Loss of bone mass in patients with Klinefelter's syndrome despite sufficient testosterone replacement.

To determine whether testosterone replacement therapy reverses the detrimental effects of hypogonadism on bone density, we measured the total body, lumbar spine and proximal femur bone mineral density (BMD) by dual-energy X-ray absorptiometry in 14 patients with Klinefelter's syndrome on long-term testosterone replacement therapy and compared the results with 14 age- and sex-matched normal controls. Seven of the patients were receiving oral testosterone undecanoate thrice daily (240 mg/day) and the others were having intramuscular testosterone enanthate injections once every 3 weeks (250 mg/injection). Their serum testosterone levels were maintained within the normal limits (10-40 nmol/l). We showed that patients on testosterone replacement had decreased amount of bone density in the left femoral neck when compared with the controls (p < 0.01). Similar decreases were also observed in the left Ward's triangle (p < 0.01) and in the left trochanter (p < 0.05). There were no significant differences in the total body and the lumbar spine measurements in these two groups of subjects. No correlation was found between the BMD values of femur and the duration of testosterone treatment in the patients with Klinefelter's syndrome. The type of testosterone treatment was also not associated with significant differences in BMD. In conclusion, sufficient testosterone replacement with currently available methods does not reverse the decrease in bone mass associated with hypogonadism in patients with Klinefelter's syndrome.

Synergism between the antinociceptive effects of intrathecal midazolam and fentanyl on both A delta and C somatosympathetic reflexes.

The interaction between the antinociceptive effects of fentanyl and midazolam, administered intrathecally (i.t.), was examined in dogs. Midazolam 1 mg (i.t) depressed the A delta and C fibre mediated somatosympathetic reflexes to 68.3 and 85.2% of control values and then reduced the subsequent doses of fentanyl (i.t.) required to abolish these reflexes by 50%. After midazolam (1 mg, i.t.) the ED values for fentanyl were markedly less than the theoretical predicted additive values. This indicates synergism between the effects of fentanyl and midazolam.

Relative binding free energy calculations of DNA to daunomycin and its 13-dihydro analogue.

We present the results of thermodynamic integration/molecular dynamics on the 13-dihydrodaunomycin.d(CGTACG) complex and 13-dihydrodaunomycin in vacuo based on the GROMOS force field. The objective is to calculate the binding free energy differences between two complexes--13-dihydrodaunomycin.d(CGTACG) and daunomycin.d(CGTACG). The results are in agreement with the experimental data, i.e. the theoretical binding free energy difference of 13-dihydrodaunomycin.d(CGTACG) and daunomycin.d(CGTACG) is 2.1 +/- 1.6 kcal/mol which accords well with the experimental value of 0.6 kcal/mol. The free energy contributions of different interactions and different groups have also been analysed.

Nerve-dependent expression of high polysialic acid neural cell adhesion molecule in neurohypophysial astrocytes of adult rats.

The adult hypothalamo-neurohypophysial system of the rat retains the capacity to express highly polysialylated isoforms of neural cell adhesion molecule normally expressed in developing tissues. Here we report that the expression of these isoforms in neurohypophysial astrocytes (pituicytes) may be regulated by neurosecretory cells. In the intact neurohypophysis a strong and homogeneously distributed immunostaining for the "embryonic", highly sialylated form of neural cell adhesion molecules was detected by light-microscopic immunocytochemistry. By electron-microscopy, both neurosecretory axons and pituicytes were immunoreactive for this isoform. However, in contrast to the rather uniform staining on nerve fibres, polysialic acid immunolabelling on glial surfaces was uneven: immunostaining could be observed on glial surfaces facing neuronal elements, but not at contact sites between pituicytes. In addition, most glial and neuronal elements were heavily and evenly labelled with the polyclonal antibody recognizing "total" neural cell adhesion molecule. Surgical transection of the hypophysial stalk, a procedure that eliminates descending neurosecretory axons from the neurohypophysis, resulted in the complete disappearance of polysialic immunoreactivity from the neurohypophysis. The electron-microscopic analysis confirmed that cell surfaces of pituicytes lacked this immunoreactivity after the lesion. When residual neurosecretory axons were observed following an incomplete lesion, immunoreactivity on axons and glial processes was maintained. Transection did not affect the distribution of "total" neural cell adhesion molecule. We postulate that the presence of neurosecretory axons in the neurohypophysis is necessary to maintain the capacity of pituicytes to express immunoreactivity for the polysialylated isoforms of neural cell adhesion molecule but not the neural cell adhesion molecule itself since immunoreactivity for "total" neural cell adhesion molecule was unaltered after hypophysial stalk transection.(ABSTRACT TRUNCATED AT 250 WORDS)

Assessment of human sperm acrosome reaction by flow cytometry: validation and evaluation of the method by fluorescence-activated cell sorting.

OBJECTIVE: To determine the applicability of flow cytometry to assess human sperm acrosome reaction. DESIGN: Prospective evaluation of semen samples incubated overnight for the development of spontaneous acrosome reaction or exposed to the calcium ionophore A23187 (5 microM) for 3 hours for induction of the acrosome reaction. SETTING: University-affiliated tertiary care center. PATIENTS: Normal healthy volunteers. INTERVENTIONS: The spermatozoa were stained with fluorescein isothiocyanate (FITC)-labeled pea agglutinin. The labeled samples were assessed visually and also subjected to analytic flow cytometry and fluorescence-activated cell sorting. MAIN OUTCOME MEASURES: Acrosome reaction assessed visually and by flow cytometry. RESULTS: Flow cytometric analysis showed a single peak of FITC fluorescence in the washed semen samples. A second peak of lower FITC fluorescence intensity was noted after overnight incubation or exposure to A23187, suggesting loss of fluorescence, which indicated the occurrence of the acrosome reaction. There was a statistically significant correlation between the assessment by the two methods (n = 41). However, although the mean difference between the methods was small (2.49%), the difference between the two methods for each individual sample can vary between -24% to +29%. When the sperm cells were subjected to cell sorting based on green fluorescence intensity, reanalysis and visual scoring verified that the low intensity peak contained a majority of acrosome-reacted spermatozoa (77.52% +/- 2.39%). CONCLUSION: These results validated the flow cytometric method for assessment of acrosome-reacted spermatozoa. Although flow cytometry is more objective and less time consuming when many samples are assessed at the same time, the visual method remains a useful and practical procedure in the clinical andrology laboratory.

Differential effects of alfentanil, fentanyl, pethidine and lignocaine administered intrathecally on nociceptive responses evoked by low and high frequency stimulation of somatic nerves.

We have studied in anaesthetized dogs the effects of alfentanil, fentanyl, pethidine and lignocaine administered intrathecally on nociceptive responses evoked by low and high frequency supramaximal electrical stimulation of the tibial and radial nerves. Doses were selected to abolish both A delta and C fibre somatosympathetic reflexes to single stimuli. Pethidine and lignocaine eliminated reflex pressor and heart rate responses to repeated single stimuli and almost completely abolished responses to train stimulation. The pressor response to single stimuli was abolished by fentanyl and reduced by alfentanil, but these drugs did not reduce significantly this response to train stimulation, suggesting a stimulation rate-dependent effect. Of the opioids, only pethidine had an effect comparable to that of lignocaine. The absence of sympathetic block and antagonism by naloxone imply a lack of significant local anaesthetic effect. We suggest that the greater analgesic efficacy of pethidine is a result of endogenous synergism between a minor local anaesthetic and a major opioid effect.

Lack of a ceiling effect for intrathecal buprenorphine on C fibre mediated somatosympathetic reflexes.

We observed that buprenorphine 20 micrograms kg-1 i.v. in dogs caused an initial significant reduction in both A delta and C fibre mediated somatosympathetic reflexes evoked by tibial and radial nerve stimulation, to approximately 75% and 70% of control values. Larger doses (up to 100 micrograms kg-1 i.v.) had progressively less effect and the mean responses were depressed to only approximately 65% and 55% of control, indicating a ceiling effect. Buprenorphine 450 micrograms intrathecally (i.t.) completely abolished tibial C fibre reflexes, but 20% of A delta responses could not be eliminated with doses up to 1050 micrograms i.t. Fentanyl 100 micrograms kg-1 i.v. or 150 micrograms i.t. after buprenorphine i.v. or i.t., respectively, had little additional effect. This study confirms the limited ceiling effect of buprenorphine on nociceptive reflexes when administered systemically, and provides evidence that when administered i.t. in sufficient doses it abolishes the C responses (lack of ceiling effect for C responses), but the A delta responses show a plateau or ceiling effect.

Urinary excretion of deuterium-labeled folate and the metabolite p-aminobenzoylglutamate in humans.

Stable isotopic methods were employed to determine the proportion of ingested folate excreted as the metabolite p-aminobenzoylglutamate (pABG, free and N-acetyl) in urine. Adult male subjects (n = 7) were maintained on a 4.54 mumol/d (2 mg/d) folate saturation regimen. After 7 d, subjects were given a mixed oral dose containing 0.67 mumol (300 micrograms) each of bideuterofolic acid (d2) and tetradeutero-folic acid (d4). Urine was collected for the following 48 h and analyzed for folate and pABG. The extent of deuterium labeling of urinary folate and pABG was determined by gas chromatography-mass spectrometry. Urinary total pABG excretion increased less than twofold as a result of the folate saturation, whereas urinary folate increased over 10-fold. Urinary d2 and d4 folates each contained 14-15% of the respective oral doses of labeled compounds, whereas urinary d2 and d4 pABG comprised only 0.98-1.15% of the labeled doses. Molar ratios (d2/d4) of excreted folate and pABG indicated that there was no in vivo isotopic discrimination between the labeled folates. Urinary pABG accounted for 5.1 +/- 0.6% of total ingested folate, whereas labeled pABG was about 6.7-7.6% of the excretion of labeled compounds (i.e., labeled folate + pABG). This study indicated that pABG is not a major excretory product during folate supplementation, but its relative importance may increase in conditions of reduced folate nutriture.

Human spermatozoal tail hypo-osmotic swelling test, motility characteristics in hypotonic saline, and survival of spermatozoa after cryopreservation.

Normozoospermic semen samples (n = 82) were examined to investigate whether the degree of sperm tail swelling in hypo-osmotic medium (fructose and sodium citrate; 150 mOsm/l), and motility characteristics after a 15-min exposure to hypotonic saline (sodium chloride; 150 mOsm/l) could predict the cryosurvival rate (% post-thaw motility/% pre-freeze motility x 100%) of spermatozoa after cryopreservation by the liquid nitrogen vapour freezing method using the TEST-glycerol-egg yolk buffer. The CellSoft automated semen analyser was used to analyse sperm motility in pre-freeze and post-thaw semen samples, and after exposure to hypotonic saline. Sperm tail hypo-osmotic swelling and sperm motility in pre-freeze semen showed no significant correlations (P > 0.05) with the cryosurvival rate. There were significant correlations (P < 0.05) between the cryosurvival rate and the following sperm motility parameters in hypotonic saline: % motility (r = 0.2846), motility index (% motility x curvilinear velocity; r = 0.2809) and % decrease in motility index from the baseline value in semen (r = 0.3378). The % decrease in motility index after hypotonic saline treatment was significantly less (P < 0.05) in the normal (> or = 50% cryosurvival rate; mean +/- SEM 5.9 +/- 3.2%; n = 33) compared with the subnormal (< 50% cryosurvival rate; 27.3 +/- 4.8%; n = 49) cryosurvival groups. This parameter was also determined, by multivariate discriminant analysis, to be capable of classifying each pre-freeze semen sample into either cryosurvival group with 69.5% accuracy.(ABSTRACT TRUNCATED AT 250 WORDS)

Specific enhancement by fentanyl of the effects of intrathecal bupivacaine on nociceptive afferent but not on sympathetic efferent pathways in dogs.

BACKGROUND: Bupivacaine alone, or in combination with opioids, has been shown to provide adequate pain relief without motor paralysis. This study examined the effects of bupivacaine administered intrathecally on sympathetic efferent and A delta- and C-fiber-mediated afferent pathways in dogs and the interactions with intrathecal fentanyl. METHODS: Spontaneous activity in renal sympathetic nerves was observed, as were reflex somatosympathetic responses mediated by A delta and C fibers evoked by supramaximal electrical stimulation of the tibial and radial nerve. Bupivacaine was administered intrathecally in doses of 0.5, 1, 2, and 3.5 mg, each in 0.5 ml, and 7 mg in 1 ml with or without pretreatment with 5.4 mg intrathecal fentanyl (ED25 for depression of C tibial reflexes) in each of five preparations. RESULTS: Bupivacaine caused a dose-dependent inhibition of both A delta- and C-fiber-mediated somatosympathetic responses evoked by tibial nerve stimulation. The depression of radial and tibial nerve reflexes and spontaneous renal sympathetic activity was similar. Pretreatment with fentanyl (5.4 micrograms, intrathecally) depressed tibial C-fiber reflexes by only 23.8% without any significant effect on either tibial A delta or radial A delta and C fiber responses. Fentanyl markedly enhanced the effect of subsequent doses of bupivacaine on tibial A delta and C reflexes without any additional effect on either spontaneous sympathetic activity or radial responses. CONCLUSIONS: Intrathecal bupivacaine has no selectivity for the afferent and efferent pathways, and intrathecal fentanyl acts synergistically to enhance the effect of bupivacaine on the afferent pathway without a measurable effect on sympathetic outflow.

Seminal leucocyte subpopulations and sperm function in fertile and infertile Chinese men.

The level of seminal leucocytes and the prevalence of leucocytospermia was determined in a group of fertile and infertile southern Chinese men in Hong Kong. Sixteen normal fertile semen donors and 49 men with male factor infertility were studied prospectively. None had antisperm antibodies and past or present evidence of genital tract infection. Seminal leucocytes and their subsets were analysed using monoclonal antibodies and an immunocytochemical alkaline phosphatase-anti-alkaline phosphatase conjugate technique. Seminal leucocytes were detectable in 94% and 86% of the fertile and infertile men respectively, with the predominant subset being granulocytes. Leucocytospermia (> 1 x 10(6) leucocytes/ml) was found in only one of the 49 (2%) infertile men without clinical evidence of genito-urinary infection. Inverse correlations were observed between (1) the percentage of spermatozoa with normal morphology and the number of T-helper/inducer cells, (2) the linearity of sperm movement and the number of T-lymphocytes. In conclusion, the level of seminal leucocytes and the prevalence of leucocytospermia is low in infertile Chinese subjects. The effect of seminal leucocytes on sperm function in these subjects needs further evaluation.

Translation of human hepatitis C virus RNA in cultured cells is mediated by an internal ribosome-binding mechanism.

The human hepatitis C virus (HCV) contains a long 5' noncoding region (5' NCR). Computer-assisted and biochemical analyses suggest that there is a complex secondary structure in this region that is comparable to the secondary structures that are found in picornaviruses (E.A. Brown, H. Zhang, L.-H. Ping, and S.M. Lemon, Nucleic Acids Res. 20:5041-5045, 1992). Previous in vitro studies suggest that the HCV 5' NCR plays an important role during translation (K. Tsukiyama-Kohara, N. Iizuka, M. Kohara, and A. Nomoto, J. Virol. 66:1476-1483, 1992). Dicistronic and monocistronic expression vectors, in vitro translation, RNA transfections, and deletion mutagenesis studies were utilized to demonstrate unambiguously that the HCV 5' NCR is involved in translational control. Our data strongly support the conclusion that an internal ribosome entry site exists within the 5' noncoding sequences proximal to the initiator AUG. Furthermore, our results suggest that the HCV genome is translated in a cap-independent manner and that the sequences immediately upstream of the initiator AUG are essential for internal ribosome entry site function during translation.

Ion channel activity of influenza A virus M2 protein: characterization of the amantadine block.

The influenza A virus M2 integral membrane protein has ion channel activity which can be blocked by the antiviral drug amantadine. The M2 protein transmembrane domain is highly conserved in amino acid sequence for all the human, swine, equine, and avian strains of influenza A virus, and thus, known amino acid differences could lead to altered properties of the M2 ion channel. We have expressed in oocytes of Xenopus laevis the M2 protein of human influenza virus A/Udorn/72 and the avian virus A/chicken/Germany/34 (fowl plague virus, Rostock) and derivatives of the Rostock ion channel altered in the presumed pore region. The pH of activation of the M2 ion channels and amantadine block of the M2 ion channels were investigated. The channels were found to be activated by pH in a similar manner but differed in their apparent Kis for amantadine block.

Mild phenotypic manifestation of a 7p15.3p21.2 deletion.

A 28 month old girl with dysmorphic features was found to have an interstitial deletion of the short arm of chromosome 7p15.3-7p21.2. The patient had ptosis, dacryostenosis, pectus excavatum, short hands, and her development was normal or mildly delayed. Craniosynostosis and growth retardation, which were present in two other patients with similar deletions, were not present. Because of the mild manifestations, this case expands the clinical spectrum of the 7p15-7p21 deletion phenotype.

Alcoholism in Peru.

OBJECTIVE: Alcoholism is a problem of worldwide concern. Full appreciation of this international problem requires that adequate diagnostic measures be constructed and that comparable measures for different cultures be available so that valid differences in prevalence across cultures can be detected. A Spanish-language version of the Diagnostic Interview Schedule (DIS) has been used for epidemiologic studies of alcohol abuse and dependence in Los Angeles Mexican-Americans and mainland Puerto Ricans, and the authors used the same instrument to conduct a similar study in Peru. METHOD: A population sample (N = 815) from the Independencia district of Lima, Peru, was chosen for interviews with a revised form of the Spanish translation of the DIS. Lifetime prevalence rates of alcoholism and other DSM-III diagnoses were determined. RESULTS: The prevalence of alcohol abuse or dependence was higher among the men (34.80%) than among the women (2.46%), but the onset for women was earlier. Alcoholism was strongly associated with antisocial personality disorder and with drug abuse or dependence. CONCLUSIONS: The prevalence of alcoholism for the Peruvian men is higher than prevalences for men in U.S. studies, but the prevalence among the Peruvian women is one of the lowest reported. The high prevalence among men is likely due to cultural mores but may also be linked to the stresses found in impoverished societies undergoing rapid social, cultural, and economic change.

Reproductive aging in the male brown-Norway rat: a model for the human.

Previous studies in the Sprague-Dawley rat have demonstrated that male reproductive aging is primarily a neuroendocrine dysfunction characterized by decreased pulsatile LH secretion leading to low serum testosterone (T) levels, whereas sperm production is relatively well maintained. In contrast to the Sprague-Dawley rat, the Brown-Norway (BN) rat has a longer life span, does not become obese, and experiences fewer age-related tumors of the endocrine or reproductive system, thus providing a disease-free model for studying male reproductive aging. We studied the changes in serum hormone levels and related these alterations to testicular T production, Leydig cell morphometry, and spermatogenesis in young (6 months), aging (22 months), and old (30 months) BN rats. Low serum T levels were associated with decreased LH levels in the 22-month-old (T, 0.58 +/- 0.08 ng/ml; LH, 0.45 +/- 0.06 ng/ml) and 30-month-old rats (T, 0.63 +/- 0.10 ng/ml; LH, 0.34 +/- 0.04 ng/ml) compared to those in young rats (T, 1.35 +/- 0.30 ng/ml; LH, 0.79 +/- 0.10 ng/ml). In vitro Leydig cell T production, basally and after stimulation by LH, was similar in young and old rats. The total testicular T content was lower in 30- than in 6-month-old rats. Testicular morphometry showed smaller Leydig cell volume in the old (857 +/- 97 microns3) than in the young rats (1387 +/- 103 microns3), although their number per testis remained unchanged (6 months, 22.7 +/- 1.6 x 10(6)/testis; 30 months, 25.2 +/- 3.1 x 10(6)/testis). In contrast, a marked (68.4%) reduction in the total number of Sertoli cell per testis was noted in the 30-month-old rats. The proportion of the testis occupied by seminiferous tubules was also reduced in the old rats. Significant (P < 0.05) age-related reductions occurred in tubule diameter, length of tubules, and volume of tubules and their lumens. The testicular sperm concentration and total sperm production were significantly reduced in the 22- and 30-month-old rats. These changes in seminiferous tubule function in the old rats were associated with low serum and testicular inhibin and high serum FSH levels. We conclude that aging in the reproductive axis of the BN rat is manifested by 1) lower serum T levels due to decreased pituitary LH stimulation of endogenous T production, and 2) decreased seminiferous tubule function accompanied by elevated FSH levels indicative of a primary testicular disorder.(ABSTRACT TRUNCATED AT 400 WORDS)
