[Anesthesia using methohexital for thermocoagulation of the gasserian ganglion (author's transl)].

The authors report their experience of the treatment of essential trigeminal neuralgia by percutaneous thermocoagulation of the Gasserian ganglion (400 patients). This method has always been performed on anesthetized patients, using an ultra short acting barbiturate: methohexital. Most often, two or three successive coagulations are needed to obtain the desired analgesia. No complications have resulted, although 30 p. cent of our patients were over 70 years old.

[The technique of anesthesia for the prevention and treatment of gaseous embolism in the neurosurgical sitting position (author's transl)].

The sitting position in neurosurgery gives always three problems for the anaesthesist: --maintain the cerebral perfusion pressure, --not to aggravate intracranial hypertension, --detect gaseous embolism. The clinical signs of air embolism often look like the awakening of the patient. That is why it is necessary to use specific methods to detect them: Doppler, Swan-Ganz catheter, and/or continuous CO2 recording of expired air by capnograph. Among the various methods to prevent air embolism we have used controlled respiration with a positive and expiratory pressure (P.E.E.P.). In our series (45 operations) gas embolism occurred in 13% of the patients operated upon without positive and expiratory pressure, and 0% with P.E.E.P.

Relevance of antibiotics in the treatment of brain abscesses. Report of a case with eight simultaneous brain abscesses treated and cured medically.

A case of eight simultaneous brain abscesses in the right cerebral hemisphere, treated medically after puncture of two of them, is reported. Antibiotics and hypertonic mannitol and, after seven days, dexamethasone were sufficient to cure these lesions. CT scan was of primary importance to follow the evolution under treatment. To get the best efficiency from antibiotics, identification of the microorganism and assessment of its resistance to antibiotics are necessary. It is suggested that in certain conditions medical therapy might be sufficient to treat cerebral abscess, after simple puncture to isolate the infecting agent. Corticosteroids should be avoided in the acute phase because they prevent antibiotics from penetrating the abscesses.

[Current data on pain pathways].

1--The classical notion attributing the conduction of painful messages to only the A delta and C fibres will be discussed. 2--Spinal cord cells of layers I, V and VIII received painful messages and their physiological properties will be compared. 3--The origin within the cell layers of the spinal cord of the various ascending bundles will be examined. 4--The reticular an thalamic zones (posterior ventra nucleus, mid-line and intralaminar nuclei, GMme posterior group in particular) which receive painful messages have also been studied using activation provoked by pinching, pinprick, the injection of painful substances and stimulation of the dental pulp. The possible role of the different structures thereby brought into action and the limitation of these techniques will be discussed. 5--The existence of metameric controls and controls of central origin active at a spinal, reticular and thalamic level. The absence of inhibitory controls may be responsible for the development of abnormal painful sensations which accompany certain deafferentation procedures. The reestablishment of these controls with the aid of stimulation may result and often results in the improvement of these painful syndromes. 6--Section of the dorsal routes in the chronic animal (rat and cat) may represent an experimental model for deafferentation pain seen in man.

[Investigation of the immunity status towards tetanus of a population of mechanics at the car-factory "Renault" (author's transl)].

Tetanus immunity has been studied in a population of mechanics working at the car-factory "Renault". For the study, 283 individuals were divided into 3 groups. The first group consisted of non-vaccined individuals, the second of vaccinated individuals and the third of individuals who had been boostered. The influence of different parameters on tetanus immunity status was considered, i. e. the age of the subjects, the time at which boostering was given, the serotherapy (when it was administered) and the contact with machine lubricating and cooling fluids. Clostridium tetani bacilli and spores were frequently found in aqueous machine fluids. Their presence is a potential hazard for non-vaccinated mechanics coming to contact with the fluids. Tetanus antibody levels in the sera of the tested population were determined in vivo by the toxin neutralization test. The influence on tetanus immunity of the different parameters considered in this paper was subjected to statistical analysis. From the whole population, 27% of the individuals were found to have no protection against tetanus. When age was taken into account, 53% of the individuals over 45 years old and 10% of those under 45 years old were shown to be devoid of tetanus immunity. It could be shown that younger individuals were better protected than older ones, because the formers had been immunized with adsorbed tetanus toxoid and most of the latters with fluid toxoid. Of the individuals in the third group who had received a booster injection within 15 years after primo-vaccination, 98% showed protective tetanus antibody levels in their sera in contrast to 25% when boostering had occurred more than 15 years after primo-vaccination. Contact with machine fluids was found to influence the degree of immunity of only those individuals whose boostering dated back to more than 25 years. Unexpectedly, 3 mechanics seemed to develop immunity after coming into contact with machine fluids. From the results reported here, it is concluded that tetanus immunity in vaccinated individuals should be renewed by a compulsory booster injection given every 5, 10 or, at the minimum, 15 years. Moreover, high-risk populations such as that represented by the mechanics should be immunized or boostered on commencing employment.

[Lumbar phlebography. Normal aspect and its value in diagnosis of herniated disk].

Our technique of lumbar phlebography involves selective catheterisation of the lateral sacral veins with or without catheterisation of the ascending lumbar veins. Injection of these veins, in association with abdominal compression, produces good filling of the intra-spinal epidural veins, particularly at L5-S1 level. The anatomy and radiological appearances of the epidural veins are discussed. The phlebographic signs of disc herniation are presented. The diagnostic potential of this technique seems very promising in disc pathology, especially in the diagnosis of L5-S1 herniation and lateral prolapse at all levels.

[Treatment of essential facial neuralgia by selective thermocoagulation of Gasser's ganglion].

The authors report their experience in the treatment of essential facial neuralgia by differential thermocoagulation of the gasserian ganglion. This method is based upon the fact that small pain fibres (A delta and C) are more sensitive to warmth than large touch fibres. Thus progressive and controlled thermocoagulation of the trigeminal ganglion makes it possible to obtain selective destruction of the small fibres (analgesia without anaesthesia). 76 patients were treated using the method, and in 75 cases the symptoms of neuralgia disappeared immediately. Mortality is nil and morbidity insignificant other than the effects on the trigeminal nerve. The most serious complication (Approximately 10% of cases) was that of marked hypoaesthesia, rather than pure analgesia, in the coagulated area.

[External temporal biopsy in a case of infiltrating limbic glioma: study of astrocytic membranous wrapping whorls (author's transl)].

An intracranial hypertension was found at autopsy to be due to a limbic infiltrating glioma. A cortical temporal biopsy had been done 14 months before, whose ultrastructural results are reported. Membranous whorls, made of two dense membranes with a variable separative space are seen around neurones, dendrites, axons, synapses and oligoglial cells; they derived from astrocytes and are joined by 2 types of cell junctions: gap-junctions and puncta adherentia. Relations between reactive and tumoral gliosis are discussed.
