Thimerosal is an organic mercurial compound widely used as a preservative in vaccines, eyedrops, and contact lens cleaning and storage solutions. 5 infants, 2 female and 3 male, ranging in age from 7 to 28 months and affected by atopic dermatitis (AD) diagnosed according to the Hanifin and Rajka criteria, experienced an exacerbation of their clinical condition 2-10 days after mandatory vaccinations with vaccines containing thimerosal. Cutaneous lesions of nummular eczema appeared on the trunk, limbs and face. All patients were patch tested with serial dilutions of thimerosal in petrolatum. A positive patch test reaction to thimerosal 0.1% pet. was observed in all 5 children. 3 of them also showed a positive reaction at 0.01% and 0.05% pet. Despite their thimerosal-hypersensitivity, all children completed the entire series of mandatory vaccinations, care being taken to use different needles for injection and aspiration of the vaccine. The 2-year follow-up did not reveal other episodes of exacerbation of the AD after vaccination. The present study confirms the high frequency of sensitization to thimerosal in atopic children and suggests that vaccination can cause clinical symptoms in sensitized children. Nevertheless, sensitization to thimerosal does not prevent children from continuing with mandatory vaccinations.

BACKGROUND: Compositae dermatitis involves workers such as gardeners, florists, and farmers, and less commonly people not employed in occupations at risk. OBJECTIVE: To report a case of contact sensitization caused by Compositae, the source of sensitization being both pesticides and plants. METHODS: A 65-year-old man, whose hobby was gardening, was affected by a chronic eczema of the face and the hands for 6 months that reoccurred after using fertilizers and handling plants. Patch tests with the International Contact Dermatitis Research Group standard series, the plants series and pesticides series (Hermal Trolab) were performed. RESULTS: We found the following positive reactions at day 2 and day 3: sesquiterpene lactone mix, 0.1% in petrolatum; arnica tincture, 20% in petrolatum; pyrethrum, 2% in petrolatum; captafol, 0.1% in petrolatum. CONCLUSION: Pyrethrum and its derivatives are made from Compositae plants; the arnica tincture is obtained from Arnica montana, which is also a member of the Compositae family. In our patient, the clinical manifestations were certainly caused by the exposure to sesquiterpene lactones, confirmed by the positive patch test to sesquiterpene lactone mix.
BACKGROUND AND OBJECTIVE: Long-term culture-initiating cells (LTC-IC) are the best available approximation to an in vitro assay of stem cells in humans although they still represent a heterogeneous population in terms of proliferative capacity and sensitivity to different growth factors. Human umbilical cord blood (CB) is rich in hemopoietic progenitor cells, as measured by clonogenic assays and contains stem cells capable of reconstituting the marrow after ablation in clinical transplantation. We evaluated the influence of culture conditions on the in vitro behavior of LTC-IC from CB. DESIGN AND METHODS: LTC-IC were evaluated in long-term cultures, comparing two types of murine stromal cell lines: M2-10B4 and M2-10B4 transfected with cDNAs for human G-CSF and IL-3. RESULTS: Two and five fold higher numbers of terminally differentiated cells were produced during nine weeks of culture of CB mononuclear or CD34+ cells respectively, in cultures containing a M2-10B4 IL-3 G-CSF cell line compared to cultures containing the parental cell line. Likewise, a higher number of colony-forming cells (CFC) were detected in the supernatant of cultures with the transfected cell line. In contrast, the number of CFC generated within the stromal layer, after 5 or 9 weeks of culture, was significantly higher in cultures on M2-10B4 cells than those on M2-10B4 IL-3 G-CSF. INTERPRETATION AND CONCLUSIONS: Our results show that the proliferative capacity of CB LTC-IC can be strongly influenced by culture conditions and that the frequency of LTC-IC estimated using these cell lines as stromal support is not identical.

Five women affected by androgenetic alopecia developed severe hypertrichosis of the face and limbs after 2-3 months of treatment with 5% topical minoxidil. Minoxidil was discontinued and in all patients the hypertrichosis disappeared from the face and arms after 1-3 months, and from legs after 4-5 months. Systemic absorption of minoxidil, and a high sensitivity to minoxidil of the follicular apparatus in these areas, is hypothesized.
BACKGROUND: Para aminobenzoic acid (PABA) derivatives and cinnamate are chemical sunscreens that protect against UVB (290 to 320 nm). They may occasionally produce contact and photocontact sensitization. OBJECTIVE: To report a sensitization to octyl-dimethyl-PABA and photosensitization to 2-ethylhexyl-p-metossicinnamate in a 31-year-old man. METHODS: A patient with a 3-year history of a relapsing dermatitis involving the face, neck, legs, and knees is reported. The eruption had recurred every summer after sunlight exposure. Patch tests with International Contact Dermatitis Research Group (IC-DRG) standard series and the photopatch series (Hermal-Trolab, Reinbek, Germany) using Finn chambers on Scanpor (Norgesplaster A/S, Oslo, Norway) were carried out. RESULTS: We found a positive reaction to Balsam of Peru, fragrance mix, Escalol 507, and Parsol MCX (Hermal-Trolab, Reinbek, Germany). Photopatch test revealed a positive reaction only for Parsol MCX. CONCLUSION: The incidence of allergic contact dermatitis to sunscreens is considered low. Recently sunscreens patch test concentrations have been increased from 2% to 10%. These higher percentages will probably permit the identification of more cases of sunscreens allergy in the near future.

The aim of this study was to investigate the frequency of sensitization to fatty alcohols in a group of patients with suspected cosmetic or medicament contact dermatitis. From May 1992 to September 1995, we patch tested a series of 5 fatty alcohols on 146 patients. These included 108 females and 38 males aged from 13 to 72 years (mean age 42.5). These patients, who had previously been tested with the GIRDCA standard series, were selected because their clinical lesions or histories indicated topical preparations as the possible source of their contact dermatitis. High-grade fatty alcohols (> 99% pure) were used for testing. 34 patients (23.2%), 25 female and 9 male aged from 14 to 72 years, showed a positive patch test to fatty alcohols, 33 of them to oleyl alcohol. A total of 39 reactions were detected with 5 patients showing more than 1 positive reaction. Our results show that sensitization to oleyl alcohol is not rare in patients with contact dermatitis due to cosmetics or topical medicaments.
One hundred and twenty-four sera from patients with various leukemic B-cell chronic lymphoproliferative diseases were investigated at diagnosis by ELISA for their soluble CD23 content. Immunophenotyping was carried out in all patients, and in a selected subset the mean number of membrane-bound CD23 molecules per cell was also investigated. Seventy-three patients had typical B chronic lymphocytic leukemia, 41 leukemic B-cell disorders with atypical morphological and/or immunophenotypic features, 5 had low-grade follicular cell lymphoma in the leukemic phase, and 5 had splenic lymphoma with villous lymphocytes. Soluble CD23 levels were significantly higher than in normal sera (mean +/- SD: typical B chronic lymphocytic leukemia 3,650 +/- 4,654 U/ml, atypical B chronic lymphocytic leukemia 3,440 +/- 4,671 U/ml, follicular cell lymphoma 3,200 +/- 1,511 U/ml, splenic lymphoma with villous lymphocytes 8,236 +/- 7,294 U/ml, controls 137 +/- 128 U/ml; P < 0.001). More advanced Rai's stages were related to higher soluble CD23 levels (P < 0.01), both in typical and atypical B chronic lymphocytic leukemias, the highest levels and the best correlation with the absolute number of circulating CD19+ cells (r = 0.50) being observed in the typical form. The number of membrane-bound CD23 molecules per cell was significantly higher in typical than in atypical B chronic lymphocytic leukemias (mean number 156,727 +/- 94,668 vs. 12,010 +/- 10,643, P < 0.001). Our data suggest that soluble CD23 levels correlate with the clinical and biological features of leukemic B-cell lymphoproliferative disorders.

The aim of this study was to verify the most suitable vehicle and concentration for testing Euxyl K 400 and its individual ingredients, and to evaluate the prevalence of sensitization to this preservative over the years in Italy. From January 1991 to October 1994, Euxyl K 2.5% pet. was positive in 99 patients (35 male, 64 female) out of 3455 (2.8%). Of these, 22 out of 855 patients had a positive reaction during 1991 (2.6%), 29 out of 1037 in 1992 (2.8%), 28 out of 858 in 1993 (3.3%), and 20 out of 705 in 1994 (2.8%). 51 of the 99 patients with a reaction to Euxyl K 400 2.5% pet. showed a positive reaction to dibromodicyanobutane 0.5% pet. and 2 to phenoxyethanol 5% pet. The results of patch testing with serial dilutions of Euxyl K 400 in different vehicles indicate that water is a good vehicle for testing the preservative. However, since Euxyl K 400 is only hydrosoluble to a limited extent, the maximum concentration that can be tested using water is 0.5%, and so with this concentration about 40% of sensitized patients are missed. The results of patch testing with serial dilutions of Euxyl K 400 in petrolatum demonstrate that concentrations lower than 2.5% are not suitable for detection of all sensitized patients. Euxyl K 400 in ethanol frequently causes irritant reactions without offering significant advantages in detecting sensitized patients.
