Adjuvant cytotoxic liver perfusion for colorectal cancer.

Colorectal liver metastases develop by malignant cells entering the portal venous circulation. A randomized prospective clinical trial was commenced in 1975 to assess the value of adjuvant umbilical vein cytotoxic perfusion (with 5-fluorouracil) following colorectal resection. One hundred and fifty-four patients without macroscopic liver secondaries have so far entered the trial. The mean age, sex, site and stage of the disease were similar in the control and perfusion groups and there was no statistically significant difference in postoperative complications or hospital stay (17.1 +/- 7.9 days control, 15.8 +/- 7.4 days perfusion group). So far, 23 deaths have occurred in the control group (20 due to recurrent disease) and 7 in the perfusion group (5 due to recurrent disease). Liver metastases were present in 13 control patients and 2 perfusion patients. These results show an encouraging trend and suggest that adjuvant cytotoxic liver perfusion may reduce the development of colorectal liver metastases and hence improve the subsequent prognosis.

The storage of hard-packed red blood cells in citrate-phosphate-dextrose (CPD) and CPD-adenine (CPDA-1).

The preservation of red cells "hard packed" to a hematocrit of over 80% from blood collected in citrate-phosphate-dextrose (CPD) or CPD-adenine (CPDA-1) has been investigated. After 21 days of storage, cells that had been collected in CPD solution had consumed most or all of the available glucose and manifested markedly impaired viability after reinfusion into the normal donor. In contrast, red cells prepared from blood collected in CPDA-1, a medium containing supplementary adenine and an increased amount of glucose, maintained higher glucose and adenosine triphosphate levels and, in most instances, manifested satisfactory posttransfusion viability. We emphasize that in addition to providing longer shelf life of stored blood, CPDA-1 provides a better hard-packed red cell concentrate for transfusion at 21 days.

Therapy of scleritis with cytotoxic agents.

Four patients with severe anterior scleritis that had not responded to conventional therapy were treated with cytotoxic immunosuppressive alkylating agents alone (one case) or in combination with prednisone (three cases). Two of the patients had Wegener's granulomatosis, one pyoderma gangrenosum, and one rheumatoid vasculitis. All patients responded well to this therapy with a decrease in inflammation and a healing of the sclera. Other systemic manifestations of the underlying diseases often responded well also.

A simple rapid radiometric assay for pyrimidine-5'-nucleotidase.

Pyrimidine-5'-nucleotidase deficiency appears to be an important cause of hemolytic anemia associated with basophilic stippling of the red cells. A new radiometric method for the assay of this enzyme has been developed. In this technique, 14C-CMP serves as substrate. The CMP which is not dephosphorylated to cytidine is bound to the barium sulfate precipitate which forms in the deproteinization process. The cytidine remains in solution and is counted. The method is simple and reproducible and can be carried out on large numbers of samples. Two patients with pyrimidine-5'-nucleotidase deficiency have been detected by means of this technique.

Brain metastasis from prostatic carcinoma.

Between 1959 and 1971 there were 91 patients with clinically diagnosed prostatic carcinoma who were autopsied at Roswell Park Memorial Institute. In four of these 91 (4.4%) intracerebral metastasis were found at autopsy, but only in one of these four was the diagnosis arrived at pre-mortem. This report describes the diagnosis and management of intracerebral metastasis from prostate carcinoma. It appears, on the basis of our initial experience, that the clinical diagnosis of this entity deserves more frequent consideration.

The value of palliative spinal surgery in metastatic urogenital tumors.

There were 22 patients with vertebral metastases and impending neurologic damage from prostatic, vesical and renal cell carcinoma treated by decompressive laminectomy combined with local radiotherapy and chemotherapy. The neurological recovery was complete in the prostatic carcinoma patients (10 of 10), partial in the vesical carcinoma patients (2 of 5) and unsatisfactory in the renal cell carcinoma patients (1 of 7).

The removal of leukocytes and platelets from whole blood.

Various methods for the removal of leukocytes from whole blood have been compared and a new technique has been devised. This procedure consists of passing whole blood through a bed composed of microcrystalline cellulose and alpha-cellulose. The method is rapid, reliable, removes over 99.75 per cent of the leukocytes from blood, and does not seem selectively to retain reticulocytes or to release a significant proportion of leukocyte enzymes. Most of the platelets are also removed from anticoagulated blood, and platelet-free red cells can be obtained by passing defibrinated blood over the microcrystalline cellulose-alpha-cellulose bed.

Immunofluorescent studies of ocular cicatricial pemphigoid.

Using direct and indirect immunofluorescent techniques, we studied ten patients with cicatricial pemphigoid (chronic cicatricial conjunctivitis). Serologic and histologic evaluation of 13 conjunctival, 12 mucous membrane, and 11 skin specimens from the ten patients showed that three had immunoglobulin deposition at the basement membrane zone of the conjunctiva. One patient had immunoglobulin deposited in the affected skin and oral mucosa. Two patients had circulating basement membrane zone antibodies that bound to their own tissue but not to guinea pig lip or human skin.
