A comparative study of the efficiency of hydroxyprogesterone caproate and of chlormadinone acetate in the prevention of premature labor.

The efficacy of caproate of hydroxyprogesterone and acetate of chlormadinone in preventing premature labore was compared in a controlled trial. The survey was based on 211 pregnant women with a high risk of premature delivery discovered during clinical examination. There are no significant differences between the two groups in either length of gestation, delay between the beginning of treatment and delivery or other parameters related to prematurity. The absence of evidence suggesting any significant difference between the two treatments can have three possible causes (which are discussed): the methodology, the inefficacy of the two products or the equivalent efficacy of the two products.

[Value of the study of biological autoimmunity in gyneco-obstetrical pathology].

The relationship between autoimmunity and obstetrical pathology is reviewed in the light of data from the literature and of the authors' own experience. The link, already demonstrated, between recurrent spontaneous abortion and the risk factor represented by antibodies directed against phospholipids (circulating anticoagulant antibodies of the lupus type, anticordiolipin antibody) is described, and the pathogenetic theories concerning these antibodies are presented, together with the treatments used for routine prevention of the foetal and maternal risk (corticosteroids, anti-platelet drugs, heparin). The results of screening for antinuclear antibodies in 354 non-pregnant women are reported. Antinuclear antibodies were present in titers of 1/100 in 4.2 percent and 1/50 in 8.7 percent of these women. In the same category of women the frequency of antithyroid antibodies was as high as 12 percent. These data should encourage routine examination for auto-antibodies before and during pregnancy in order to evaluate and prevent the obstetrical risk to which these women are exposed.

[Concept of risk in perinatology: the obstetrician point of view (author's transl)].

Perinatal risks involve the life and health of the mother and baby. It is possible to define both medical and socio-cultural risk factors. They are frequently associated. The surveillance of pregnancy is modulated by the clinician according to his preception of these factors. One essential medical problem is to define the field of application of some examination or therapeutic procedures: to all patients or to selected groups according to clinical criteria. There is no general rule. Some types of surveillance such as serodiagnosis of rubella or toxoplasmosis, or the electronic monitoring of labour involve all women. At the opposite, progestative treatment must be given only to patients at high risk of premature labour. From the public health point of view, it doesn't seem realistic to implement different policies according to risk. Surveillance and perinatal care must apply to all pregnant women. This policy, however, must take into account a fondamental finding of recent surveys, i.e. the deficiency of prenatal surveillance among the most exposed socio-cultural categories and regions, in order to improve information about the rights and duties of the pregnant woman, the concept of risk and how it can be reduced. The role of epidemiological surveys in defining risk factors appears fundamental. Coordination among studies would be desirable. In the future they will probably aid the clinician in real time diagnosis and decision.

[Heartburn and regurgitation in pregnancy. Efficacy and innocuousness of treatment with Gaviscon suspension].

This open trial was conducted in 50 pregnant women, presenting during the 2nd and 3rd trimesters of their pregnancy typical symptoms of gastro-esophageal reflux (pyrosis, regurgitations, retro-sternal burning sensations, dyspepsia, epigastric burning). The treatment with Gaviscon, suspension to drink, lasted 1 month at a dose of 2 tablespoons, 4 times/day after meals and in the evening at bedtime. After one month, all symptoms were improved, in a statistically significant fashion, regarding their frequency, intensity and duration. The efficacy is considered positive in 98 p. cent of the cases; the patient's impression is favorable in 70 p. cent of the cases. Tolerance is excellent and the medication is satisfactorily accepted.

[A study of the comparative efficiency of ifenprodil and ritodrine in the treatment of threatened premature labour (author's transl)].

The study compares a sequential test of a beta mimetic drug (ritodrine) and an alpha inhibitor (ifenoprodil) to show that the beta mimetic drug is superior in treating threatened premature labour, because it has a success rate of 90 per cent whereas the second has a success rate of 45 per cent. Ritodrine produces secondary effects in 69 per cent of cases.
