Andrological parameters in patients with varicocele and fertility disorders treated by high ligation of the left spermatic vein.

The results of surgical treatment of 50 subfertile patients with left varicocele are presented. Improvement of the spermiogram was noted as follows: 70% of patients had improvement of semen count, 48% of semen motility and 69.4% in the morphological pattern of spermatozoa. Pregnancy occurred in 50% of patients' wives. Eighty-four percent of the pregnancies took place during the first 12 months after the operation. A direct correlation was observed between the size of the varicocele and the number of pregnancies. Severely oligozoospermic patients with sperm count less than 10 million per milliliter semen constituted 72% of the men whose wives became pregnant. There was no statistically significant change after the operation in levels in the semen of luteinizing hormone, follicle stimulating hormone, testosterone and prolactin.

Hyaluronidase activity in split human semen.

Hyaluronidase is an acrosomal enzyme which participates in the dissolution of the cumulus oophorus matrix, containing hyaluronic acid, and is essential for the fertilization process. In this study hyaluronidase activity was determined in two fractions of split normozoospermic and oligozoospermic human semen. The method consisted of measurements of areas of digestion of hyaluronic acid in agar (Petri dishes) as compared to activity obtained by commercial testicular hyaluronidase. It was found that the first splits of semen, which are generally characterized by higher sperm density and better quality of sperm, exhibit higher enzyme activity (136.0 +/- 11.6-207.0 +/- 15.1 micrograms/mL) as compared to the second splits (117.8 +/- 10-134.0 +/- 12.9 micrograms/mL).

Total and free testosterone and estradiol in human semen.

Total and free testosterone (T) and estradiol (E2) were determined by a radioimmunoassay technique in 30 normozoospermic and oligozoospermic human semen samples. The mean amounts of total T ranged from 25.1 +/- 7.4 to 32.5 +/- 12.2 ng/100 mL, and those of the free hormone from 7.0 +/- 2.8 to 7.9 +/- 2.3 ng/100 mL. The values for E2 were 10.5 +/- 4.4 to 11.2 +/- 4.5 pg/mL and 2.3 +/- 1.1 to 3.2 +/- 2.6 pg/mL, respectively. There was no correlation between sperm density of semen and the levels of T and E2. It is suggested that the nonspecific binding of T and E2 in semen provides a physiological reservoir for active, free T and E2 and that an appropriate ratio of free T to free E2 in semen may be crucial in the processes that lead to fertilization.

Percutaneous nephrolithotomy.

Our initial experience with percutaneous nephrolithotomy in 46 randomly selected patients included 16 who had undergone previous surgery for calculi. A urologist and radiologist cooperated closely in carrying out the procedure, which was performed in one or two stages (16 and 30 cases respectively). Calculi were successfully removed from 80.4% of the patients with the use of grasping forceps or ultrasonic lithotripsy. Operating time ranged from 30 to 180 min and the average hospital stay was 8 days. Complications were few and of a transient nature. Further experience and patient selection is necessary in order to reduce the time and cost of this procedure.

Current experience with hypospadias repair.

Current experience with hypospadias repair in 77 boys during a 1-year period is presented. Thirty-nine with distal hypospadias underwent meatal advancement and glanuloplasty (MAGPI), 18 with more proximal lesion were repaired by Mathieu's flip-flap technique, and in 5 boys with severe chordee and proximal hypospadias, the transverse prepucial island flap repair was employed. Second-stage repair was performed in 10 children, and another 5 boys underwent release of chordee. The complication rate of the group was 18%, most of them fistulae. The current approaches to hypospadias repair are discussed and are supported by them.

Prophylactic effect of triethylenethiophosphoramide on the recurrence rate of superficial bladder tumors.

The prophylactic effect of triethylenethiophosphoramide (thio-TEPA) on the recurrence rate of superficial transitional cell carcinoma of the bladder was examined in three groups of patients. Patients in group 1 had been treated by transurethral resection of the tumor before the study began; thereafter, topical thio-TEPA was added to the therapeutic regimen. The mean recurrence rate in this group dropped from 1 in 11.1 months before treatment with thio-TEPA to 1 in 28.6 months during treatment. Group 2 included patients treated by transurethral resection, with the addition of thio-TEPA instillation as soon as a diagnosis of transitional cell carcinoma of the bladder was established. In this group, the recurrence rate was 1 in 34.6 months. The control group for group 2 (group 3) was treated by transurethral resection of the tumor with no thio-TEPA instillation, and in this group, the recurrence rate was 1 in 11.6 months. These results justify the prophylactic use of thio-TEPA in patients with transitional cell carcinoma of the bladder.

Angiography in the evaluation of carcinoma of the bladder.

Pelvic angiography using the Seldinger technique as a method for more accurate staging of bladder tumors is discussed. This method offers an excellent demonstration of the depth of infiltration and perivesical extension of a tumor. The procedure is considered safe, and involves no technical difficulties. In cases where there has been previous surgery and radiotherapy, there may be some risk of overstaging. We consider this method an extremely valuable tool for staging bladder tumors more accurately, and, therefore, for providing better treatment.

Estimation of recuperation of renal function in obstructive uropathy: an experimental study in rats with the use of renoscan.

In order to obtain an evaluation of the renal function after removal of a cause of obstructive uropathy, the renal scan was experimented in rats. The methods used consisted of the obstruction of the right ureter and of Hippuran131 scanning which was performed weekly. The patency of the ureter was re-established at different times and, in some cases, the left ureter was obstructed during this second operation. Delay and reduction in Hippuran131 uptake was directly related to the duration of the obstruction and recuperation of function was inversely related to the same parameter. The contralateral ureter obstruction caused a significant improvement in right renal function. Recuperation was also observed in rats reoperated on after seven or eight weeks.

Comparative study of experimentally induced benign and atypical hyperplasia in the ventral prostate of different rat strains.

The induction of prostatic lesions in the rat by hormonal manipulation and/or chemical carcinogens in different strains is well documented in literature. However, the selection of a strain for such studies was merely based on the laboratory husbandry facilities rather than on the animal genotype. It is well accepted that the morphology and function of the prostate exhibit a marked species-specific differences that makes extrapolation to other animals and man difficult. Our group has developed an experimental model for the study of rat prostatic hyperplasia and/or dysplasia induced by citral--a flavor constituent. Previous observations have revealed that the Wistar rat ventral prostate is reactive to citral, especially during its adolescent growth period as well as during the redifferentiation stage among postcastrated rats treated with exogenous testosterone. The present study presents data on the ventral prostate susceptibility of three additional strains selected by their interrelated endocrine morbidity. In order to facilitate an objective comparative analysis of the prostatic lesions a histopathological score chart was elaborated. The present findings showed that the Wistar and Sprague-Dawley strains were the most susceptible to developing benign and atypical prostatic hyperplasia both in intact and postcastrated rats. The F344 and ACl/Ztm rat strains remained refractory toward all of these treatments. Our data provide evidence that the strain genotype and, more precisely, their endocrine background, play a determinative role in the prostatic responsiveness towards citral. It is suggested that additional consideration be given to the selection of an adequate animal strain when studying experimental neoplastic transformation, especially when hormonal interactions might be involved.
