[Noise from a car airbag as a cause of acute acoustic trauma].

A case of acute acoustic trauma in 32 year male, caused by a noise from car airbag, was described. The symptoms occurring directly after the airbag shot were tinnitus and hearing loss in right ear. Patient was admitted to ENT Clinic in Gdańsk just after 5 months after an accident. Treatment--vasodilatators and oxygen hyperbaric therapy--was ineffective. Review of literature, concerning the side effects of car airbag shot, was made. A mechanism of airbag action was presented as a source of short time noise.

[Malignant ameloblastoma of the maxilla as an interdisciplinary problem].

The clinico-pathological assessment of three cases of rare dentogenic tumours of the maxilla was made (including one case described earlier). In two patients primary malignant ameloblastoma was diagnosed. In one patient benign ameloblastoma was treated few times; in this case we have observed the transformation of benign disease to malignant because the occurring lymph node metastases. The authors discussed clinical and morphological characteristics of these tumours, which are under interest of many specialists.

[Cranial osteomas of atypical localization].

Osteomas have a very characteristic localisation and frequency of occurrence. Based on a representative materials the authors present literature review in which the sequence of occurrence of osteomas in paranasal sinuses is described. A very rare occurrence of osteomas out of the paranasal sinuses is stressed and the hypothesis of development of osteomas in the sinuses and latter migration of osteomas from sinuses to nasal cavities or orbit is mentioned. Three cases of untypical size and localization of osteomas are presented, in two cases the tumors have develop primary outside the paranasal sinuses and in one case the osteoma was present in both frontal sinuses and in the ethmoid labyrinth.

Ventriculosaccular lymphoepithelioma of the larynx: case report and literature review.

BACKGROUND: Lymphoepithelioma is a very rare neoplasm of the larynx. The authors have reviewed the available published literature on the subject and found eight studies on lymphoepithelioma. Lymphoepitheliomas derive from the epithelial and lymphatic tissues of the laryngeal ventricule. The rarity of the occurrence of lymphoepithelioma makes it difficult to define its clinical features and general treatment methods. METHODS: The authors present a description of a lymphoepithelioma located in the left laryngeal sacculus, extending into the pre-epiglottic space and infiltrating the epiglottis with an ulceration of its laryngeal surface. A total laryngectomy and a left radical neck dissection were performed. After surgery, radiotherapy treatment was applied to the left cervical and laryngeal areas. Two years after the initial radiotherapy, the patient died during palliative chemotherapy (for metastases). RESULTS: In patients with lymphoepithelioma of the larynx, the following can be observed: (1) bulging of the laryngeal ventricule, with or without ulceration of the laryngeal surface of the epiglottic mucosa; (2) early lymph node and remote matastases. CONCLUSIONS: In the early stages of lymphoepithelioma, radiotherapy is the method of choice. In advanced cases of lymphoepithelioma, radiotherapy supplements surgical treatment.

[Pathological and clinical assessment of polypoid hypertrophy of the vocal cords].

Polypoid hypertrophy of mucosa of the vocal cord is the most common non-neoplastic disease of the larynx. This hypertrophy has a form of simple polyp of vocal cord or polypoid hypertrophy of the whole length of one or both vocal cords. In most cases a course of the disease is typical. In a few cases we can observe changes in polyp epithelium, i.e. hyperplasia or even dysplasia. Clinical and histological analysis of 809 patients treated because of laryngeal polyps (by Kleinsasser microsurgical procedure) within the period of 1981-1995 at ENT Clinic in Gdańsk was made. Sex and age of the patients, macroscopic picture of the disease as well as treatment and results were assessed. Special attention was paid to the problem of polyp epithelium rebuilding. In a few cases, polyp of the vocal cord should be assessed as praecancerous state.

[Laryngeal papilloma in the material of ENT department, Gdańsk, 1965-1994].

Laryngeal papilloma is a non-malignant epithelial neoplasm of viral etiology. The disease occurs in children and in adults, and- apart from the same microscopic structure--its clinical course is different in those age groups. The authors presented a material of 210 patients with laryngeal papilloma: 76 children and 134 adults. In both groups a clinical analysis with regard to symptomatology, therapy, recurrences, complications and microscopic structure was made. Our observations show that clinical significance of laryngeal papilloma results from the tendency for recurrences and extralaryngeal spread by transmissibility--in childhood, and from the possibility of malignant degeneration--in adults.

[Ocular and orbital symptoms of Wegener's disease].

Wegener's granuloma occurs most commonly in upper respiratory tract, in lungs and in kidneys. In many cases ocular and orbital disturbances may be accompanied by changes in the nose and sinuses. These disturbances occur as: superior orbital fissure syndrome, orbital apex syndrome or ocular fascicle atrophy. The authors present the symptoms of these disturbances in 3 patients treated at Otolaryngological Clinic, Medical University of Gdańsk. On the bases of clinical symptoms and radiological results (CT and MRI) the assessment of pathogenesis of ocular and orbital symptoms was made in each of these cases, with special regard to the mode and result of the treatment.

Benign neoplasms of the larynx.

In a 45-year period (from 1948 to 1992) 291 patients with histologically proved benign neoplasms of the larynx were observed and treated. The most frequently occurring tumors were papillomas, being 95.2% in whole material. In most cases of benign tumors of the larynx intralaryngeal excision of the tumor was used. Clinical data and results of treatment were presented and discussed.

[Extramedullary plasmacytomas of the larynx].

The authors present four cases of solitary extramedullary plasmocytoma of the larynx, a very rare laryngeal tumor, seen in ENT Department, Medical University of Gdańsk in years 1948-1994. They were 0.16% of all malignant laryngeal neoplasms and 23.5% of non-epithelial neoplasms of the larynx in this period. Characteristics for these tumors, epidemiological and clinical features were discussed.

The history of military otolaryngology in Grudzadz from 1920 to 1995 is presented. The organisation of the Otolaryngology Department, successive directors, assistants and volunteers of the Military Hospital are shown.

Prognostic significance of lymph node reactivity in patients with laryngeal carcinoma.

Apart from the presence or absence of metastases in patients with laryngeal carcinoma, there are changes of reactivity in cervical lymph nodes that are seen histologically as nonspecific reactive lymphadenitis. These changes are morphological manifestations of the regional lymphatic system reactivity in relation to tumor, and their presence may be of prognostic value. To test this hypothesis, 45 patients with laryngeal carcinoma were examined for the evidence of node reactivity. The classification applied by Tsakraklides et al. was used, defining patterns of lymphocyte predominance, germinal center appearance (indicating lymph node stimulation) and whether or not lymphocyte depletion had occurred. The presence of node reactivity was compared with long-term (5-year) survivals in the group of patients studied. By applying statistical analysis it was found that lymph node stimulation was a favorable prognostic factor, while its absence worsened prognosis.

[Partial surgery in treatment of laryngeal cancer in the material of ENT department in Gdansk].

Partial operations in treatment of laryngeal cancer in 112 patients was presented. This material was divided on two groups: 14 patients with supraglottic cancer and 98 patients with glottic cancer. Types of operations, their results and failures of treatment was presented and discussed. Special attention was paid to the problem of vocal cord mobility in qualification of the patients to partial operations of the larynx.
