Pericentral hepatic fibrosis and intracellular hyalin in diabetes mellitus.

Five patients with maturity-onset diabetes mellitus and hepatomegaly were studied. This group in cluded moderately obese females in whom hepatomegaly and abnormal liver studies were associated with a marked elevation in the erythrocyte sedimentation rate. Liver biopsies from all 5 showed fatty steatosis and pericentral fibrosis. In 3 of the 5, fibrous bridging between central veins and portal tracts was present. Collagen surrounded swollen hepatocytes, some of which contained intracellular hyalin indistinguishable from Mallory's hyalin. Polymorphonuclear neutrophils and regenerating nodules were not present. None of the patients had a history of alcohol use, and in 2 followed closely for 2 yr, alcohol was never detected in random blood samples. The recognition of this morphologic lesion may help to clarify the significance of this finding as an intermediate lesion between fatty steatosis and cirrhosis in diabetics.

Gestational choriocarcinoma and thyrotoxicosis.

A case of a 26-year-old woman with metastatic choriocarcinoma and clinical and biochemical thyrotoxicosis is described. This represents the eighth reported case of the association of choriocarcinoma and thyrotoxicosis. Serial monitoring of serum thyroxine (T4) and thyroid stimulating hormone (TSH) levels correlated precisely with the beta sub-unit human chorionic gonadotropin (hCG) level and the quantitation of host tumor burden. The development of a hypermetabolic syndrome in patients with choriocarcinoma may be due to secondary thyrotoxicosis from either the TSH-like activity of hCG or from the concomitant production of molar thyrotropin by the tumor.

Primary histiocytic lymphoma of the esophagus.

Squamous cell carcinoma represents the most common primary malignant lesion of the esophagus (1). The initial manifestation of lymphosarcoma infrequently occurs in the gastrointestinal tract, and in one large series, there were no lymphosarcomas presenting in the esophagus (2). In the case described below, a primary histiocytic lymphoma (large cell lymphoma) was discovered in the distal esophagus.

Palliation of hepatic metastasis.

Between 1971 and 1975, 55 patients underwent palliative radiation therapy for symptomatic hepatic metastasis. Most patients received 2400 rad in 300 rad fractions to the entire liver. There were 31 patients who received concomitant chemotherapy, and 14 who were prior chemotherapy failures. Ninety percent of the patients with symptomatic pain and liver enlargement and significant palliation of their symptoms. The median survival of the entire group was 4.5 months, while those patients experiencing an excellent response (21) had a median survival of 9 months. The median survival of patients having an excellent response to radiation is comparable to that of patients having regional arterial chemotherapy while incuring fewer complications. The overall complication rate of those patients completing therapy (50) was 12%.

Intraaterial hepatic infusion and intravenous adriamycin for treatment of hepatocellular carcinoma: a clinical and pharmacology report.

Four patients received intraarterial (ia) hepatic infusion and 10 received intravenous (iv) adriamycin for hepatocellular carcinoma. Four of each group are evaluable. The remaining 6 patients died within 14 days of intravenous therapy and are, therefore, considered nonevaluable. Patients received 2 to 9 courses of adriamycin every 3 weeks. One half of each group of evaluable patients had partial responses (pr). The group had pr for 22.5 weeks (range 8 to 37). The iv group had pr 27.2 weeks (range: 16 to 38.5). Mean survival was 21 weeks for nonresponders, and 43 weeks for responders. Intraarterial infusion did not protect patients from adriamycin toxicity. Cardiac and liver toxicity were not seen, but marrow and gastrointestinal toxicity developed at 1.2 X 10(-7)M adriamycin serum level. Adriamycin disappearance curves after ia and iv therapy were similar for similar bilirubin levels, and prolonged with hyperbilirubinemia. Ascites fluid did not accumulate detectable adriamycin. Pharmacokinetics are described in this report.

Increased frequency of HLA-DRW4 in chronic active hepatitis.

HLA-A, B, C and DRw typing was performed on peripheral blood lymphocytes of 17 adults with the diagnosis of chronic active hepatitis made by liver biopsy as the only criterion for study. An increase in the frequency of HLA-DRw4 (71 vs. 24%, p less than 0.005) was observed, but there was no increased frequency of HLA-A1, B8 or any other HLA locus specificity.

The uses of multiple causes of death data to clarify changing patterns of cirrhosis mortality in Massachusetts.

Evaluation of death rates from cirrhosis of the liver in Massachusetts since 1950 indicate a changing pattern suggestive of a substantial increase in the frequency with which cirrhosis is being recorded as the underlying cause of death. By comparing the relationship between the frequency with which cirrhosis is mentioned on the death certificate with its occurrence as the underlying cause of death, it would appear that the increase in death rate represents a real change. Additional evidence is presented to indicated that the increase is occuring in the urban center of the state. Males aged 40-49 and females aged 50-59 have shown the greatest changes. In spite of similar changes occuring on the national scene, the increase appears to be substantially greater in Massachusetts. The evaluation of multiple cause of death as mentioned on death certificates can allow for assessment of significant co-factors contributing to a fatal outcome in a disease like cirrhosis of the liver.

Rapid screening test for detecting hepatitis-associated antigen.

Hepatitis-associated antigen can be detected within 2 hours by using an electrophoretic technique and cellulose acetate membranes saturated with antibody. The speed of the technique now allows testing of blood intended for transfusion on the day of collection, and the sensitivity of the method compares favorably with standard immunodiffusion.
