Latent carcinoma of prostate at autopsy in seven areas. The International Agency for Research on Cancer, Lyons, France.

A world-wide comparative study of the frequency and characteristics of latent carcinoma of the prostate was undertaken in seven areas, using standardized methods and "blind" microscopic evaluation in order to reduce selection and observer bias. The morphological features of 350 latent carcinomas found in 1,327 prostates were examined. Two Chinese populations, from Hong Kong and Singapore, showed a low frequency of latent carcinoma in comparison with western Europeans in Sweden and the Federal Republic of Germany and negroes from Jamaica; an intermediate position was found for Israelis and black Ugandans. The frequency of small latent carcinomas was about 12% in all the areas investigated and did not vary with age. Rates for larger latent carcinomas increased sharply with age ans showed an area-to-area variation resembling that of clinical carcinoma of prostate. The small carcinomas were almost exclusively situated in the outer half of the prostate and latent carcinomas of all sizes were evenly distributed between the anterior and posterior halves of the prostate and the right and left sides of the outer prostatic shell. Certain disagreements in diagnosis were noted when the sections from each area were evaluated independently by a different pathologist. Most of these disagreements were resolved by re-reading the sections; their occurrence had no significant effect on the geographical comparisons.

Mucin histochemistry of human cholangiocarcinoma.

Histochemical studies of mucins were carried out on 17 autopsy specimens of cholangiocarcinomas associated with clonorchiasis. All tumours produced epithelial mucins which were mixtures of neutral and acid mucopolysaccharides without any histochemically demonstrable components of sialic and uronic acids. The mucins were qualitatively similar to that secreted by normal and Clonorchis-infested bile ducts. However carboxymucins were reduced and sulphomucins were absent or present only in trace amounts in the neoplastic epithelium; in the hyperplastic epithelium in clonorchiasis sulphomucins were abundant. The HID-AB technique for sulphomucins is valuable in differentiating hyperplastic bile ducts and cholangiocarcinoma.

The role of Chinese medicine in New York City's Chinatown. 1.

This study reports on use patterns of traditional Chinese and Western medical resources in New York City's Chinese community. Extensive data from field-work and interview are presented which provide significant new information concerning the health-care attitudes and problems of this unique community.

Mucin-producing cholangiocarcinoma: an autopsy study in Hong Kong.

Fifty autopsy cases (35 male and 15 female) of mucin-secreting cholangiocarcinoma in Chinese were reviewed. The peak incidence was in the 7th decade for males and in the 6th for females. Massive (37), multinodular (8), diffuse (1) and hilar (4) types were recognized grossly. The hilar tumours arose from the main intrahepatic ducts and the other types originated from smaller ducts. The overall association with stones was 20% and clonorchiasis 92%. Cirrhosis occurred in only 4% of cases. There was an association between the degree of mucin secretion and the presence and severity of clonorchiasis.

Health risk appraisal modifies cigarette smoking behavior among college students.

To assess whether Health Risk Appraisal (HRA), a computer-scored lifestyle analysis questionnaire, can result in significant changes in health behavior, a controlled clinical trial with one-year follow-up was conducted among entering freshmen at an urban state university. Three hundred fifty entering freshmen were each assigned to one of four groups: HRA with feedback, HRA without feedback, initial control, and final control. Twenty-two per cent of the nonsmokers in the no-feedback group, as compared with 5% in the feedback group, acquired the habit of cigarette smoking (p less than 0.001). Furthermore, 26% of the smokers in the feedback group, as compared with 6% in the no-feedback group, were able to quit smoking (p less than 0.05). Among those unable to quit smoking, 59% in the feedback group, as compared with 19% in the no-feedback group, were able to reduce their cigarette consumption by more than six cigarettes per day (p less than 0.01). Health Risk Appraisal, when accompanied by feedback counseling, was an effective health promotion tool to help prevent nonsmokers from acquiring the habit and to modify cigarette smoking behavior among college freshmen.

Partial patellectomy induces a decrease in the proteoglycan content in the remaining patellar articular cartilage. An experimental study in rabbits.

We studied the alterations and distribution of the proteoglycan (PG) content of the remaining patellar articular cartilage after unilateral partial patellectomy in 13 rabbits. Sagittal sections of the patella were prepared and stained with Safranin O for quantification of changes in the PG content of the patellar articular cartilage using a commercially available imaging analysis system. Our findings suggest that partial patellectomy results in a decreased PG content in the remaining patellar articular cartilage. In addition, the postoperative development of metaplasia in the scar tissue next to the healing interface may represent a compensatory response, which could prevent a further reduction in the PG content and hence the development of osteoarthritis in the remaining patellar articular cartilage.

Experience of palliative home care according to caregivers' and patients' ages in Hong Kong Chinese people.

PURPOSE/OBJECTIVES: To identify the relationship between family caregivers' reported difficulty in managing caregiver tasks and ages of caregivers and patients. DESIGN: Cross-sectional, descriptive survey. SETTING: A hospice homecare program in Hong Kong. SAMPLE: Twenty-nine Chinese family caregivers who had experienced at least weekly caregiving responsibility for more than two months and were able to read and understand Chinese. METHODS: Respondents completed a caregiver task inventory. Four homecare nurses assisted in the distribution and collection of questionnaires. MAIN RESEARCH VARIABLES: Caregiver tasks and age and patient age. FINDINGS: Caregivers' age was negatively correlated with reported difficulty in overall tasks and in interpersonal ties. The patients' age was negatively correlated with reported difficulty in direct care to patients, intrapersonal tasks, and overall tasks. CONCLUSIONS: The younger the caregiver, the more difficulty he or she experienced in the caregiving role, particularly in the maintenance of social and family ties. Caregivers of younger patients experienced more difficulty in most aspects of caregiving tasks. More research with a larger sample size is required to fully investigate the effect of age on the family caregiving experience and the validity of the caregiver task inventory. IMPLICATIONS FOR NURSING PRACTICE: Nursing support and preparation to younger caregivers and caregivers of younger patients are suggested in the practice of palliative home care.

Pulmonary thromboembolism and venous thrombosis in the Chinese.

In 3,876 autopsies performed between 1964 and 1974 in adult Hong Kong Chinese, the incidence of pulmonary thromboembolism was 0.75%. In a prospective study of 70 autopsies, the right lungs were inflated, fixed, slab-sectioned, and studied for pulmonary thromboembolism. Twenty-seven per cent had evidence of pulmonary thromboembolism, although in only one case was the lesion the cause of death. In another 62 autopsies, routine dissection of pelvic and both lower extremity veins revealed that 18% had evidence of thrombosis. Eighty-three per cent of these had pulmonary thromboembolism; in 3 of these 62 cases (5%) pulmonary thromboembolism was the cause of death. Massive pulmonary thromboembolism in the Hong Kong Chinese is much less frequent than in Caucasians. The finding of many small asymptomatic pulmonary thromboemboli in both racial groups suggests that pulmonary circulation performs a physiologic filtering function. The rarity of massive clots in the Chinese warrants further investigation and special attention to biomechanics of thrombosis formation and detachment.
