A comparative evaluation of the effects of propafenone and lidocaine on early ventricular arrhythmias after acute myocardial infarction.

A double-blind, placebo-controlled trial comparing the antiarrhythmic effects of lidocaine (given intravenously as a bolus injection of 100 mg followed by an infusion of 2 mg min-1) and propafenone (given as a bolus of 105 mg followed by 300 mg orally every 8 h) was conducted in the first 24 h following acute myocardial infarction. Analysis of ventricular arrhythmias was carried out by Holter recordings. The three treatment groups, propafenone (36 patients), lidocaine (28 patients), and placebo (25 patients), did not differ with respect to age, gender, prevalence of previous infarction, delay from the onset of pain to hospitalization, clinical features on entry (the patients with heart failure or malignant arrhythmias were excluded), site of acute myocardial infarction, or CPK peak. A decrease in the number of ventricular premature beats was noted with lidocaine, but was not statistically significant. The analysis of the first 8 h showed trends suggesting that only lidocaine could suppress complex arrhythmias, couplets and ventricular tachycardia. The drugs were well tolerated. Mean plasma concentrations of propafenone and lidocaine were 517 +/- 464 ng ml-1 and 3.84 +/- 1.10 mg l-1, respectively. In conclusion, this study does not favour the use of propafenone as an alternative to lidocaine therapy during the acute phase of myocardial infarction.

Endometriosis of the small bowel. Case reports and review of the literature.

Endometriosis has been reported with increasing frequency in the literature in recent years, particularly endometriosis in nongynecological sites. Primary endometriosis of the ileum is of particular concern due to the high incidence of small bowel obstruction. The treatment in the past has been castration and resection of the affected bowel, with no guarantee of cure. Two cases are presented which indicate that the hormonal therapy with Danocrine and laser vaporization of endometriotic lesions involving the ileum may alter the natural history of progressive ileal obstruction. A comprehensive review of the literature was undertaken which revealed 204 reported cases of endometriosis of the ileum. New recommendations regarding management of small intestinal endometriosis are proposed.

Children's item and other errors on time-compressed, sequenced rhyming words.

This investigation provided normative data for the performance of 36 M and 36 F children in 3 groups averaging 8, 10, and 12 yrs old, presented time-compressed (TC) rhyme sequences under multitalker masking and no masking conditions. Analysis of both item and order errors yielded significant effects for TC and masking conditions. These effects were consistent with findings for stimuli previously found useful in assessing auditory perceptual problems, thereby supporting the diagnostic potential of these stimuli. Analysis of the order errors also yielded several higher-order interactions which appeared to be clinically negligible but supportive of the suggestion that order errors may be especially sensitive to subtle pathological problems.

[Morphological evidence of synovial spaces in the uncovertebral region of cervical discs (author's transl)].

The existence of synovial differentiation at the lateral aspect of cervical discuss fissurations up to the development of joint-like spaces with synovial lining have been proved in all decades with the aid of a special investigation technique. A considerable individual variability as well as variable differentiations within the same cervical spine did not allow any correlation between the development of synovial spaces and pathological alterations or pathological load of the spine. The morphogenesis of the described synovial spaces has been discussed with respect to morphological and functional causal factors.

Fusarium tricinctum T-2 Toxin Inhibits Auxin-promoted Elongation in Soybean Hypocotyl.

T-2 toxin, a mycotoxin produced by Fusarium tricinctum, inhibited elongation of excised hypocotyl sections of Glycine max var. Hawkeye 63. Auxin-promoted elongation was inhibited more severely than was control elongation, and a 1 hour preincubation of 5 mum toxin prevented the induction of a faster rate of elongation by auxin. While the inhibition of elongation by cytokinin was similar to that of the toxin, the mode of action of the two compounds appeared to be different, i.e. their effects on elongation were additive, and only kinetin promoted radial enlargement. Toxin treatment did not diminish cytokinin-induced radial enlargement. The properties of the plasma membrane, as measured by electrolyte leakage, were not affected by the toxin.

[Vertebral joint ganglion].

We describe a further case of ganglion of the vertebral joint at L4/L5. We give a review of literature about ganglions (synovial cysts) of the vertebral joint and discuss their etiology and their localisation.
