[First grade students in former East and West Berlin: socio-emotional adjustment and relation to attachment patterns of the children].

Adaptation to school of two samples of children (45 from East and 43 from West Berlin) grown up in two different political systems until the "Mauerfall" was studied over the first two school years using information from teachers in school and kindergarten, parents, and the children themselves. Cluster analysis revealed 3 patterns of adaptation behavior: the first subgroup (61% of the sample) showed no problems at all, for a second subgroup (22%) teachers reported more adaptation difficulties, whereas the third subgroup (17%) was rated by their mothers as distractible, hard to motivate and less sociable. Sex differences turned out to be more significant than political-system differences. Adjustment patterns to school were also related to the child's quality of attachment, to the reported number of stressful life events happened to the family, as well as to mother's and teacher's existential anxiety.

Removal of lipid A and Pseudomonas aeruginosa endotoxin from dialysis fluids by high-flux polysulfone ultrafilter (dialyzer).

Ultrafilters (dialysis membranes) are generally considered to be impermeable to bacterial endotoxins (lipopolysaccharides) contaminating dialysates used for hemodialysis therapy, due to the self-aggregating properties of its molecules in aqueous media. The aim of the present investigation was to monitor the efficiency of high-flux polysulfone ultrafilters in removing lipid A and lipopolysaccharide (LPS) from dialysis fluids. To evaluate the safety of high-flux polysulfone membranes, an in-vitro circulation system (measuring lipopolysaccharide and lipid A penetrate from the ultrafilter to the other side and vice-versa), was utilized. Peritoneal dialysis solution was spiked with various concentration of Pseudomonas aeruginosa lipopolysaccharide (LPS) as well as lipid A diphosphoryl (E. coli F-583). Endotoxin and lipid A concentrations were detected by a chromogenic limulus amoebocyte lysate (LAL) assay. This investigation indicates that polysulfone ultrafiltration represents an efficient system to obtain an endotoxin and lipid A free dialysate when contaminated peritoneal solution was circulated through the dialysis membrane.

Effect of a clinical case manager/clinical nurse specialist on patients hospitalized with congestive heart failure.

The purpose of this study was to evaluate the effect of case management by a Clinical Case Manager/Clinical Nurse Specialist (CCM/CNS) on hospitalized length of stay and hospital charge throughout a 12-month period for patients with congestive heart failure. A total of 491 patients were discharged during 1997 with a diagnosis-related group code of 127. Of this number, 88 were case managed by a CCM/CNS. The remaining 403 received the usual management of their care. The group who were case managed by the CCM/CNS demonstrated significantly shorter length of stay (t = 5.40, P < 0.00) and lower hospital charges (t = 4.26, P < 0.00) than the patients with congestive heart failure who were not case managed. Secondary analysis indicated a significant interaction between case management and involvement of a cardiologist in the care of the patient. Patients whose care involved a cardiologist without case management by a CCM/CNS demonstrated significantly greater (alpha = 0.01) length of stay and hospital charges than patients who were case managed by a CCM/CNS or patients whose care did not involve a cardiologist.
