A comparison between ilioinguinal-iliohypogastric nerve block performed by anaesthetist or surgeon for postoperative analgesia following groin surgery in children.

A study was performed to compare postoperative analgesia in children undergoing groin surgery. Patients were randomly allocated to receive ilioinguinal-iliohypogastric (I-I) nerve blocks using 0.25% plain bupivicaine (0.5 ml.kg-1) performed either percutaneously by the anaesthetist after the induction of general anaesthesia, before surgery commenced, or intraoperatively, under direct vision, by the surgeon. Analgesia was assessed by pain scoring in the recovery room and at hourly intervals for eight h postoperatively. Analgesic requirement was also noted. Statistical analysis of the results revealed no difference in pain score between groups treated either by anaesthetist or surgeon. However, children under two years of age had significantly higher pain scores than those over two.

Neonatal nurse practitioners: a descriptive evaluation of an advanced practice role.

This article describes the introduction of the neonatal nurse practitioner (NNP) role into a neonatal intensive care unit and reports the opinions and attitudes of physicians, registered nurses, and families regarding the NNP role. In addition, an overview of NNP education and credentialing mechanisms is presented. Surveys were used to assess the opinions and attitudes of obstetricians, pediatricians, family practitioners, registered nurses, and parents. The response rate ranged between 42 percent and 56 percent depending on the group surveyed. In general, both professionals and families were very satisfied with the clinical skills, knowledge base, and contributions to patient care that the NNP provided. Several different advantages and disadvantages to the NNP role in the NICU were identified by each professional group and by the parents.

How to evaluate lung fields on the neonatal chest X-ray film.

The general principles of evaluating lung fields have been presented. These concepts will be applied in future columns as specific disease entities are discussed. Before the column focuses on the specific cardiopulmonary pathologies, however, there will be one more column addressing general principles. Evaluation of cardiac size will be covered in the next column.

Physiological characteristics of tremor after chronic lithium therapy.

Twenty-three patients taking lithium carbonate alone in therapeutic dosage for a mean period of 43 months (range 6--108 months) and 23 control subjects matched for age and sex had the frequency and amplitude of postural tremor measured and extrapyramidal symptoms assessed by standard techniques. Patients on lithium had a significantly lower peak frequency of tremor than control subjects (P less than 0.05) and significantly greater extrapyramidal symptoms (P less than 0.01), indicating that tremor appearing after chronic lithium therapy is likely to be an extrapyramidal symptom. Such symptoms are common long-term adverse effects of lithium carbonate.
