[Wild domestic pigeons--a public health problem of increasing significance in East Germany].

In many cities of the GDR the number of wild house pigeons is increasing. Because of their distribution, noxious effects and hygienic importance they are considered by law as health pests. Therefore, their populations have to be regulated in such a way to minimize the noxious effects caused by them. The State Hygiene inspectorate is responsible for organization and surveillance of health pest control. At present in the GDR two procedures are allowed applying chemical substance for pest control by specialists. A catching procedure, applicable everywhere, is well proving a success. By restauration of old buildings and closing of lofts in new ones it is possible to take prophylactic measures against the increasing number of pigeons. The municipal administration should coordinate the possible measures in the respective territory.

Patterns of anthracycline retention modulation in human tumor cells.

Laser excitation of cellular doxorubicin and daunomycin content (with or without incubation in the presence of efflux blockers such as phenothiazines or verapamil) was studied in cells from leukemic peripheral blood, bone marrow aspirates, and ascites and pleural fluid of solid tumor patients. Selected examples are presented to show that heterogeneity in cellular anthracycline retention as well as sensitivity to efflux modulators is seen in human tumor cells. Several tumor subpopulations differing in their cellular retention of anthracyclines or sensitivity to modulators were seen. In serial tumor samples from patients (pre- and post-treatment with anthracycline-containing protocols), initial drug retention and sensitivity to efflux modulators was followed by lack of drug retention and insensitivity to modulators. The present study shows that in view of the variables encountered in drug-retention characteristics and sensitivity to efflux modulators, one needs to screen tumor samples before recommending use of any particular transport modulator to enhance drug retention and sensitivity of drug-resistant cells.

[Psychiatric consultant care in a general hospital in association with inpatient crisis intervention].

This study examined the psychiatric care given at a general hospital associated with a clinical crisis-intervention ward in Munich. The correlations between frequency of suicide attempts, psychiatric crisis and the transfer of patients (treated by attending physicians) to a crisis-intervention ward of particular interest. Of the total 6,004 patients in the general hospital, 227 (130 female, 97 male) were psychogenetic reactions (36.6%) or alcohol-, medicine-, and drug-dependent cases (26.8%). The average patient age in acute crisis was significantly lower than that of other patients. Suicide syndrome was found in 91 cases; 23 patients were transferred to the crisis-intervention ward within 24 h and 7 after 24 h. There were significantly more women than men. The configuration-frequency analysis indicated three significant types: (1) "crisis-patients" (N = 18) with suicide syndrome, psychiatric crisis patients who were moved to the crisis-intervention ward within 24 h; (2) typical "non-crisis patients" (N = 111) without positive indication of these three variables; and (3) a group of 48 patients who, in spite of an acute crisis and suicide syndrome, were not transferred to the crisis-intervention ward. In the follow-up study 1 year later, another group of patients (N = 23), which had refused admission to the crisis-intervention ward, confirmed the high-risk expectation when compared with the patients who were admitted.

A randomized multicenter trial of cyclophosphamide, Novantrone and 5-fluorouracil (CNF) versus cyclophosphamide, Adriamycin and 5-fluorouracil (CAF) in patients with metastatic breast cancer.

As of August 1984, 115 women with advanced breast cancer have been randomized to receive a combination of either cyclophosphamide, Novantrone (mitoxantrone) and 5-fluorouracil (CNF) or cyclophosphamide, Adriamycin (doxorubicin) and 5-fluorouracil (CAF). Seventy-one percent of all patients were post-menopausal and 44% of CNF patients and 57% of CAF patients were estrogen receptor (ER) negative. Slightly over 30% of all patients had received hormonal therapy or chemotherapy in an adjuvant setting. Hematologic toxicity was similar in regard to platelet counts but slightly lower nadirs were experienced with CNF therapy than with CAF. However, there were fewer dosage decreases with CNF. Significantly less nausea and vomiting were observed with the CNF regimen compared to CAF. Moreover, alopecia was reduced appreciably in patients who received CNF. The response rate to CNF for the first 38 eligible and evaluable patients was 42%, and for 53 eligible and evaluable patients who received CAF the response rate was 45%, a non-significant difference. Median response durations were similar also, 140 days for CNF and 168 days for the CAF regimen. Time to treatment failure was similar for both regimens. CNF is an effective regimen for patients with advanced breast cancer, with less toxicity than CAF.

Phase I study and pharmacokinetics of weekly high-dose 13-cis-retinoic acid.

In an attempt to increase the peak plasma levels of 13-cis-retinoic acid (13-cis-RA) and its efficacy in vivo, a Phase I study and pharmacokinetics of weekly high-dose, oral 13-cis-RA was conducted in 23 cancer patients who were refractory to conventional treatments. At 200 mg/sq m, the mean peak plasma level of 13-cis-RA was 1.5 +/- 0.1 (SE) micrograms/ml; at 400 mg/sq m, the mean peak plasma level increased to 3.8 +/- 0.7 micrograms/ml. Further increases of the 13-cis-RA dose up to 1800 mg/sq m did not lead to proportional increases in either the mean peak plasma levels or area under the curve, indicating a saturable absorption phenomenon. The terminal half-life was highly variable (range, 2.8 to 101.3 h) and was not related to the dose given. A secondary peak plasma concentration was seen in five patients, suggesting enterohepatic circulation. The toxicities such as headache, cheilitis, dry skin, and dry eyes were frequent on the weekly schedule but were not dose-limiting. One patient had an elevation of the triglycerides of 2 to 5 times the upper limit of normal; five patients had an elevation of 1.1 to 2 times normal. No objective responses were observed to treatment with 13-cis-RA. Of 20 patients receiving an adequate trial of the drug, 18 showed progression of their cancer, and two had stable disease.
