[Therapeutic results using combined antibiotic therapy in chronic carriers].

From 1969 to January 1976 82 chronic carriers underwent a sanitation treatment. This treatment consists in an antibiotic combination treatment with ampicillin and canamycin during 24 to 18 days, in which case possibly simultaneously the cholecystectomy is striven for. In chronic carriers of S. typhi in 37 cases the combined antibiotic-operative treatment led to the permanent success. In chronic carriers of S. paratyphi B 21 of 22 cases were present. In patients with condition after cholecystectomy in every case one failure was present. In purely conservative approach the coefficient of success was 50 and 78%.

[Release of antibiotics from bone cement and penetration to infected cortical bone (author's transl)].

Addition of antibiotics to bone cement is used more and more in orthopedics and traumatology. Indications are discussed: 1. infection prophylaxis in replacement arthroplasty, 2. therapy and recidive prophylaxis in infected arthroplasty, 3. therapy of chronic osteomyelitis. Our own experiments show release of antibiotics from bone cement Palacos and penetration into cortical bone after experimental osteomyelitis. Carbenicillin and Cephalotin show high level of concentration for 2 weeks, after 3 weeks there is no concentration evident. Lincomycin and Gentamycin in the first days show high tissue levels, after decrease and increase again after second week. Gentamycin was examined during 10 months showing high levels after this time.

Design and biomechanical evaluation of a cementable endosteal blade implant.

A cementable endosteal blade implant has been developed and evaluated. Inherent in the design are the following factors: minimization of stress concentrators, ease of implantation, and high resistance to loosening. Other potential advantages of this design as compared with conventional endosteal blade implants include reduced hazards of postoperative infection and reduced likelihood of metallic corrosion. Six conventional endosteal implants and six cementable implants were installed in steer mandibles using standard dental surgical techniques and Simplex-brand methyl methacrylate bone cement. They were loaded in uniaxial compresstion at a loading rate of 0.0122 in./min. the stiffness (S), deformation at 900-lb load (D900), proportional limil (PL), and load at 0.04 in. deformation (L0.04) were calculated for each test. The cementable design showed a twofold increase in stiffness, only 17% of the deformation at 900lb, more than twice the proportional limit, and at least twice the load at 0.04 in. deformation when compared with similar values for the conventional endosteal implants. This study reveals that, in addition to being unstable in bone, conventional endosteal implants are also unstable when used with acrylic bone cement. The new design should eliminate most of the problems associated with endosteal blade implantation.

Different renal oxidation rates of intramolecular carbon atoms in lactate and pyruvate.

In order to get more detailed information on the different renal modes of utilization of individual intramolecular carbon atoms, formation rates of 14CO2 and metabolic products from single-carbon labelled exogenous substrates have been studied in isolated tubular fragments from rat kidney cortex. The results showed that carboxyl groups from acetate, lactate, and pyruvate were converted to CO2 at a much higher rate than substrate-2-C or 3-C. This effect was not due to a physico-chemical splitting. The corresponding C-2 or C-3-atoms were incorporated into newly formed glucose to a higher extent than carboxyl groups. Apparently, the maximum chance to be converted to CO2 for an intramolecular carbon of an exogenous substrate lies at the stereospecific site of action of decarboxylating enzymes. With rising distance from this point the probability to be built into glucose (i.e. to escape the oxidative pathway) increases. Due to the different share of the intramolecular substrate carbon (in the total CO2 formation) without the use of single-carbon labelled substrates one cannot calculate the oxygen requirements for an observed 14CO2 formation.

Contribution of long chain fatty acids to the energy supply of the rat kidney cortex.

Tubular fragments from rat kidney cortex were isolated by collagenase and suspended in an incubation medium containing a combination of several renal substrates. Substrate concentrations were in the physiological range. O2 uptake, total CO2 production, and the 14CO2 production from U-14C-labeled palmitate and oleate were measured. During the first minutes of incubation the CO2 production from palmitate and oleate was 10.5% or 6.3%, respectively, of the total CO3 produced. The RQ was 0.897. A subsequent decrease of the total CO2 production at a constant uptake of oxygen indicated a rising contribution of fatty acids to the fuel of respiration. The renal preference for substrates other than longchain fatty acids is discussed.

Acute changes of the Na-K-ATPase-activity in plasmamembranes of the isolated, cell-free perfused rat kidney.

The Na-K-ATPase-activity of the isolated, cell-free perfused rat kidney was investigated. Na-load and perfusion rate could be varied independently. The ATPase-assay was carried out on plasmamembranes (700-1200 g fraction) isolated and purified after 60 min perfusion. Both the specific Na-K-ATPase-activity and the fractional Na-reabsorption showed an indirect correlation with the Na-load as well as with the perfusion rate. The Na-K-ATPase-activity of plasmamembranes obtained from the isolated kidney at physiological Na-load fits well to control values prepared from unperfused control kidneys. The present data are in accord with the results of earlier micropuncture experiments, where acute saline loading diminished the rate of Na-absorption.

Synovial chondromatosis and instability of the proximal tibiofibular joint.

A patient with synovial chondromatosis involving both posterior compartments of the right knee and proximal tibiofibular joint had continued pain and weakness after partial synovectomy. The "Helfet Test" for proximal tibiofibular joint instability was positive; the fibular head was prominent and tender. Following excision of the proximal fibula, the patient was symptom free. This case demonstrates that it is essential to evaluate all joints involved with a disease process prior to surgical intervention.

Substrate oxidation and inhibition of sodium transport in the isolated perfused rat kidney.

The relationship between the oxidative substrate-turnover-rate and the active Na+-transport was investigated. Our special interest was directed to the possible dependence of the oxidative turnover-rate of one selected substrate from the mode of reduction of the Na+-transport. The following methods were used: 1. Blocking the active Na+-transport by the addition of ouabain to the perfusion medium (0, 5 mg/ml); 2. Reduction of the sodium concentration in the perfusion medium to 100mM substituting NaCl by Tris/HCl; 3. Reduction of the perfusion pressure resulting in a low GFR and a low tubular Na+-load. The perfusion medium contained the following substrates in normal plasma concentrations: Na-palmitate, Na-oleate, Na-propionate, Na-acetate, D-glucose, Na-pyruvate, Na-L-lactate and Na-glutamate. The substrate in question was uniformly 14C-labelled; its amount oxidized per unit time was calculated on the basis of the 14CO2 formed. The following substrates were studied; Na-acetate, Na-pyruvate and Na-L-lactate. There were no significant differences between the results of the three methods described.
