Transactivation of the human immunodeficiency virus promoter by human herpesvirus 6 (HHV-6) strains GS and Z-29 in primary human T lymphocytes and identification of transactivating HHV-6(GS) gene fragments.

Human herpesvirus 6 (HHV-6) can activate the human immunodeficiency virus (HIV) promoter and accelerate cytopathic effects in HIV-infected human T cells. This study examines the regions of the HIV promoter required for HHV-6 transactivation in a heterogeneous population of primary human T lymphocytes with or without antigenic stimulation. Two different strains of HHV-6, GS and Z29, transactivated the HIV promoter. The GS strain transactivated the promoter in both stimulated and resting T cells, while the Z29 strain increased HIV promoter activity only in stimulated T cells. Three DNA clones containing HHV-6(GS) genomic fragments transactivated the HIV promoter in cotransfected T cells. A 21.4-kb DNA clone, pZVB70, showed the highest transactivating ability, while two other DNA fragments, pZVB10 (6.2 kb) and pZVH14 (8.7 kb), showed lower activity. One of these clones, pZVH14, activated the HIV promoter construct containing a mutation in the NF kappa B site. However, this mutated NF kappa B promoter was not transactivated during HHV-6(GS) infection or after cotransfection with pZVB70 or pZVB10. These data indicate that the NF kappa B sites of the HIV promoter are essential for its transactivation during HHV-6(GS) infection. By increasing HIV promoter activity in primary T lymphocytes, HHV-6 may consequently increase HIV replication, leading to an increase in the cytopathic effect on coinfected human T cells.

The UK Northern region genetic register for familial adenomatous polyposis coli: use of age of onset, congenital hypertrophy of the retinal pigment epithelium, and DNA markers in risk calculations.

A polyposis register has been established in the Northern Region of England. A total of 48 families with 71 living affected subjects has been identified during the first three years of operation, a prevalence of 2.29 x 10(-5). Indirect ophthalmoscopy identifies the majority of gene carriers by showing multiple areas of congenital hypertrophy of the retinal pigment epithelium (CHRPE). The absence of this sign in families limits its value where a relative with CHRPE has not been identified. Combining eye examination with data on age of onset and linked DNA markers is highly effective in carrier exclusion; 38% of 528 first, second, and third degree relatives had their carrier risk reduced to less than 1 in 1000. Even with such assurance many subjects will request continued bowel screening at a reduced frequency. Little interest has been shown in prenatal diagnosis. The principal value of a genetic register with domiciliary nurse visiting is the reduction in early mortality among unrecognised gene carriers.

[Anhidrotic ectodermal dysplasia].

The case of an hyperthermia-induced sudden death in an infant with anhidrotic dysplasia is reported. This disease, easily diagnosed, should now be better known and its early assessment, with thermal monitoring might improve the initial severe prognosis. A genetic counselling might be possible.

Distinctive case. Adult mesenchymal hamartoma of the liver: report of a case with light microscopic, FNA cytology, immunohistochemistry, and ultrastructural studies and review of the literature.

We are reporting a case of a mesenchymal hamartoma of the liver (MHL), found incidentally in a 69-yr-old woman. By light microscopy, the tumor was composed of cellular undifferentiated mesenchymal tissue, with pronounced cystic degeneration, and staining strongly positively for vimentin and negatively for cytokeratin. The cysts contained mucoid material and were lined by modified mesenchymal cells, staining positively for vimentin and negatively for epithelial or endothelial markers. Bile ducts scattered within the tumor and on its periphery stained positively for cytokeratin. No alpha-fetoprotein (AFP) or alpha 1-antitrypsin was identified. Fine needle aspiration (FNA) cytology of the tumor revealed loose clusters of oval cells with benign-appearing nuclei within a mucoid background. As observed by electron microscopy, the tumor cells were oval to spindle shaped with interconnected cytoplasmic processes forming intercellular cystic spaces, appearing to be the precursors of the larger cysts seen by light microscopy. MHL was initially considered as a tumor restricted to childhood; however, a few cases in young adults have since been described. With this case, the possibility of MHL occurrence in advanced age is documented.

[Analgesia-sedation in pediatric resuscitation: indications, modalities].

Recently in pediatric intensive care, sedative and analgesic drugs are being used more frequently, not only for painful and stressful procedures, but also to improve the techniques of intensive care and the patient's comfort. Benzodiazepines are the most common drugs used for sedation, and narcotics for pain.

Pharmacokinetics of midazolam in critically ill neonates.

Midazolam is a water soluble benzodiazepine, with a short elimination half-life in adults and children. An IV bolus of midazolam 0.2 mg.kg-1 was administered to 10 critically ill neonates receiving intensive care who required sedation. The plasma clearance was 6.85 ml.min-1 and the elimination half-life was 6.52 h. Midazolam was well tolerated during and after administration. Because of its short half-life compared to diazepam, midazolam could be used during the neonatal period to produce brief rapid sedation.

[Serious acute hepatocellular insufficiency in children: EEG prognostic features].

Fulminant Hepatic Failure (FHF) in children leads to 60 to 90% of mortality without age influence, depending on the studies. Twenty-two children with this disease had daily EEG, 15 died, seven survived. The neurological electroclinical study evidenced a stereotyped evolution of the EEG anomalies which were not age-related, but correlated to clinical stages of coma and to severe coagulation disorders; 5/22 children with clinical and/or EEG convulsive disorders died. One child exhibited triphasic slow waves and died from paracetamol intoxication. A prospective study of a larger population could help to recognize factors of decision for hepatic transplantation.

The relationship between cardiovascular responses in the laboratory and in the field.

The psychophysiological responses to laboratory stressors are often examined because it is believed that such responses relate to responsiveness in real life situations. This belief has seldom been tested. The changes in heart rate, pulse transit time, and respiration rate produced by a variety of laboratory tasks (active and passive coping and physical exercise) were related to ambulatory measures of heart rate in 32 young men. The field measures were the difference in heart rate between the waking day and when asleep, and estimates of the variability of heart rate during the day, derived from time series analyses. Average changes in heart rate and pulse transit time during specific tasks did not relate consistently to heart rate in the field. However, an active coping index, derived from the ratio of the peak heart rate during an active coping task to the peak during physical exercise related to all the field measures of heart rate responsiveness. This index, which may relate to measures of additional heart rate and heightened sympathetic response to stress, also correlated positively with Trait Anxiety and elevated basal sympathetic arousal, as measured by skin conductance level. Measures of the cardiovascular response to a passive coping task, the cold pressor, and exercise did not relate to heart rate responses in the field. The findings suggest that heightened cardiac responsiveness in real life is exhibited by subjects who show elevated peak responses to active coping stressors specifically.

The viral envelope gene is involved in macrophage tropism of a human immunodeficiency virus type 1 strain isolated from brain tissue.

Human immunodeficiency virus type 1 (HIV-1) strains isolated from the central nervous system (CNS) may represent a subgroup that displays a host cell tropism different from those isolated from peripheral blood and lymph nodes. One CNS-derived isolate, HIV-1SF128A, which can be propagated efficiently in primary macrophage culture but not in any T-cell lines, was molecularly cloned and characterized. Recombinant viruses between HIV-1SF128A and the peripheral blood isolate HIV-1SF2 were generated in order to map the viral gene(s) responsible for the macrophage tropism. The env gene sequences of the two isolates are about 91.1% homologous, with variations scattered mainly in the hypervariable regions of gp120. Recombinant viruses that have acquired the HIV-1SF128A env gene display HIV-1SF128A tropism for macrophages. Furthermore, the gp120 variable domains, V1, V2, V4, and V5, the CD4-binding domain, and the gp41 fusion domain are not directly involved in determining macrophage tropism.

Measuring vitality.

Subjective perceptions of mental and physical energy were measured by the use of visual analogue scales (VAS) over the course of a week in a group of normal volunteers. These variables were found to correlate, highly both with one another and with assessments of vigour and fatigue measured with a standard adjectival format. In addition, high correlations were found with a measure of positive affect. These five variables also displayed characteristic patterns of diurnal variation. Physical and mental energy, vigour and positive affect were highest in the morning, falling progressively and significantly over the day. In contrast, fatigue showed the opposite pattern. Extraversion showed positive correlations with physical and mental energy, vigour and positive affect, and negative correlations with fatigue and negative affect, most being significant (P less than 0.05). Conversely, neuroticism showed a negative correlation with the first four energy variables but was positively related to fatigue and negative affect, most correlations again being significant. Physical and mental energy, vigour and positive affect also showed a positive correlation (P less than 0.01) with a measure of happiness. It is concluded that VAS-derived ratings of physical and mental energy are reliable indicators of self-perceived vigour and fatigue. They are easy to record and show sufficient short-term stability to be used in more extensive studies both of well-being and of its physiological correlates.

[Severe thrombocytopenia induced by heparin in an infant with acute myocarditis].

A severe heparin-induced thrombocytopenia is reported in a child suffering from acute myocarditis. Thrombocytopenia (80.10(9)/l) occurred on day 5 (D5) of heparin therapy which was thus discontinued during 8 hours but reintroduced on the view of a negative platelet aggregation test using standard heparin. On D10, while thrombocytopenia persisted the presence of two intra-cardiac thrombi led to replace standard heparin by low molecular weight heparin (Fraxiparin), associated with vitamin K antagonist (phenindione). On D12, a 3rd intra-cardiac thrombus required immediate discontinuation of Fraxiparin therapy. Platelet aggregation tests performed on D14 were positive in the presence of standard heparin and of low molecular weight heparin, thus demonstrating the existence of thrombocytopenia induced by standard heparin and secondly by low molecular weight heparin. This observation led the authors to discuss the diagnosis and the therapeutic management of heparin-induced thrombocytopenia.

The intracolonic bypass tube for left colon and rectal trauma. The avoidance of a colostomy.

Traumatic perforations of the left colon and rectum are most frequently managed by procedures that include the formation of a colostomy. Primary repair without colostomy is much less commonly employed. We report nine patients with traumatic perforations of the left colon and rectum treated with the intracolonic bypass tube (ICBT) without concomitant colostomy. In all these patients we believe the standard treatment would have included fecal diversion. Four patients sustained blunt trauma and five sustained penetrating trauma. Healing of the colonic anastomosis occurred in all cases, and the ICBTs were passed per rectum between the tenth and nineteenth days postoperatively. On the basis of this study, we conclude that the ICBT has a role in the treatment of selected injuries of the left colon and rectum as a safe means of avoiding a colostomy.
