 of the 15 subscales had concurrent validity. The 4 subscales relating to auto eroticism did not distinguish between groups. Significant changes were demonstrated in 4 subscales following successful therapy of the sexual dysfunction. The scale was shown to be reliable.
