[Palliative treatment of malignant stenoses of the esophagus and cardia: laser therapy versus laser + high-dose-rate Iridium 192 afterloading therapy. A prospective randomized study].

In order to compare the local efficiency of two different therapy modalities, 37 patients with advanced inoperable tumors of esophagus and cardia were evaluated hitherto in a prospective randomized study. Twenty patients received an exclusive laser treatment, 17 were additionally submitted to three high-dose-rate iridium-192 afterloading irradiations (single dose at the reference point 1 cm radially to the source: 7 Gy). In both groups, 15 patients died whose survival times were not different (125 versus 130 days), however, a beneficial influence was exerted by the afterloading therapy with respect to the duration of the palliative effect (67 versus 28 days) and the hospitalization time (25 versus 48 days).

[Epidemiology of dysplastic nevus].

The dysplastic naevus is regarded as a precursor of malignant melanoma of the skin and has distinctive features that are apparent on clinical and particularly histological examination. Epidemiological studies on the incidence of dysplastic naevi in Europe have not yet been reported. A total number of 500 white males (median age: 19 years) have been examined. Macroscopically dysplastic naevi were detected in 2.2%. This result corresponds to reports from the United States of America and Australia.

Variability in clinical expression of Menkes syndrome.

Six patients with Menkes syndrome are described, who differ from patients with the classical form of Menkes syndrome because of their longer survival; some of them also exhibited a milder manifestation of symptoms. Based on the present data and a summary of seven case reports describing Menkes patients with long survival, it may be possible to divide these patients into two subgroups: one group of severely affected patients with long survival and another group of very mildly affected patients with late onset of symptoms. Perhaps only the latter represents a true subgroup of Menkes syndrome. The possible benefits of copper therapy are discussed.

[Therapy of paroxysmal hypokalemic paralysis: experience with diclofenamid].

A case of familial hypokalemic periodic paralysis is discussed in a boy whose first paralysis occurred at 11 years of age. At first the paralysis was controlled by acetazolamid and potassium therapy. Later, frequent attacks of paralysis and muscle weakness developed necessitating a change of therapy. Diclofenamid has now already been administered for 2 years. It is well tolerated and has suppressed further attacks.

Palliation of obstructive oesophageal and cardial carcinomas with laser and endoluminal iridium radiation.

The combination of laser with endoluminal 192-iridium radiation for the palliation of malignancies of the oesophagus and the cardia has been introduced to overcome the short-lived effect of laser treatment alone. Concluding from early experience, this method seems to prolong the effect of laser treatment alone, with a low risk of complications. However the initial optimistic results, claiming a permanent relief of dysphagia in 80% of the patients, cannot be reproduced under prospective conditions. The combination of laser and 192-iridium radiation with other palliative methods, such as external radiation and/or chemotherapy is feasible and remains to be studied in a comparative way.

Encapsulation of thiosulfate: cyanide sulfurtransferase by mouse erythrocytes.

Murine carrier erythrocytes, prepared by hypotonic dialysis, were employed in the encapsulation of several compounds including [14C]sucrose, [3H]inulin, and bovine thiosulfate:cyanide sulfurtransferase (rhodanese), a mitochondrial enzyme which converts cyanide to thiocyanate. Approximately 30% of the added [14C]sucrose, [3H]inulin, and rhodanese was encapsulated by predialyzed erythrocytes, and a decrease in the mean corpuscular volume and mean corpuscular hemoglobin was observed. In the encapsulation of rhodanese a recovery of 95% of the erythrocytes was achieved and an 85% equilibrium was established. The addition of potassium cyanide (50 mM) to intact, rhodanese-loaded erythrocytes containing sodium thiosulfate resulted in its metabolism to thiocyanate. These results establish the potential use of erythrocytes as biodegradable drug carrier in drug antagonism.

Immunization with synthetic peptides of a Plasmodium falciparum surface antigen induces antimerozoite antibodies.

Polypeptides expressed on the surface of merozoites, the invasive stage of the asexual blood cycle, are good candidates for the development of malaria vaccines. Five synthetic peptides with predetermined specificity deduced from a genomic DNA clone coding for the NH2-terminal portion of the main merozoite surface polypeptide of Plasmodium falciparum were evaluated for their capability to raise antibodies that react with the P. falciparum merozoites. Antibodies induced by two of the peptides (3 and 5) reacted with the membrane surfaces of seven of seven isolates of P. falciparum from different geographic areas. Antibodies against peptide 4, which contains a repeated amino acid sequence (Gly-Gly-Ser and Val-Ala-Ser), reacted with six of seven isolates. Structural analysis of the deduced polypeptides suggests that peptide 3 is exposed at the surface of merozoites. When it was used to immunize monkeys, three of the four animals were partially protected from a challenge infection that induced a fulminant infection in control animals.

[Density of rapid eye movements during active sleep in preterm newborn infants].

Rapid eye movement (REM) density has been helpful as a reliable index of phasic eye movements activity during REM (active) sleep in adults. We evaluated this index in 28 newborns, at 34 to 39 weeks of conceptional age (CA). The 5 hours polysomnographic recording during sleep included electroencephalogram, electrooculogram, electrocardiogram, pneumogram, nasal thermistor for detecting airflow, and continuous oxygen saturation (tcPO2) monitoring. REM density was measured by two distinct criteria: REM density as the duration of phasic eye movements in relation to REM periods (REMP). Such index varied from 10.6 to 14.1% and was higher in the 36-37 weeks CA group. REM density was calculated as a percentage of 10-second epochs of REM sleep in which phasic eye movements occurred. This criterion confirmed the narrow range of such index in this age bracket (39.9-44.6%), was higher in the first REMP and in the 36-37 weeks CA group. Our data suggest that REM density is a consistent phasic REM sleep feature in preterm newborns with levels close to the adult population, in spite of the gradual decrease of REM sleep duration with age. Higher indices were also found in the first REMP and in the 36-37 weeks CA group by both methods.

The primary structure of transcription factor TFIIIA has 12 consecutive repeats.

Analysis of the amino acid sequence of transcription factor TFIIIA from Xenopus laevis reveals the presence of 12 repeating structures, each about 30 residues in length. These segments have been aligned and their secondary structure predicted. The repeats each contain two invariant cysteines and two invariant histidines, perhaps to coordinate a zinc cation. Possible nucleic acid interaction modes are discussed.

Protein normal-mode dynamics: trypsin inhibitor, crambin, ribonuclease and lysozyme.

We have developed a new method for modelling protein dynamics using normal-mode analysis in internal co-ordinates. This method, normal-mode dynamics, is particularly well suited for modelling collective motion, makes possible direct visualization of biologically interesting modes, and is complementary to the more time-consuming simulation of molecular dynamics trajectories. The essential assumption and limitation of normal-mode analysis is that the molecular potential energy varies quadratically. Our study starts with energy minimization of the X-ray co-ordinates with respect to the single-bond torsion angles. The main technical task is the calculation of second derivative matrices of kinetic and potential energy with respect to the torsion angle co-ordinates. These enter into a generalized eigenvalue problem, and the final eigenvalues and eigenvectors provide a complete description of the motion in the basic 0.1 to 10 picosecond range. Thermodynamic averages of amplitudes, fluctuations and correlations can be calculated efficiently using analytical formulae. The general method presented here is applied to four proteins, trypsin inhibitor, crambin, ribonuclease and lysozyme. When the resulting atomic motion is visualized by computer graphics, it is clear that the motion of each protein is collective with all atoms participating in each mode. The slow modes, with frequencies of below 10 cm-1 (a period of 3 ps), are the most interesting in that the motion in these modes is segmental. The root-mean-square atomic fluctuations, which are dominated by a few slow modes, agree well with experimental temperature factors (B values). The normal-mode dynamics of these four proteins have many features in common, although in the larger molecules, lysozyme and ribonuclease, there is low frequency domain motion about the active site.

The stillborn fetus: placental histologic examination in determining a cause.

This investigation was undertaken to determine whether or not histologic examination of the placenta contributed to a better understanding of the cause for an intrauterine fetal death. The placentas of stillborn fetuses delivered after the 20th gestational week were examined during a 4.5-year period (January 1979 to June 1983). Adequate information about the clinical history, autopsy examination, and placental histological examination was available in 89 cases. Delivery was usually within the first week after the fetal death. Significant histologic aberrations in the placenta were found in 87 (98%) cases. The most frequent abnormalities were those of vascular insufficiency, hemorrhagic endovasculitis, retroplacental hematomata, acute chorioamnionitis with fetal involvement, and erythroblastosis/hydrops. Histologic abnormalities were supportive of prior impressions in 67 (77%) cases, contradictory to prior impressions in ten (11%) cases, or the sole contributors in explaining the cause of death in ten (11%) cases. Routine histologic examination of the placenta after a recent fetal death provides helpful information in counseling the parents and in planning any future childbearing.

On the use of sequence homologies to predict protein structure: identical pentapeptides can have completely different conformations.

The search for amino acid sequence homologies can be a powerful tool for predicting protein structure. Discovered sequence homologies are currently used in predicting the function of oncogene proteins. To sharpen this tool, we investigated the structural significance of short sequence homologies by searching proteins of known three-dimensional structure for subsequence identities. In 62 proteins with 10,000 residues, we found that the longest isolated homologies between unrelated proteins are five residues long. In 6 (out of 25) cases we saw surprising structural adaptability: the same five residues are part of an alpha-helix in one protein and part of a beta-strand in another protein. These examples show quantitatively that pentapeptide structure within a protein is strongly dependent on sequence context, a fact essentially ignored in most protein structure prediction methods: just considering the local sequence of five residues is not sufficient to predict correctly the local conformation (secondary structure). Cooperativity of length six or longer must be taken into account. Also, we are warned that in the growing practice of comparing a new protein sequence with a data base of known sequences, finding an identical pentapeptide sequence between two proteins is not a significant indication of structural similarity or of evolutionary kinship.

How good are predictions of protein secondary structure?

The three most widely used methods for the prediction of protein secondary structure from the amino acid sequence are tested on 62 proteins of known structure using a program package and data collection not previously available. None of these methods predicts better than 56% of the residues correctly, for a three state model (helix, sheet and loop). The algorithms of Robson et al. [J. Mol. Biol. (1978) 120, 97-120] and Lim [J. Mol. Biol. (1974) 88, 873-894] are the best of those tested. New methods, now under development, can be tested against this benchmark.
