tcpO2, arterial blood flow and lactate/pyruvate modifications induced by a single dose of naftidrofuryl IV during stage III lower-limb arteriopathies.

A single dose of 600 mg of naftidrofuryl perfused IV during three hours in 12 stage III arteriopathic patients did not modify the average blood flow at the calf but did significantly increase transcutaneous oxygen pressure (33 vs 52 mmHg) and also improved lactates (1.44 vs 0.88 mmol/liter), pyruvate (59 vs 79 mumol/liter), and lactates-pyruvates ratio (33.48 vs 20.92).

How to join and participate in a medical corporation.

Some nurse practitioners are becoming shareholders of medical corporations. This article discusses preliminary plans and specific areas to consider before joining a medical corporation. Information is also presented on precautions that the NP should consider taking before becoming a shareholder in a medical corporation, and the benefits the NP can expect from such involvement.

Tuberculosis as an endemic and nosocomial infection among the elderly in nursing homes.

We studied reactivity to tuberculin skin testing in nearly all nursing home residents in Arkansas. Only 12 per cent of the 12,196 newly admitted residents were tuberculin positive, as compared with 20.8 per cent of the 13,441 residents who were first tested more than a month (mean, 30 months) after admission. The proportion of persons who were positive on initial testing varied greatly with the time spent in the home before testing. Those who were not reactive on initial testing had a 5 per cent rate of conversion for each year spent in a home with a known recent infectious case (within three years) and a 3.5 per cent rate for each year in a home with no recognized recent case. Active tuberculosis developed in only 1 of 534 persons with positive tuberculin tests or previous reactions who were treated with isoniazid, but in 79 (2.4 per cent) of 3270 persons who were not (P less than 0.001). The disease developed in only 1 (0.16 per cent) of 605 persons whose tests converted to positive and who were treated with isoniazid, as compared with 45 (5.9 per cent) of 757 whose tests converted but who were not treated (P less than 0.001). We conclude that new infection with tuberculosis is an important risk for nursing home patients and that greater care should be taken to detect and treat new infections before the disease develops and the infection spreads.

Congenital posthemorrhagic hydrocephalus: report of a case.

A newborn infant with hydrocephalus was found to have residua of subependymal-intraventricular hemorrhage and associated obliterative arachnoiditis. This process is well known in the postnatal period but not as an in utero event. The present article documents antenatal intraventricular hemorrhage as a cause of hydrocephalus.

DNA packaging in mouse spermatids. Synthesis of protamine variants and four transition proteins.

A comparison of the protein compositions of mouse late-step spermatids and cauda epididymal sperm has revealed that the relative distribution of the two amino acid sequence variants of mouse protamine differ markedly in spermatids and sperm. Sonication-resistant spermatids contain the two variants in a ratio of 1:1, while the ratio of these two proteins in cauda epididymal sperm is approx. 2:1. Labeling studies in vivo have shown that this difference is due, in part, to an asynchrony in the time of synthesis of the two protamine variants. Both proteins are synthesized in late-step spermatids, but synthesis of the tyrosine variant in sperm chromatin begins approximately one day before synthesis of the more predominant histidine variant. Analyses of the time of synthesis of protamine and the four transition proteins in late-step spermatids allowed us to estimate the spermatid stage in which these proteins are deposited on DNA and relate these events to the onset of sonication resistance in maturing spermatids. These results indicate that: (1) synthesis and deposition of protamine begins coincident with the onset of sonication resistance in early step 12 spermatids; (2) protamine deposition is complete by mid-step 15; and (3) synthesis of the transition proteins occurs coincident with protamine synthesis.

Psychosocial morbidity in the first three months following stoma surgery.

One hundred and six consecutive subjects were seen in hospital a few days after undergoing surgery for bowel disease that necessitated the formation of a stoma. They consisted of patients with cancer (74), colitis (17) and diverticular disease (15). At this initial assessment details of psychological and social functioning were obtained for the preceding three months before operation. At 3 months post-operatively the subjects were interviewed in their own homes, 87 subjects were available (12 died, 7 refused) and again psychological and social assessments were made. Seventeen per cent of males and 19% of females had moderate or severe psychiatric disturbance and there was also a significant number of patients with various social disturbances. Physical diagnosis did not significantly affect psychiatric outcome. There was, however, high pre-operative psychiatric disturbance and the relevance of this in assessing post-operative symptoms is discussed.

Bacterial air counts obtained with a centrifugal (RCS) sampler and a slit sampler--the influence of aerosols.

Samples of air collected from an air-conditioned hospital room with a 'Biotest RCS' centrifugal air sampler yielded (assuming a sampling rate of 40 l/min) air counts that were approximately twice those obtained with a modified conventional ' Casella ' slit sampler. However, for samples taken during the generation of an aerosol of spores of Bacillus globigii, the slit sampler gave counts that were apparently three- to tenfold higher than those with the centrifugal sampler. For a few minutes after switching off the nebulizer, higher counts were still obtained with the slit sampler, but then the centrifugal sampler again gave consistently higher counts. Our results illustrate that attempts to measure the number of bacteria in air are influenced by the size of the bacterial particles and the sampler that is used.

Response to indomethacin and hydrochlorothiazide in nephrogenic diabetes insipidus.

Four boys with classical nephrogenic diabetes insipidus have been treated by indomethacin, by hydrochlorothiazide, and by the combination of indomethacin and hydrochlorothiazide. Hydrochlorothiazide treatment was slightly more effective than indomethacin treatment in reducing the urine volume and increasing the urine osmolality. The combination of indomethacin and hydrochlorothiazide appeared to be additive and especially helpful in infants and young children before the autonomy of drinking.

A neuronal surface glycoprotein associated with the cytoskeleton.

A cytoskeleton-associated glycoprotein of 130-kilodalton molecular mass (GP 130) was purified from a nonionic detergent-insoluble fraction of 10-16-d-old chicken embryo brains. GP 130 is tightly associated with other proteins in actin-containing complexes (Moss, D.J., 1983, Eur. J. Biochem., 135:291-297); thus, pure protein preparations were obtained only after the partial dissociation of the complexes with the zwitterionic detergent, dimethyl dodecyl glycine (EMPIGEN BB), followed by ion-exchange chromatography and electrophoresis on preparative SDS polyacrylamide gels. Specific monoclonal and polyclonal antibodies were raised to GP 130 and used to examine its distribution in the developing nervous system. Experiments with these antibodies revealed that GP 130 is confined to nervous tissue and is restricted to the surface of neurons in cultures derived from both the central and peripheral nervous systems. This novel glycoprotein is immunologically unrelated to the neuronal cell adhesion molecule (N-CAM), or to vinculin, a protein of similar molecular mass which has been suggested to link actin filaments to the plasma membrane. In the developing chicken embryo brain, GP 130 is first detectable around day 8 after fertilization and increases to approximately 50% of its adult level by embryonal day 13. In contrast, no increase is observed over a similar developmental period in sciatic nerve. In the adult chicken, GP 130 is most abundant in brain and has a particularly high content in areas rich in dendrites and synapses.
