Hepatocellular carcinoma in the U.S.A., etiologic considerations. Localization of hepatitis B antigens.

The serologic and tissue markers of hepatitis B virus (HBV) were studied in 50 patients in whom hepatocellular carcinoma (HCC) was confirmed at autopsy. Serologic and tissue markers included serum hepatitis B surface antigen (HBsAg), tissue HBsAg, tissue hepatitis core antigen (HBcAg), and serum antibody to HBcAg (anti-HBc). Twenty-two patients had HCC arising in alcoholic cirrhosis; 2 of the 22 (9.1%) had one or more of the HBV tissue and serologic markers. This infection rate is similar to the rate of 7.9% observed in 63 control alcoholic cirrhotic patients without HCC. In contrast, 15 of 20 (75.0%) patients with HCC in nonalcoholic chronic active liver disease showed evidence of active HBV infection. One of 8 patients with HCC in normal liver had serum HBV markers. This result indicates that there is an extremely high prevalence of HBV infection among HCC patients with nonalcoholic chronic liver disease in the U.S.A. The prevalence of HBV infection in these patients is as high as that observed in Asia and Africa. Thus, it can be concluded that the lower prevalence rate of active HBV infection in HCC patients in the U.S.A. is the result of statistical dilution of HCC-B-viral disease by the large numbers of the alcoholic cirrhotic patients with HCC, and that if chronic active hepatitis type B were as common in the United States as it is in Africa and Asia, the frequency of occurrence of HCC might also be as high.

An immunoperoxidase technic for the demonstration of the hepatitis B surface antigen in human livers.

A sensitive method for demonstrating the site of hepatitis B surface antigen (HBsAg) in fixed tissues embedded in either paraffin or araldite is described. The method employs the peroxidase-rabbit antiperoxidase linkage through goat antirabbit to rabbit anti-HBsAg. In staining hepatitis antigen in agar, comparison of fixation (using three common fixatives) with unfixed precipitation arcs revealed no recognizable differences in antigenicity induced by fixation. The method allows confirmation of positive reaction by appropriate blocking controls. The technic is compared with the orcein stain of Shikata and found to be somewhat more sensitive but slightly more time-consuming.

Hepatocellular carcinoma presenting as acute spontaneous haemothorax.

Hepatocellular carcinoma may present in various ways, but only a few instances present with symptoms of distant metastases. This report describes a case of hepatocellular carcinoma presenting with acute haemothorax and haemorrhagic shock secondary to spontaneous rupture of chest wall metastasis. Hepatocellular carcinoma is a frequent malignancy in Asia and should be considered in the list of differential diagnosis of spontaneous haemothorax.

The expression of hepatocyte growth factor (HGF) and interleukin 6 (IL-6) in damaged human liver and kidney tissues.

BACKGROUND/AIMS: Hepatocyte growth factor (HGF) is a multipotent factor involved in tissue regeneration, tumor invasion and a lot of cellular repair processes. Recent studies revealed that interleukin 6 (IL-6) level was elevated in damaged human liver and kidney tissues. Since IL-6 is the major cytokine involved in cellular response to tissue injuries, its relationship with HGF in damaged human liver and kidney is suspected. METHODOLOGY: We examined the RNA transcripts of HGF and IL-6 in 16 hepatic and 5 renal tumor tissues using in situ hybridization techniques. RESULTS: Either HGF or IL-6 RNA transcripts was prominently detected in 86% of the samples (14 liver and 4 kidney). Intense signals were found in malignant cells. In liver tissues, intense signals were detected at the rim of regeneration, while in kidney tissues, signals were found in tubular and glomerular epithelium. Both HGF and IL-6 RNA transcripts were simultaneously detected in 11 hepatic (69%) and 4 renal (80%) samples. CONCLUSIONS: The coexistence of the HGF and IL-6 transcripts suggests a close relationship of two cellular factors possibly with a complex genetic regulation.

The growth and behaviour of pig retinal pigment epithelial cells in culture.

The retinal pigment epithelium contains melanin and lipofuscin. It is believed that in the in vivo system, the incomplete degradation of phagocytosed outer segment discs leads to the formation of lipofuscin. Our results showed that pig RPE cells can be successfully cultured using standard culture techniques without addition of specific growth factors. In this system, the autofluorescent material is formed mainly from the degradation of pigment granules. This culture system may provide an excellent model for studying of diseases related to the retina.

Fine structural observations on the human sperm nuclear matrix.

The nucleus of the mammalian sperm is formed after a series of morphological and biochemical changes during spermatogenesis. The human sperm nucleus, after sequential extraction with detergents, nuclease and ammonium sulfate, consists of a fibroskeletal structure which maintains the original nuclear shape. The chromatin-depleted skeleton is formed by thick and thin fibers as well as electron-dense patches of different sizes. These highly branched matrix fibers had average diameters of 35 and 12 nm. Polarization of the fibroskeletal structure is apparent and can be used as a good model to study the function of nuclear matrix in nuclear compartmentation in germ cells.

Strongyloidiasis in a patient with IgA deficiency.

With rising frequency of immuno suppression particularly due to use of various immuno suppressive agents and increasing incidence of human immuno deficiency virus infection, incidence of opportunistic infection due to various parasites are also on the rise. Persistent strongyloides stercoralis infection which normally also infects healthy individuals is documented in a case of hypogammaglobulinemia reported in the present paper emphasizing it to be another potential opportunistic infection which one may encounter in near future.

Electrophoretic analysis of nuclear matrix proteins in human hepatocellular carcinoma.

The nuclear matrix is the non-chromatin skeleton of the nucleus. This structure contributes to the shape of the nucleus and regulates various nuclear functions. In this study, nuclear matrix proteins of human normal liver, a liver cancer cell line, HepG2, and hepatocellular carcinomas (HCC) were investigated. Using high resolution two-dimensional polyacrylamide gel electrophoresis, the nuclear matrix proteins of 3 normal liver and 14 HCC were compared and contrasted. A high degree of similarity between normal liver, HepG2, and HCC nuclear matrix protein patterns was found. Two HCC specific nuclear matrix proteins were identified. Among these, one protein (HCC-1, Mr 62 kd, pI 5.3) appeared in all tumor samples and HCC-2 (Mr 33.25, pI 5.3-5.5) was present in 9/11 tumors, but absent in normal liver and HepG2. Our results indicate the presence of HCC specific nuclear matrix proteins. These matrix proteins may be used as markers for HCC.
