[SPECT (201T1) of the myocardium after experimental infarction].

On the basis of an experimental model the extent was studied to which even small intramural myocardial ischemia can be demonstrated by SPECT. For this purpose ligature of the Ramus interventricularis anterior (n = 10) or of the Ramus circumflexus (n = 2) of the left coronary artery was performed in 12 dogs to produce myocardial lesions of the anterior and posterior wall. Transmission computed tomography (TCAT) served as a control method in 10 dogs showing that 6 lesions were transmural and 4 intramural. All infarctions could be detected by SPECT with a rotating gamma camera (TCAT: 9/10). TCAT after contrast media enhancement with its morphological advantages (high resolution) should, however, not be used in the acute phase of myocardial infarction because of its hemodynamic side effects. Thus, SPECT with 201T1 remains the method of choice even for the proof of minor intramural (acute) ischemic lesions.

[Listeriosis during pregnancy (author's transl)].

Eight cases of materno-fetal listeriosis were discovered at the University Women's Hospital of Basel from May 1977 until June 1980. This represent an incidence of 0,15% of all births. This infectious disease has often a fatal course for the unborn child, therefore it is important to know the clinical manifestations occurring with it. Listeriosis during pregnancy has a typical-two-stage course: During the first phase we see commonly a flu-like illness abating rapidly, about two weeks later fever starts again and premature contractions ensue, but no therapy is successful in controlling the fever and the premature labour. The usual fate for the unborn child is stillbirth or premature delivery with subsequent neonatal death due to prematurity, RDS, sepsis and meningitis. The low fetal and neonatal survival rate can be improved by two relatively simple measures: 1) a high index of suspicion with early diagnosis, 2) an early treatment with ampicillin either in the antepartal or neonatal stage. We review the epidemiology, the bacteriology, the serology and the histo-pathology of this relatively rare but important disease during pregnancy.

[On the treatment of carcinoma of the thyroid (author's transl)].

Treatment of carcinoma of the thyroid includes, in addition to possible total surgical removal of thyroid and tumour tissue, obligatory radio-iodine treatment as well as administration of thyroid hormone in high dosage. Patients with anaplastic carcinoma as well as those with differentiated carcinoma which has broken into vessels or capsule or metastasized, are additionally given percutaneous radiation treatment. Cumulative survival rate for papillary thyroid carcinoma, treated according to this schema, was for women 95% after three years, 83% after five and 53% after ten years. Corresponding survival rates in men were 93%, 78% and 20% respectively. Rates for follicular thyroid carcinoma in women were 87% at three years, 75% at five and 50% at ten years. In men corresponding figures were 94, 77 and 50%. Dividing patients with differentiated thyroid carcinoma (210) into various risk groups, those with papillary and follicular carcinoma manifesting at an age under 45 years had a better prognosis than that after 45 years. Worst prognosis was with anaplastic thyroid carcinoma, when survival rates at one, three and five years were 41, 28 and 18%, respectively.

[Induction of abortion during the second and third trimester of pregnancy with sulprostone (author's transl)].

Report on 114 terminations of pregnancy during the second and third trimester of pregnancy with a new prostaglandin E2 derivative "Sulprostone". In 84 cases legal therapeutic abortions were induced. In 30 women an abnormal pregnancy was terminated. "Sulprostone: is a prostaglandin with selective activity in the uterus. It can be administered parenterally or locally. 48 women received intravenous sulprostone, 35 patients had extraamniotic injections and 31 patients had extra-amniotic injections following an intramuscular injection for priming of the cervix the night before. In 103 women the pregnancy was terminated within the first 24 hours after the administration of sulprostone (90.3%). 8 patients (7.0 %) had a cervical dilatation over 2 cm. In 3 women no effect was noted. In 1 patient the prostaglandin application was stopped because of severe vomiting. The mean administration abortion interval was 12 hours and 54 minutes following extra amniotic application and 10 hours and 30 minutes with extra amniotic application and following intramuscular priming. In 5 patients the abortion was completed after the priming administration. More than 90% of the patient's required analgesia. There were 32.4% side effects of mild character. The results and the methods are described and compared to other methods.

[Animal studies of quantitative renal occlusion scintigraphy (author's transl)].

Radio-active plastic microspheres with an average diameter of 15 mu were used in eight dogs to measure renal perfusion and the results compared with time-activity curves before and after constriction of the left renal artery. It was shown that the values obtained for renal blood flow by functional scintigraphy are not accurate, particularly for moderate or low blood flow. The reason for this is primarily the distortion of the intravenously injected radio-active bolus and the consequent deviation from the so-called delta function.

Determination of liver and spleen perfusion by quantitative sequential scintigraphy: results in normal subjects and in patients with portal hypertension.

Quantitative sequential hepatosplenic scintigraphy was performed to determine the arterial and portal components of the total liver circulation in 135 patients (no liver disease in 20, liver cirrhosis and portal ;hypertension in 115). Portal circulation in healthy patients is calculated to be 70.4 +/- 6.2% of the total liver blood flow, whereas patients with portal hypertension showed a clear reduction of portal perfusion to 20.2 +/- 10.9%. Thirteen of 20 patients having portosystemic shunt surgery showed no portal perfusion. This new, noninvasive diagnostic technique yields vital information particularly useful in ;the surgical evaluation of portal hypertension. Other indications are also discussed.

[Single-photon emission computed tomography (SPECT) of the liver with a rotating gamma camera system (author's transl)].

In a total of 202 patients conventional scintigraphy as well as emission computed tomography (SPECT) of the liver were performed. In 123 patients focal lesions could be excluded whereas in 30 cases the presence of liver tumors was verified. In 5 of the respective cases correct diagnosis was only possible by SPECT, in 9 further cases SPECT improved the diagnostic accuracy (topography, size and number of defects). In patients with normal findings of the liver, conventional scintigraphy yielded false-positive findings in 9 cases and SPECT in 7 cases. By combination of both methods the false-positive diagnoses could be reduced to 5. 48 patients had cirrhosis of the liver: in all of them conventional camera scintigraphy was superior to SPECT because of reduced activity accumulation in the liver (insufficient statistics).

[The scintigraphic features of cirrhosis of the liver (author's transl)].

A retrospective study of 101 patients with histologically confirmed cirrhosis of the liver and portal hypertension was carried out in order to evaluate the accepted scintigraphic criteria. In only 20% were all the essential criteria present. The absence of generally accepted important scintigraphic signs does not exclude the diagnosis of cirrhosis of the liver. Specificity of liver scintigraphy in cirrhosis of the liver is fairly low, but sensitivity of the method is almost 100%.

[Data acquisition techniques in functional heart scintigraphy. Advantages and disadvantages, Diagnostic significance (author's transl)].

Although equilibrium gated blood pool scintigraphy has become a topic of utmost interest during the last few years, there are still considerable variations in the sequential scintigraphic methods used by different groups. In view of the fact that there seems to be a lack of detailed knowledge of the various available techniques of data acquisition, the respective methods are collated in order to consider their advantages and disadvantages. From this, the most commendable technique for the scintigraphic evaluation of left ventricular function is derived.

[Changes in thyroid function after application of iodinated contrast media to normal subjects (author's transl)].

In 39 patients without thyroid disorder the influence of radiographic contrast media (Conray 60, Biligram) on the thyroid function was studied. 6 weeks after the application of the iodine compounds a significant increase of T3 in blood serum could be established being accompanied by decreased TSH level and reduced response of TSH rpoduction to TRH injection. These findings showed to be even more marked 6 months later. It is assumed that the observed hormonal changes may possibly be due to a persistent Wolff-Chaikoff block induced by diffusion of free iodide and subsequent shifting of the intrathyroidal MIT/DIT ratio, which is finally followed by an increase of T3-levels in serum.

[Quantitative gated nuclear cardiography for noninvasive evaluation of left ventricular function].

In 43 patients with angiographically proven coronary heart disease and in 24 patients without heart disease, gated blood pool scintigraphy was done under resting conditions with 15 mCi 99mTcHSA in LAO projection. Scintigraphic data were collected in list mode by means of a gamma camera connected to a computer system (Siemens 330). A cumulative scintigram sequence with a time resolution of 100 frames/s was constructed using several hundred heart cycles of equal duration. After background correction volume curves of the left ventricle were obtained and the following parameters were calculated: EF, dV/dt max,dV/dt min. In all patients with coronary heart disease, these parameters were reduced depending on the severity of the stenotic lesion. The changes of dV/dt min and to a greater extend of dV/dt max were more pronounced than those of the EF. These results are in keeping with invasively obtained data which show that a decrease of myocardial compliance is one of the earliest manifestations of CHD. Quantitative gated nuclear cardiography allows, therefore, noninvasive evaluation of left ventricular function even under resting conditions.

[Evaluation of hepato-biliary functional scintigraphy with 99mTc diethyle-IDA. Findings in normal livers (author's transl)].

Hepato-biliary functional scintigraphy was performed on 51 patients with normal livers after in the injection of 99mTc diethyle-IDA. The results were analysed according the morphological criteria (demonstration of the bile duct, intra-hepatic ducts, gall bladder and renal tract) and functional criteria (T max, half-time biliary excretion values, development of activity in the bile duct, in the gall bladder and in the gut). Demonstration of the bile duct and gall bladder was successful in all cases if the examination was carried out after an appropriate interval following food. 84.3% of patients showed a gall bladder excretary pattern of the tracer and 15.7% a gall bladder storage pattern. Correspondingly, bile appeared in the duodenum at 16.5 +/- 6.5 minutes (excretion) or 79.3 +/- 49.5 minutes (storage). The left hepatic duct was shown in 58.8%, the right hepatic duct in only 58.8%. Sixty minutes after the injection no activity could be determined in the bile ducts beyond the liver parenchyma. The gall bladder could be identified after 26.6 +/- 24.4 minutes, occasionally it was much later, up to 110 minutes. The liver function curve showed a Tmax level of 6.8 +/- 2.7 minutes and half value time of 15.8 +/- 6.7 minutes. At 5 minutes after injection the renal pelvis could be identified in 19.6%, but in no case was this still present beyond 15 minutes. Duodeno-gastric reflux was not demonstrated.
