Oral solutions for infantile gastroenteritis--variations in composition.

Four different carbohydrate electrolyte solutions were provided for children under 18 months with acute gastroenteritis treated as outpatients. Osmolality and sodium content were measured in samples of solutions as given by the parents. All types of feed were made up with marked inaccuracy. Osmolality was sometimes unacceptably high in solutions containing glucose, while the highest osmolality for sucrose solutions hardly exceeded the correct value for glucose solutions. Most parents could use a sachet with reasonable accuracy although there were still wide extremes of errors. The ideal preparation for use in developed countries may be a sachet containing sucrose and electrolyte, particularly if such sachets could be made generally available and not just for use in hospitals and clinics.

A positive inotropic response of guinea pig isolated atria to histamine not mediated via H1 or H2 receptors.

The positive chronotropic responses of guinea pig isolated right atria to histamine were antagonized by metiamide (pA2, 5.95) thus confirming their H2 receptor classification. The positive inotropic responses of paced left atria were antagonized to some extent by mepyramine to give a pA2 value of 7.87, indicating the involvement of H1 receptors. A limit to the shift of cumulative dose-response curves for the inotropic response suggested an H1 receptor resistant component. The inotropic response to sequentially administered histamine was biphasic. On lowering the temperature to 25 degrees C, the two components became more demarcated and separated by a negative phase. Only the primary positive component and the negative component were antagonized by mepyramine. At 38 degrees C, the response was similarly converted to a monophasic one. The residual responses were resistant to metiamide and propranolol antagonism and therefore not mediated via H1, H2, or beta-adrenergic receptors.

Clinical cerebral blood flow measurement with inhaled xenon and CT.

Cerebral blood flow measurement has not yet been attempted with xenon and CT in patients. Xenon washout was studied in three patients receiving subanesthetic concentrations of xenon. Cerebral enhancement after 30%--50% inhaled xenon was demonstrated without serious side effects. Clinical cerebral blood flow measurements with xenon and CT may be feasible.

Effect of general practitioners' advice against smoking.

During four weeks all 2138 cigarette smokers attending the surgeries of 28 general practitioners (GPs) in five group practices in London were allocated to one of four groups: group 1 comprised non-intervention controls; group 2 comprised questionnaire-only controls; group 3 were advised by their GP to stop smoking; and group 4 were advised to stop smoking, given a leaflet to help them, and warned that they would be followed-up. Adequate data for follow-up were obtained from 1884 patients (88%) at one month and 1567 (73%) at one year. Changes in motivation and intention to stop smoking were evident immediately after advice was given. Of the people who stopped smoking, most did so because of the advice. This was achieved by motivating more people to try to stop smoking rather than increasing the success rate among those who did try. The effect was strongest during the first month but still evident over the next three months and was enhanced by the leaflet and warning about follow-up. An additional effect over the longer term was a lower relapse rate among those who stopped, but this was not enhanced by the leaflet and warning about follow-up. The proportions who stopped smoking during the first month and were still not smoking one year later were 0.3%, 1.6%, 3.3%, and 5.1% in the four groups respectively (P <0.001).The results suggest that any GP who adopts this simple routine could expect about 25 long-term successes yearly. If all GPs in the UK participated the yield would exceed half a million ex-smokers a year. This target could not be matched by increasing the present 50 or so special withdrawal clinics to 10 000.

The role of plasmapheresis in renal transplantation.

Plasmapheresis has been studied in ten patients who received cadaveric renal allografts. In six patients (group A), plasma was carried out on the first post-operative day and then on alternate days until six treatments had been completed. No beneficial effect was observed but treatment was well tolerated and there were no complications. One patient retains a functioning graft. Plasma exchange was carried out on six consecutive days in five patients (group B) who were actively rejecting their grafts and in whom conventional methods of treatment had failed. All five patients showed improvement in graft function. Rejection was reversed in four patients and was contained in one patient. Subsequently rapid recrudescence of graft rejection occurred in one patient, extended graft survival was seen in two patients, and the other two patients have retained life supporting grafts. Plasmapheresis may have a part to play in the treatment of renal allograft rejection and further evaluation of this treatment is indicated.

Late replicating X chromosomes in human triploidy.

The status of X-chromosome replication was studied in twenty-seven 69,XXY and nine 69,XXX human triploids in which the parental origin of the additional haploid set was known from the study of chromosome heteromorphisms. Among the 69,XXY triploids, fourteen had no late replicating X, two had one late replicating X in all cells examined, and eleven had two populations of cells, one with late replicating X chromosome, and one without any. Among the 69,XXX triploids, four had a single late replicating X, and five had two populations of cells, one with one late replicating X, and one with two late replicating X chromosomes. There was no correlation between the parental origin of the triploidy and the type of X-chromosome inactivation. However the number of late replicating X chromosomes was significantly lower in cultures grown from fetal tissue when compared with those grown from extra-embryonic tissue. In cultures derived from extra-embryonic tissue there was a significant correlation between the gestational age of the sample and the proportion of late replicating X chromosomes. The older the specimen, the greater the number of late replicating X chromosomes.

Computed tomography in the follow-up of medulloblastomas and ependymomas.

The course of 36 patients with medulloblastoma and ependymoma was evaluated prospectively by clinical examination, radionuclide (RN) studies and computed tomography (CT). Seventeen of the 36 patients (47%) had tumor recurrence. Twelve (41%) of the 29 patients with medulloblastoma had recurrent tumors of which 7 of 12 (58%) were at the primary site and 2 of 12 (17%) were within the ventricles while 10 of 12 (83%) were in the subarachnoid space. Five of the 7 patients with ependymoma had recurrent tumors. In 4 of the 5 patients tumor recurred at the primary site while subarachnoid seeding occurred in 2 of 5 patients (40%) and intraventricular metastases were found in 4 of 5 patients (80%). Progressive ventricular enlargement often accompanied subarachnoid seeding, presumably secondary to obstruction of cerebrospinal fluid (CSF) flow in the subarachnoid pathways. CT and RN scans were frequently complementary in detecting tumor recurrence.

Importance of imagery in maintenance of feedback-assisted relaxation over extinction trials.

Two groups of 16 female subjects undergoing frontalis EMG feedback training were identified according to whether they used imagery or passive concentration to achieve relaxation. The ability of these 2 groups to maintain relaxation during extinction sessions 2 and 7 days after termination of training was then investigated. Although both groups were equally successful in relaxing the frontalis during training, the group using imagery was significantly more successful in achieving relaxation during extinction sessions.
