We describe 10 new cases of bacteremia due to Stomatococcus mucilaginosus and review eight other cases that have been described in the literature. The most common clinical presentations were endocarditis, catheter-related infection, and septicemia. Commonly associated risk factors were intravenous drug abuse, cardiac valve disease, the presence of foreign bodies (especially indwelling vascular catheters), and immunocompromised states. S. mucilaginosus bacteremia is readily treatable with antibiotics. This organism is of low virulence, but appears to be an emerging pathogen. Infection due to S. mucilaginosus is likely to be underreported because the organism may be easily misidentified and information on it is not included in the databases of many automated microbiologic identification systems.
AIMS: The aim of the present study was to determine if urinary excretion of type I collagen N-terminal telopeptides (UrNTx) and deoxypyridinoline (UrDPD) and serum levels of type I collagen C-terminal telopeptides (SeCTx) differed in patients with rheumatoid arthritis (RA) compared with populations matched for age and gender with and without osteoarthritis (OA). The correlation of markers of bone turnover with disease activity in patients with RA or radiographic severity in patients with OA was also examined. METHODS: Patients with RA aged >50 years (men) and >60 years (women) were identified from computer databases at two tertiary referral centres for rheumatology. Strict exclusion criteria were applied to avoid the effects of factors known to influence markers of bone turnover. Patients with RA and OA were matched for age and sex with a control population free of known arthritic disease and a population with OA. Bone markers were assayed in serum and urine. Urine markers were measured on three consecutive days and mean values used to minimize day-to-day variability of these analytes. RESULTS: The level of UrNTx was elevated in patients with RA compared with normal controls and patients with OA. UrNTx and UrDPD correlated with markers of disease activity in patients with RA (erythrocyte -sedimentation rate and C-reactive protein), but not with -clinical signs of inflammation (swollen and tender joint counts). Patients with OA failed to show any correlation between markers of bone turnover and radiographic severity. CONCLUSIONS: These data support a role for the use of UrNTx and UrDPD in further studies of the patho-physiology of RA and in longitudinal studies designed to modify the course of clinical disease.
The Ardran-Crooks kVp test cassette is widely used in diagnostic radiology to provide a rapid, simple, noninvasive measurement of x-ray tube potential. A modified version of this cassette called the Wisconsin kVp Test Cassette was introduced commercially in the U. S. in 1972. Since then, the method of calibration of these cassettes has changed significantly. Wisconsin kVp Test Cassettes calibrated by the manufacturer prior to August 1982 may yield underestimates of kVp measurements, particularly when using the 90-110 and 110-130 kVp regions with single-phase units. In August 1982 significant improvements in the calibration methods were implemented. The resultant change in calibration is demonstrated by data from the Centers for Radiological Physics. Present calibration methods are believed to be accurate within the greater of +/- 2 kVp or 2% of actual peak tube potential. Proper use of the cassette is necessary to achieve this level of accuracy.
A number of biological responses to angiotensin II have been demonstrated to be modulated acutely by cations, but the exact mechanism has not been elucidated. We have utilized a radioreceptor assay for angiotensin II to determine whether this acute regulatory mechanism could be related to a change in either number or affinity of angiotensin II binding to receptors. Three target tissues were used (adrenal glomerulosa, glomeruli, and uterine smooth muscle). Both mono- and divalent cations influence the kinetics of angiotensin II binding in a similar manner in all tissues. Divalent cations increase the number of receptor sites to a greater extent than did monovalent cations, while monovalent cations changed binding affinity to a greater extent than did divalent cations. These studies demonstrate that both the number and affinity to a greater extent than did monovalent cations, while monovalent cations changed binding affinity to a greater extent than did divalent cations. These studies demonstrate that both the number and affinity of angiotensin receptors can be rapidly modulated by a variety of cations.
