[Experience with echocardiographic examination of patients with left bundle branch block under intensive care].

In the 3 and a half year patient material of their Intensive Care Unit the authors studied the diagnostic value of the echocardiography in left bundle branch block. From the 925 patients treated this period they analysed retrospectively the clinical data of 51 patients with left bundle branch block. In 37 cases acute myocardial infarction was suspected (group A), 14 patients were hospitalized because of severe left ventricular dysfunction without chest pain (group B). The echocardiography was performed always within 48 hours after admission. In 18/37 respectively 8/14 cases the left bundle branch block had already been known from the previous examinations. In the group A the enzyme test verified acute myocardial infarction in 23 patients, in each of them segmental wall motion abnormality--either akinesis or dyskinesis--was detected by echocardiography. In group B 5 dilated cardiac diseases, 1 aortic and 2 mitral vitiums were verified by echocardiography. From the 6 patients of the group B the wall motion abnormalities were caused by acute myocardial infarction in 1 patient and by a previous myocardial infarction in 5 patients. At the Intensive Care Unit 8, and during the further observation in hospital 5 patients died. In each cases the section reinforced the findings of the echocardiography. According to the authors' experiences, echocardiography is proved to be a very useful help in the acute cardiac care of patients with left bundle branch block.

Endoscopic clip-knot suturing technique: preliminary report of application in retroperitoneal ureterolithotomies.

A newly developed laparoscopic knot-substituting technique is presented and applied in cases of retroperitoneal laparoscopic ureterolithotomies. The authors substituted the difficult laparoscopic knotting techniques with the technique of using one or two clips to hold the sutures tight, which shortens and simplifies the endoscopic suturing. This technique is a great advantage, mainly in cases of major reconstructive laparoscopic operations. The technique and advantages of retroperitoneoscopy are discussed as well.

[A case of successful operation of left atrial myxoma causing cerebral embolism].

The authors describe the case report of a young woman in whom a left atrial myxoma causing cerebral embolism was diagnosed by echocardiography. On the ninth day after the development of neurologic symptoms the tumor was excised by an emergency intervention. After the operation the condition of the patient improved considerably. In connection with the case the authors underline the role of echocardiography in the detection of sources of embolism.

The effect of dimethoate, dichlorvos, and parathion-methyl on bone marrow cell chromosomes of rats in subchronic experiments in vivo.

The three organophosphorous insecticides dimethoate, dichlorvos, and parathion-methyl were investigated in subchronic experiments on bone marrow cell chromosomes. In the literature these compounds were reported to exhibit both positive and negative results in mutagenicity tests demanding further investigations in subchronic tests. The treatment of different groups of male Wistar rats lasted for 6 weeks with 5 treatment days per week at doses of 1/100, 1/75, and 1/50 of the LD50. Following the last treatment, bone marrow cell chromosomes were prepared. The frequency of cells revealing any aberrations as well as numeric and structural aberrations were evaluated. In this test both dimethoate and dichlorvos demonstrated mutagenic effects following subchronic treatment of Wistar rats, while parathion-methyl at doses of 1/100, 1/75, and 1/50 of LD50 displayed no significant mutagenicity.

[Percutaneous nephropexy].

We present a new, minimally invasive method for the treatment of nephroptosis. We are suggesting the more spreaded use of this method after the good results we found during the long term observation of our 40 patients with percutaneous nephropexy.

Laparoscopically controlled and assisted percutaneous transperitoneal nephrolithotomy in a pelvic dystopic kidney.

Two renal pelvic stones were removed percutaneously from a pelvic dystopic kidney using a transabdominal, transperitoneal approach. The puncture was made antegrade and controlled partly fluoroscopically and partly laparoscopically. Where the kidney was covered by bowels, these were displaced with forceps inserted through another laparoscopic trocar. The dilation and stone removal were performed traditionally. As the kidney was just behind the uterus, the authors considered extracorporeal lithotripsy contraindicated in this case, so the only minimally invasive resolution was percutaneous nephrolithotomy, which could be performed only with the aid of laparoscopy.

[Percutaneous transhepatic cholecysto-lithotripsy].

Authors were the first in Hungary to perform the percutaneous transhepatic cholecystolithotripsy for the removing of gallbladder stones. The method can be performed easily with the necessary skill and it is not too troublesome for the patients. They present their beginning experience.

Percutaneous cholecysto-lithotripsy.

Experience with the first 40 cases of percutaneous transhepatic cholecystolithotripsy in Hungary are reported. The indications and contraindications, results and complications of endoscopic gallstone removal performed with good results are analysed. The follow-up of patients after PTCL is considered important.

[7 cases of polyarteritis nodosa].

The recognition of polyarteriitis nodosa is a difficult task for the clinician. The extraordinarily varied symptoms of this illness involve mostly general complaints and the mostley combination of some organic symptoms. In their histologically proved seven cases the most frequent original clinical symptoms were the long lasting feverish condition, the loss of body weight as well as numbness and pain of limbs. The anamnesis of three patients involved the asthma bronchiale. The laboratory findings were characterized first of all by an accelerated erythrocyte sedimentation rate and in four cases anaemia has developed. In four cases eosinophilia and in two ones high blood-pressure were observed. The disease-process of their patients is varying; a 30 year old man's death was caused right after the admission to the hospital by an acute myocardial infarction.

[Endoscopic removal of urinary calculi in children].

In 55 children less than 14 years old urinary calculi have been removed endoscopically. In larger stones the ultrasonic lithotripsy has been used. Due to our experience and a review of the literature it was found that the majority of urinary calculi in children could be removed endoscopically. The open operation should be used in exceptional cases. Problems, results and complications of the endoscopic approach are described in detail.

[Endoscopic removal of urinary calculi in children].

The authors present their experience with the endoscopic kidney stone removal in pediatric patients. They treated 55 children under 14 with this method. If the stone was large they used ultrasonic disintegration. On the basis of their own experience and the technical literature they assert that most of the pediatric kidney stones are removable in this way and there is no reason for the existence of open surgery only in exceptional cases. They detail the difficulties of the performance, their results and complications. On the basis of these they emphasize that the aversion from the endoscopic kidney stone removal in pediatric patients--if someone has the necessary skill--is causeless.

Ultrasonic destruction and endoscopic removal of kidney, ureteral, bladder and urethral stones.

Endoscopic removal of renal calculi was introduced at our department in 1984, first in Hungary and East-Europe. Renal, ureteral, bladder and urethral calculi are all endoscopically removed partially following ultrasonic destruction. The procedure is similarly used in cases of solitary kidney or of kidneys with staghorn calculi. Several hundreds of successful cases have proved the necessity and importance of the procedure.

Aujeszky's disease of sheep: experimental studies on the excretion and horizontal transmission of the virus.

Five 5-month-old merino lambs were nasally inoculated with 10(5.0) TCID50 of Aujeszky's disease virus (ADV). The dynamics of virus excretion in the nasal discharges--in agreement with the histologic findings--indicated that ADV also replicates in extraneural sites, in the upper and lower parts of the respiratory tract. The virus was excreted continuously in the nasal discharges, even during the incubation period. The titres, with certain fluctuations, increased gradually up to the final stage of the fatal disease. Following the onset of the clinical disease, the titre of excreted virus (ranging from 10(4.0) to 10(6.0) TCID50/0.1 ml) was comparable with the ADV content found in the nasal discharge of naturally infected piglets. However, the horizontal transmission of ADV to contact lambs failed.
