Tolerance and effectiveness of recombinant interleukin-2 (r-met Hu IL-2 [ala-125]) and lymphokine-activated killer cells in patients with metastatic solid tumors.

The tolerance of a recombinant human interleukin-2 (rIL-2, r-met Hu IL-2 [ala-125], Ortho Pharmaceutical) and its antitumor effectiveness in combination with lymphokineactivated killer (LAK) cells was examined in 26 patients with metastatic solid tumors, including 14 renal cell carcinomas, seven melanomas, three extragonadal germ cell tumors refractory to chemotherapy and two colon carcinomas. rIL-2 was administered as a bolus at 30,000 U/kg every 8 h on days 1-5 and days 12-19. Leukapheresis was done on days 8-12, lymphocytes were incubated with rIL-2 for 3 or 4 days in vitro and reinfused on days 12, 13 and 15. The mean number of reinfused cells was 5.1 x 10(10) per patient. IL-2 dose and schedule were adjusted according to toxicity. Patients received a median of 100% (range 87-100%) of planned rIL-2 on days 1-5 and a median of 71% (range 13-100%) on days 12-19. Capillary leak syndrome with hypotension and impaired renal function and CNS toxicity were the major reasons for dose modification. Patients with renal cell carcinoma, most of whom underwent a prior nephrectomy, had a reduced tolerance to rIL-2. Partial responses were documented in three renal cell carcinomas and one melanoma. The median response duration was 5.5 (range 1-6) months.

Anticancer activities as well as antiviral and virus-enhancing properties of aqueous fruit extracts from fifty-six European plant species.

Several plant-derived drugs are used in medical oncology today. Since only a small part of the flora has been tested for any kind of bioactivity intensive further screening may be rewarding. Fifty-six plant extracts were studied attempting to explore the feasibility of an assay that screens cytotoxic, antiviral and virus-enhancing activities in the same test. We made use of the property of an avian influenza virus replicating in a human breast cancer cell line. During the first 3 days of the test the cytotoxicity of the extracts was evaluated by phase-contrast microscopy. From the 4th day on when viral cytopathogenic effect became manifest we were able to identify antiviral and virus-enhancing activity among some of those extracts not showing cytotoxicity during the first 3 days of incubation. Aqueous extracts from the fruits of 56 plant species belonging to 22 families were screened. Twelve species exhibited cytotoxic, eight antiviral, five virus-enhancing and 31 no activity. These results show that the replication of a myxovirus in a human tumor cell offers the possibility of screening cytotoxic, antiviral and virus-enhancing activity in the same assay.

Cytostatic activity of commonly used tricyclic antidepressants.

Tricyclic antidepressants (TA) often used in the supportive care of cancer patients exhibit cytostatic properties on human renal cancer cells in vitro. Complete growth inhibition was observed with imipramine and clomipramine at concentrations similar to that of nitrogen mustard. Clinical side effects of TA (cardiotoxicity, agranulocytosis) and their binding to nucleotides may point to a mechanism of action similar to that of anthracyclines. Careful recording of TA applications during cancer chemotherapy or better randomized studies with or without TA should define the antineoplastic influence of TA in cancer chemotherapy.

[Follow up examinations in cytostatic therapy].

There are three main reasons for routine follow-up examinations after cytotoxic chemotherapy in cancer patients: assessment of treatment efficacy; rating of drug side-effects; and detection of relapse. Routine controls can only be efficiently performed with a thorough knowledge of the biology of the tumor and of the therapeutic efficacy of the available cytotoxic drugs. The frequency and type of follow-up examinations depend mainly on the curative or palliative treatment potential. Examples of superfluous controls are mentioned. By prevention of unnecessary examinations the primary care physician could play an important role in cut down of health care costs.

[Immunotherapy of tumors using interleukin-2 and lymphokine-activated killer cells].

The tolerance and effectiveness of an interleukin-2 analog (rIL-2) in combination with lymphokine activated killer (LAK) cells has been studied. 14 patients with metastatic solid tumors, including 8 renal cell carcinomas, 3 melanomas and 3 extragonadal germ cell tumors refractory to standard therapy, were treated for a 3-week period. rIL-2 was administered as a bolus at 30,000 U/kg every 8 hours on days 1-5 and days 12-19. Leukapheresis was done on days 8-12, and lymphocytes were incubated with rIL-2 for 3-4 days in vitro and reinfused on days 12, 13, and 15. rIL-2 dose and schedule were adjusted according to toxicity. The major side effects were constitutional symptoms and a capillary leak syndrome with hypotension and impaired renal function. Partial responses were documented in 2/8 renal cell carcinomas and 1/3 melanomas. The response durations were 1, 5 and 6 months.

Long-term results of two Swiss AML studies.

The Swiss group for clinical cancer research (SAKK) completed two first line protocols for the treatment of acute myelogenous leukemia (AML). In the first protocol (SAKK AML 74, from August 1974 to April 1977; 107 patients) the effectiveness of immunization with viral oncolysate during maintenance treatment was tested. After successful induction treatment, the patients were randomized in two groups: Group A: monthly maintenance chemotherapy for 2 years and group A+IT, which received the same maintenance chemotherapy regimen plus, on day 15, injection of viral oncolysate. Of the 107 patients, 57 (53%) achieved complete remission, 29 then being randomized to group A, 28 to group A+IT. As of August 1984, there is no statistical difference between the survival times of the two groups (p = 0.288). Ten years after the start of the study there is still no clear plateau of the survival curve. Nine percent of all patients (18% of the remission patients) are alive today. With the treatment of the mid-seventies, therefore, the cure of a patient suffering from AML was a rare event. The second protocol (SAKK AML 77, from April 1977 to April 1982; 162 patients) was designed to evaluate the usefulness of a prolonged maintenance treatment after early consolidation. The 74 patients who were still in remission after early consolidation treatment were assigned to either maintenance chemotherapy or to observation only. At 3.5 years after the last patient's entry there was no difference between the groups in duration of survival (p = 0.332).(ABSTRACT TRUNCATED AT 250 WORDS)

Tetracycline in the treatment of malignant effusions: evidence for a cytostatic action of the decomposed drug.

During the in vitro growth of human carcinoma cell lines an inhibition of cell growth in media containing tetracycline was observed at a concentration which was about 1000 times below the one reached in the pleural fluid of malignant effusions after tetracycline instillation. During short time drug-cell contact only previously heated tetracycline showed growth inhibition like doxorubicin, a substance of similar structure and origin. Freshly prepared tetracycline inhibited cell growth only after several minutes of drug cell contact. Our observations in vitro suggest that decomposed tetracycline plays an important role in the control of malignant effusions.

[Effectiveness and toxicity of high-dosage Ara-C. Clinical observations in 10 cases and review of the literature].

Ten patients with acute myeloid leukemia were treated with a high-dose ara-c regimen (3 g twice daily for 6 days). 5 patients (50%) achieved a complete remission lasting for a median duration of 5 months. These patients had all achieved a complete remission following standard treatment with ara-c. High-dose ara-c was ineffective in 2 patients resistant to standard ara-c protocols. The most important side effects of high-dose ara-c were conjunctivitis and cerebellar symptomatology. In one case a severe diffuse encephalopathy with cerebellar predominance set in at a remarkably late stage (45 days after starting treatment). Our results and those of other authors suggest that treatment with high-dose ara-c should be used with caution.

[Cytostatic agents and defense].

The defence mechanisms of the human body against harmful influences from the environment are manifold. The aggressions on the integrity of the organism are physical, chemical, or biological in nature. In this paper the interactions of the commonly used cytotoxic drugs with various defence mechanisms are discussed. Impairment of these defence mechanisms by cytotoxic drugs results in a whole variety of clinical pictures, such as delayed wound healing, secondary malignant neoplasms or many different infectious diseases. These cytotoxic drug-induced diseases are summarized under the term IDDSS (Induced or Iatrogenic Defence Deficiency SyndromeS).
