[Dosage aerosols; problems with the inhalation technic].

The inhalation techniques of 77 patients using a dosing aerosol were videotaped before, immediately after and (in 55 cases) three weeks after they viewed an instruction video tape. The inhalation technique was assessed on 9 points. Before the instruction only 2% had a correct and 49% a completely faulty technique. After instruction, these figures were 65% and 13%, respectively. After three weeks, 12% were found to have regressed to only partially correct use.

The significance of the patient-therapist relationship in the treatment of obsessive-compulsive neurosis.

Results are presented of two studies into the predictive value of the therapeutic relationship in the treatment of 60 and 25 obsessive-compulsive patients. A relationship was found between the therapist's score on the Barret-Lennard Relationship Inventory, assessed after the second and 10th sessions. As far as the patients' scores were concerned, however, a significant correlation with outcome was found only after the 10th session.

Exposure and response prevention in patients with obsessions.

The outpatient treatment of patients with obsessions alone is often said to be less successful compared to the outpatient treatment of patients with both obsessions and compulsive symptoms. This article presents the outcome of the treatment of 26 obsessive patients. The success rate, measured after 10 treatment sessions, was 73%. At follow-up after 12-36 months, improvement appeared to have been maintained or to have increased in 61.5% of the patients, figures comparable to the outcome of patients with both obsessions and compulsions.

Hydrogen breath test in schoolchildren.

The frequency of negative hydrogen breath tests due to colonic bacterial flora which are unable to produce hydrogen was determined after oral lactulose challenge in 98 healthy Dutch schoolchildren. There was a negative result in 9.2%. The probability of a false normal lactose breath test (1:77) was calculated from these results together with those from a separate group of children with lactose malabsorption (also determined by hydrogen breath test). A study of siblings and mothers of subjects with a negative breath test did not show familial clustering of this condition. Faecal incubation tests with various sugars showed an increase in breath hydrogen greater than 100 parts per million in those with a positive breath test while subjects with a negative breath test also had a negative faecal incubation test. The frequency of a false negative hydrogen breath test was higher than previously reported, but this does not affect the superiority of this method of testing over the conventional blood glucose determination.
