Evoked potentials in shoe-workers with minimal polyneuropathy.

The aim of the present research was to investigate early impairments of central nervous system in subjects exposed to a low toxic level of industrial solvents for several years. Therefore a group of fifteen shoe-workers, with symptoms of minimal polyneuropathy, and a group of twenty, sex and age matched, healthy volunteers were selected for multimodal evoked potentials studies. The statistical analysis revealed a decrease of VEP and SEP amplitudes as well as a significant higher percentage of cases with BAEP V/I ratio between 0.5 and 1 among patients. The wrist-Erb's point conduction velocity was, also, found lowered in them. In accordance with the experimental results, the systematic employment of multimodal evoked potentials is proposed for screening the subjects exposed to neurotoxic influence of industrial solvents.

[Post-radiation mandibular fibrous histiocytoma. Apropos of a case and literature review].

Postirradiation sarcoma of the head and neck has rarely been reported. A case of malignant fibrous histiocytoma (M.F.H.) occurring in the mandible is presented. Head and neck region was previously irradiated for Hodgkin's disease. The interval from initial radiation to diagnosis of sarcoma was 10 years. Aggressive treatment with early radical excision is the treatment of choice. In spite of extensive surgery, the patient died one year after treatment with local recurrence.

Production of bacteriolytic activity in the oral cavity by nutritionally variant streptococci.

Microorganisms from the oral flora were examined for the production of bacteriolytic substances. Among human viridans group streptococci, only one group of strains with thiol-dependent properties was shown to secrete enzymes with bacteriolytic activity on heat-killed cells of Micrococcus luteus on double-layer nutrient agar plates. By morphology, culture requirements, and biochemical properties, they were found to conform to descriptions of nutritionally variant streptococci (NVS). Bacteriolytic activity was shown to be a constant property of all of the human oral NVS isolated and a property of some reference strains of NVS from clinical sources. No other known species of viridans group streptococci demonstrated bacteriolytic activity. Analysis of bacteriolytic activity could be a useful tool for both the isolation and identification of this fastidious group of microorganisms.

Advances in evoked potential recording. A historical review.

A brief survey is exposed on the main advances in evoked potential recording techniques, since Caton's paper in 1875. The Berger first EEG records and the Adrian's studies on alpha rhythm, the stimulus-evoked alpha blocking, the K-complex and the vertex response, the effect of attention on the early vertex negativity are briefly outlined. Furtherly the P300 components and stimulus discrimination are remembered. The superimposition and averaging techniques have led to the latest developments in brain evoked potential recordings until to the subcortical far-field auditory and somatosensory recordings. The motor potentials and the latest results with electrical and magnetic coil cortex stimulation are shortly reported.

[Malignant maxillofacial non-Hodgkin's lymphoma. Apropos of 15 cases involving the naso-sinus cavities].

Non-Hodgkin's lymphomas include malignant conditions developing within lymphoid tissue, both lymph nodes and extranodal lymphoid tissue. Due to its rich lymphatic supply, the cervico-facial region is frequently involved: 10 to 15% of all NHL (1, 2, 8, 15), i.e. a similar incidence to primary GI involvement (4). The cervical glands and lymphatic structures of the Waldeyer ring are most often and most classically involved. The nasosinusal cavities may be involved, producing diagnostic problems which have to be resolved by the specialist (6). Between 1972 and 1989, 20 cases of NHL involving the bony cavities of the face were seen at the Centre Antoine-Lacassagne. These include the 15 cases reported below involving the naso-sinusal cavities.

Immature teratoma primary of the uterine cervix. First case report.

The extragonadal germ cell tumours represent 1-2% out of all the germ cell tumors. The retroperitoneal and the mediastinic spaces are the most common localizations. Visceral localizations are very infrequent. We present a case, the first described in literature, of immature teratoma of the uterine cervix in a 13 year old girl.

[Intra-arterial chemotherapy of ORL cancer. Value of an implantable infusion system and scintigraphic monitoring].

The technical difficulties and complications related to arterial catheterisation have inhibited the development of regional chemotherapy in ENT oncology despite the numerous theoretical advantages associated with this method. We report our recent experience with a completely implantable infusion system; 12 systems were inserted, 11 unilateral and 1 bilateral. Treatment was carried out in 8 patients and the perfect biocompatibility of the material was confirmed. Good distribution of the infusion to the tumor zone and lack of cerebral diffusion were confirmed before treatment using an infusion of free Technetium.

[Malignant non-Hodgkin's lymphoma of the bony cavity of the face. Apropos of 17 cases].

Malignant non Hodgkin lymphomas (NHL) are fairly frequent, but involvement of the bony cavities of the face is less common than invasion of the cervical lymph nodes and the lymphoid components of Waldeyer's ring. In connection with a series of 17 personal observations, the authors discuss the main features of NHL and review the diagnostic problems and therapeutic alternatives. Two histologic-clinical entities can be defined: orbital NHL: generally stage I, with a low or intermediate histoprognostic grade and a good prognosis; naso-sinusal NHL: often locally advanced, these lesions are often associated with other visceral disease sites; the prognosis for these intermediate or high histo-prognostic grade lesions is much more somber.

Acute controlled study of the dose-response relationship of fenoterol, ipratropium bromide and their combination.

A double-blind study was carried out in a group of 9 patients with partially reversible airway obstruction, to evaluate the bronchodilator effect of gradually increased doses of fenoterol (cumulative doses: 100, 200, 400, 800 and 1,600 micrograms), ipratropium bromide (40, 80, 160, 320 and 640 micrograms) and a combination of these (100 + 40, 200 + 80 micrograms, etc.), denominated Duovent. The three treatments were carried out at random at the same time on 3 days, usually not consecutive days, whereby 1, 1, 2, 4 and 8 puffs (altogether 1, 2, 4, 8 and 16 doses) of each product were given in identical pressurized aerosols at intervals of 60 min. Respiratory (spirometry and flow-volume curve) and cardiovascular (blood pressure, heart rate) parameters were measured under basal conditions and 60 min after each administration. On average, with all three products, the bronchodilatory effect showed a linear correlation with the logarithm of the dose given. With the same number of puffs the side-effects (tremor in particular) of the combination fenoterol and ipratropium bromide or Duovent (FIB) were markedly fewer than those of fenoterol, while ipratropium bromide was very well tolerated. The three dose-response curves (parallel) show that, in respect of an increase in FEV1, 1 puff of Duovent (100 micrograms fenoterol + 40 micrograms ipratropium bromide) is approximately equal to 2 puffs of fenoterol (200 micrograms) and 4 puffs of ipratropium bromide (160 micrograms). FIB, therefore, makes it possible to maintain the same bronchodilatory effect as the full dose (400 micrograms) of fenoterol while improving the therapeutic value.

Controlled clinical study of a long-term treatment of chronic obstructive lung disease using a combination of fenoterol and ipratropium bromide in aerosol form.

A group of patients suffering from chronic obstructive lung disease, selected according to clinical criteria and respiratory function, were treated daily for at least 84 days using a metered aerosol containing a combination of a beta 2-agonist (fenoterol) and an atropine-like drug (ipratropium bromide). Every 14 days, respiratory and cardiovascular function was measured and analysed statistically. Information was compiled daily by the patients themselves on dyspnoea, cough, peak flow, any increase in dosage over the recommended dose and any side-effects that might have developed. The results obtained demonstrated that the combination of fenoterol and ipratropium bromide produced clear improvements in respiratory function and symptomatology throughout the observation period. The drug was well tolerated by the majority of patients.

The use of pigtail catheters for retrieval of foreign bodies from the cardiovascular system.

Snare retrieval of foreign bodies is a common procedure but in certain cases, location of the foreign body requires another system. We present three cases in which only the use of a pigtail catheter gave a good grip for gentle traction and removal or repositioning of the foreign body. A careful review of the world literature up to December 1982 gave rise to two previous similar reports. Both our experience and that found in the literature seem to suggest that in cases where there are no free ends to snare, pigtail catheters may be the first instrument used to start retrieval.

Erroneous interpretation of pulmonary capillary wedge pressure in massive pulmonary embolism.

Pulmonary capillary wedge pressure (WP) values obtained after catheterization of a pulmonary artery (PA) totally occluded by thrombus may be erroneously interpreted. The left lower lobe PA was catheterized in 13 anesthetized dogs. Measurements of mean pulmonary artery pressure (MPAP) and WP were performed before and after total occlusion of the PA by a clamp located 1 cm from the tip of the catheter. There was a difference between pulmonary artery occluded pressure (PAoP), MPAP, and WP, both in normovolemic and overload conditions (p less than .01). The PAoP was not equivalent either to WP or to MPAP. In massive pulmonary embolism, WP may not reflect left ventricular end-diastolic pressure (LVEDP).

One-stage surgical treatment of cardiac and pulmonary echinococcosis.

The case of a 26-year-old woman with cardiac and pulmonary echinococcosis is presented. Surgical treatment of the intrathoracic disease was done in a one-stage operation. Under cardiopulmonary bypass, a left ventricular hydatid cyst and a lower lobe pulmonary cyst were removed. The patient is completely asymptomatic and is leading a normal life two years after operation.
