[Myositis ossificans generalisata or calcinosis interstitialis universalis, a difficult differential diagnosis].

Palpable indurations, calcified nodules, osseous plates and clasps in shoulder, upper arms, thighs and in the back and pelvis muscles are typical signs for several progressive diseases or are associates with some systemic diseases of youth. These changes imply greatly decreased range of motion of all limb joints and increasing retardation of growth and development and often lead to early death. Although often described, the etiology of these diseases is not known. We observed some years ago a particularly serious case, where the connective tissue of almost all the muscles of the skeleton, the subcutis and the ligaments of the limb joints were penetrated by amorphous calcifications and moreover ectopic ossifications were seen in the perirenal adipose tissue and in the interstitial adipose and connective tissue of the striped muscles. Ossifications and calcifications respectively are typical for different diseases, and therefore in our described case it was difficult to decide for a definitive diagnosis.

Hypophosphatasia congenita letalis.

Hypophosphatasia is a serious enzymatic defect, where the serum alkaline phosphatase is considerably diminished or completely absent. The urine or serum contain excessive quantities of phosphoethanolamine.. The illness manifests itself with severe disorder of mineralisation in the skeletal system. We report a child with extremely severe manifestations. There was no bony cranial vault and all the extremities were shortened and thick. The alkaline phosphatase was extremely low and the secretion of phosphoethanolamine and proline was considerably increased. The differential diagnosis and the prenatal diagnostics will be mentioned in this case report.

[Visceral larva migrans. Successful treatment with fenbendazole (author's transl)].

Visceral Larva migrans (VLM) is a parasite, which produces a disease by nematode species. Specific hosts are certain mammals. After penetrating the human organism, these parasites never mature to adult worms but can produce a lot of various symptoms which are dependent on the kind of manifestation. Perilous illnesses are described. The incidence is undoubtedly more than is realized. A new microprecipitation test has proved to be highly specific for the diagnosis. An effective nontoxic treatment in human medicine--exept for mebendazole--was not known until now. We report about the illness of a patient with all typical laboratory findings and clinical signs. The application of Fenbendazole for the first time in humans showed to be highly effective against this ubiquitous illness.

Identification and suppression of secondary structures formed from deoxy-oligonucleotides during electrophoresis in denaturing polyacrylamide-gels.

Nature and stability of secondary structures formed from DNA-fragments during electrophoresis in urea containing polyacrylamide-gels were investigated. Duplices and especially hairpin loops were found to be surprisingly stable. Polyacrylamide-gels containing high concentrations of formamide effectively melt these structures. Such gels will improve nucleotide sequence analysis in regions of dyad symmetry that often cause 'band compressions' under standard electrophoresis conditions.

Electron microscopy shows periodic structure in collagen fibril cross sections.

X-ray diffraction was used to monitor the effects of electron microscope fixation, staining, and embedding procedures on the preservation of the three-dimensional crystalline order in collagen fibrils of rat tail tendon. A procedure is described in which the characteristic 3.8-nm lateral spacing is preserved, with increased contrast, in the diffraction pattern of the embedded fiber. This spacing is correlated with the separation between the tangentially oriented equally spaced lines of density observed in electron microscope ultrathin fibril cross sections of the same material. Optical diffraction of electron micrographs gives an objective measure of the periodicity and suggests that the fibril is composed of concentrically oriented crystalline domains. These observations, when combined with a recent interpretation of the native x-ray diffraction data [Hulmes, D. J. S. & Miller, A. (1979) Nature (London) 282, 878-880] suggest a tentative model for the three-dimensional structure of collagen fibrils.

Distribution of HLA histocompatibility antigens, ABO blood groups and Rh antigens in alcoholic liver disease.

The distribution of 16 antigens of the HLA-A and 15 antigens of the HLA-B series of HLA system, the blood groups ABO, and Rh antigens were studied in 40 alcoholics with cirrhosis, 18 alcoholics without cirrhosis, and in normal control subjects. The group of alcoholics with cirrhosis showed a significantly high frequency of HLA-B13 (corrected P less than 0.01) when compared with normal subjects, while the frequency of HLA-B13 was similar to normal in alcoholics without cirrhosis. On the basis of these findings, its seems that the carriers of HLA-B13 are more susceptible to liver damage caused by alcohol. Both groups of alcoholics and the normal controls had a similar distribution of ABO blood groups and Rh antigens.

[Mode of delivery, postnatal condition and neonatal outcome in preterm infants].

There were under examination 578 preterm infants of two groups--28. up to 31. and 32. up to 36. week of gestation--after birth of vertex as well as breech prevention. Postnatal condition and neonatal outcome were put into relation to the mode of delivery. We compared Apgar-Score (one and five minute value), morbidity on respiratory distress syndrom as well as rate of survival and neonatal mortality in spontaneous delivery with and without episiotomia, with specula delivery, Shute-forceps and vacuum extraction of vertex presentation as well as with breech presentation after vaginal delivery and primary Caesarean section. In respect of the management of the second stage of labour it is our opinion that prophylactic additional measures in preterm delivery of vertex presentation after 32 weeks of gestation are not necessary and that this question should be examined in a larger study of much more cases. But we were able to demonstrate that up to 32. week of gestation well-timed episiotomia of optimal size is necessary. Our good experiences in breech presentation between 31. and 35. week of gestation treated by obligate Caesarean section have to prove true in future.

Further evidence for dissipative energy migration via triplet states in photosynthesis. The protective mechanism of carotenoids in Rhodopseudomonas spheroides chromatophores.

The protection action of carotenoids against irreversible photodestruction was discovered in photosynthetic bacteria by Stanieda and coworkers. In green plant material it was found by Wolff and Witt (1969) Z. Naturforsch, 24b, 1031-1037 and (1972) Proc. 2nd. Int. Congr. Photosynthesis Res. Stresa (Forti, G., Avron, M. and Melandri, A., eds.), Vol. 2, pp. 931-936, Dr. W. Junk, N. V. Publ. The Hague) that the formation of special carotenoid triplet states (via very rapid energy transfer from excited chlorophylls) and their fast radiationless decay in tau1/2 approximately 3 microns is at least one mechanism for the protective action of carotenoids to irreversible photooxidation of the chlorophylls. Hence, it is anticipated that the same mechanism might be realized also in bacteria. The present study gives evidence for such a "triplet valve" to be established also in bacteria. This conclusion was derived from the following observations: 1. The light-induced difference spectrum shows a bleaching of a carotenoid at three characteristic wavelength between 400 and 500 nm. A positive peak around 533 nm indicates the formation of a carotenoid triplet state. 2. The absorption changes can be induced by red light which excites only bacteriochlorophyll. This indicates an energy transfer from bacteriochlorophyll to carotenoids. 3. The light-induced carotenoid triplets decay radiationless in 3 microns in air-saturated aqueous suspensions of the chromatophores. 4. The carotenoid triplet formation occurs only at actinic flash intensities where the photosynthesis becomes saturated. 5. Addition of dithionite, which blocks photosynthesis, markedly increases the extent of carotenoid triplet formation. The different types of exciton migration within the photosynthetic unit are discussed, especially the routes leading to the dissipation of excess excitation energy.
