Allergens of the house dust mite, Dermatophagoides pteronyssinus, in patients with mite allergic rhinitis: a clinical investigation by intracutaneous skin tests and nasal provocation tests.

To determine the allergens of mite allergic rhinitis, we studied 31 patients with mite allergic rhinitis by skin tests and nasal provocation tests (15 for skin and 16 for nasal tests) using 6 fractions of Dermatophagoides pteronyssinus (Dp) extract differing in molecular weights (15, 25, 32, 53, 95 and 190 kDMW). In skin testing, patients showed intense positive reactions to the fractions of 15, 25, 32, 95 and 190 kDMW, among which the most patients showed positive reactions to the fractions of 15 and 25 kDMW. Significant differences were found in patients' positive reactivity among each fraction and between low (15 and 25 kD) and high (95 and 190 kD) molecular weight fractions as well. In nasal provocation tests, patients showed intense positive reactions to the fractions of 15, 32, 53 and 95 kDMW, especially to the fractions of 15 and 95 kDMW. Furthermore, the insidence of positive reactions to the 15 kDMW fraction was significantly higher than that to any other fraction in the skin tests (P < 0.05). From these results, the low molecular weight fraction, 15 kDMW, is considered to be the main allergen of this mite and the high molecular weight fractions, 95 and 190 kDMW, may also be considered to be allergens of this mite.

Changes in oral functions and muscular behavior due to surgical orthodontic treatment.

The faces of fifteen adult patients who had undergone surgical orthodontic treatment were videotaped frontally and laterally in order to observe their oral circumferential behavior for function and their facial muscle posture at rest. As the ANB angle became 0.9 degrees from -3.5 degrees and the occlusal relationship improved to normal, facial morphology became harmoneous. In general observation, habits of facial muscle tensity at rest was reduced and lip sucking following forward tongue thrusting was decreased after treatment. Pronunciation of previously unclear sounds such as 'Ta' and 'Sa' improved and breath leakage decreased after treatment. The cooperative motions between lips and tongue improved significantly. Although a few subjects still had weak lip tensity and facial muscles, habits of tongue thrusting during swallowing were generally reduced and swallowing motions became more natural. As the movement of tongue tip became smoother, rhythmical chewing improved, and they became capable of chewing on both sides. Good relationships between morphology, oral function, and muscular behavior are very important in stabilization of the normal dentition after surgical orthodontic treatment. Normally maintained forces of balanced occlusion reduce the possibility of relapse.

Eosinophil cationic protein in the nasal secretions of patients with mite allergic rhinitis.

In 25 patients with mite nasal allergy and 13 healthy control subjects, levels of eosinophil cationic protein (ECP) and histamine in nasal secretions were examined before and after challenge with mite extract. The ECP level was found to be significantly higher in the washings taken 30 minutes after challenge compared with the washings taken before challenge (P < .05). ECP levels measured both before and after mite extract challenge were found to be significantly higher in the patients than in the controls (P < .01). These facts suggest that the ECP affects the nasal mucosa by causing injury to ciliated cells and by acceleration of the processes of modification and inhibition of the allergic reaction.

Seasonal change in maximal expiratory flow-volume pattern in patients with Japanese cedar pollenosis.

Fifteen patients with Japanese cedar pollenosis were examined for lower airway function. Flow-volume patterns obtained from flow-volume and volume-time curves during the pollen season (March) and outside of the pollen season (June) were evaluated. In a previous report we classified maximal expiratory flow-volume (MEFV) curves in five patterns from A to E. In the present study, the patterns did not vary between the two periods except in one patient. Eleven patients out of 15 showed type E patterns, in which the flow-volume curve was concave along its entire course. In most of the patients with severe or moderate symptoms of allergic rhinitis only during the pollen season, the curve shifted to the right, but the parameters of the curves did not increase significantly outside of season. These findings suggest that patients with Japanese cedar pollenosis suffer from continuous latent peripheral airway obstruction. Extremely slight changes in the flow rate were detected by comparing the curves obtained during the two periods.

Ankylosing spondylitis successfully treated with methotrexate.

A 37-year-old man with ankylosing spondylitis (AS) and psoriasis who was successfully treated with methotrexate (MTX) is reported. In 1980, he had low back pain, limited motion in the lumbar spine, radiological findings of bilateral sacroiliitis, and HLA-B27 positivity. In January 1991, he developed psoriasis and he had difficulty in performing desk work in spite of treatment with antirheumatic drugs. In May 1991, MTX 7.5 mg/week per os was started. Joint symptoms, psoriasis, and acute phase reactants improved within 1 month after the treatment and this improvement continued for more than 6 months after the treatment. After discharge he was able to return to his job.

[A case of cervico-mediastinal tuberculous lymphadenitis].

A 27-year-old female presented to our hospital because of anterior chest pain and left cervical tumor. Her chest X-ray film showed bilateral swelling of the superior mediastinum, which was composed of multiple mediastinal masses with obliteration of adjacent mediastinal fat on chest CT film. A specimen obtained from a left cervical lymph node histologically showed granuloma formation with central caseous necrosis, and the diagnosis of cervicomediastinal tuberculous lymphadenitis was therefore made. Both cervical tumor and mediastinal masses decreased in size following antituberculous chemotherapy. Mediastinal tuberculous lymphadenitis in young adults is rare. It is difficult to differentiate this condition from other mediastinal tumors, although its CT findings are characteristic.

[Studies on screening for hard-of-hearing children by questionnaire. 2. Comparison of response rates to individual questions in 1-year-6-month-old children with normal hearing and hearing loss using a health screening questionnaire].

We report the results of studies on responses to questions on hearing impairment, comparing 165 school children whose hearing was diagnosed as normal at the time of admission to primary school and 37 children with hearing loss attending facilities for children with this disability. These children had been covered by similar surveys as part of the health screening for 1-year-6-month-old infants conducted by municipal authorities. Significant differences between the two groups were noted in the response rates to all questions on hearing. Similar differences were noted in the responses to questions on family history of hearing impairment. There were no significant differences in the incidences of asphyxia and jaundice at birth. The most striking differences were noted in the response rates to questions 6, 7, 8, 10, 11, 16, and 18. Most useful and universally valid in this survey were questions 8, 10 and 18 in addition to the question on whether or not parents suspected infantile hearing loss.

A unique case of reducing body myopathy.

The clinical, electromyographic, and histologic characteristics of a 17-year-old girl with reducing body myopathy are described. She is, to our knowledge, the oldest reported case and the only patient described with severe mitral valve prolapse and scoliosis. Electromyography demonstrated spontaneous positive sharp waves and fibrillation potentials with many low-amplitude, short, polyphasic motor unit potentials. The right deltoid muscle was characterized by focal areas with large fibers associated with increased endomysial connective tissue and "split" fibers. Purple-gray sarcoplasmic masses stained with trichrome were PAS-negative, appeared as "empty" spaces with both ATPase and NADH-TR, and stained darkly with menadione NBT. The features described expand the clinical presentation of this myopathy, and may lead to a better understanding of its etiology.

Electrical stimulation of pelvic floor musculature by percutaneous implantable electrodes: a case report.

A forty-year-old man with reflex urinary incontinence due to spinal cord injury was treated with electrical stimulation of the pelvic floor musculature. In this case we employed percutaneous implantable electrodes and an external pulse regulator. After 4 weeks of stimulation incontinence was improved and urodynamically maximum cystometric capacity increased from 220 ml to 350 ml. Our method is easy and not invasive. This technique can be an alternative for the electrical stimulation for urinary incontinence.

IgE, IgG, and IgG4 antibody titers to fractionated house dust mite antigens in nasal allergy patients.

Using 6 fractions differing in molecular weight of Dermatophagoides pteronyssinus (Dp)-antigen, we measured by enzyme-linked immunosorbent assay (ELISA) the titers of specific IgE, IgG and IgG4 antibodies against Dp antigen in sera of allergic subjects who were sensitive to house dust mite. We intended to evaluate which Dp fraction acts as the major antigenicity for allergic subjects. Results were as follows: 1) In comparison with normal controls, the titer of IgE antibody specific to crude Dp antigen was evaluated, but no significant difference was found among the titers of IgE antibody against each Dp fraction. 2) The titer of IgG antibody against the fraction with a high molecular weight (190 KD, 95 KD) was significantly higher than the titer of the 15 KD fraction in the nasal allergy patients. 3) The 15 KD fraction induced significant elevation of the titer of IgG4 antibody. It suggests that the low molecular weight fraction may act as the major allergenicity of Dp-antigen for inducing both IgE and competitive IgG4 antibodies, although other fractions induce significant IgE responses in patients with nasal allergy.

[On the problems of the antituberculous drug resistance].

In this part, we will discuss exclusively about the technique of measurement. It is desirable especially for the proportion method that two inoculations with the same doses give the same number of colonies and when diluted, the colonies decrease exactly parallel to it degree of dilution. But this ideal is not necessarily realized and especially the difference between the dilution of 10(-3) and 10(-4) mg/ml on the drug free medium and that of 10(-2) and 10(-3) on the drug containing medium is fairly remarkable: the former is not so large but the latter sometimes varies tremendously with the drugs and their concentrations. So it is necessary to use always two inoculations for all specimens and the resistance is calculated on the medium inoculated with the same doses, for that, colonies on the control must be countable and suitable number that is 50-300. Further too concentrated solution like 10(-2) mg/ml must not be used for the proportion method. The selection of the most suitable two dilutions is specially important for this method. Original one is 10(-3) and 10(-5). Unfortunately we did not follow this formula and at first we adopted the combination of 10(-3) and 10(-4), then 10(-2) and 10(-4) finally 10(-2) and 10(-3) which was the worst. Now we searched for the dilution which gave the suitable number of colonies from our several thousand cases and found that 10(-4) was the most excellent and gave this number in 63%, next 10(-5) in 14%, and then 10(-3) in 7.2%, 10(-2) in only 1.0%. With the combination of 2 solutions 10(-3) and 10(-5)--original one--was most excellent (94.8%) then 88.4% with 10(-4) and 10(-5). The combination of 10(-2) and 10(-3) was worst, it gave only 8.9%. As the proportion method demands the most strict procedures for all cases, it is necessary as simplify as possible, thus in every day practice, only the drugs most frequently used (3 or 4 drugs) and one concentration for each drug could be examined. Further we discussed on the difference between DH-SM and SM, direct and indirect method and problem of plug.

[The effect of a specific thromboxane A2 antagonist, AA-2414 on airway hyperresponsiveness induced by ozone exposure in dogs].

To determine whether thromboxane A2 (TxA2) is involved in airway hyperresponsiveness induced by ozone exposure, we studied the effect of a specific TxA2 antagonist, AA-2414 on ozone-induced airway hyperresponsiveness in seven dogs. Airway responsiveness to inhaled methacholine was determined by modified Astograph (7 Hz oscillation method), and numbers of neutrophils in the peripheral blood, neutrophil counts in bronchoalveolar lavage fluid (BALF), the levels of TxB2 and 6-keto-Prostaglandin F1 alpha (6-keto-PGF1 alpha) in BALF were measured before and after ozone exposure, and after ozone exposure with pretreated AA-2414. Ozone exposure was carried out for 2 hr at an ozone level of 3.06 +/- 0.06 ppm (mean +/- SE). Airway responsiveness to inhaled methacholine increased significantly after ozone exposure (p less than 0.01), and the hyperresponsiveness induced by ozone exposure was inhibited significantly by pretreated AA-2414 (p less than 0.01). Numbers of neutrophils in the peripheral blood and neutrophil counts in BALF increased after ozone exposure, and these increase were not inhibited by pretreated AA-2414. There was no apparent change in the levels of TxB2 in BALF after ozone exposure and after ozone exposure with pretreated AA-2414, however the levels of 6-keto-PGF1 alpha in BALF decreased after ozone exposure and after ozone exposure with pretreated AA-2414 (p less than 0.1). These results suggest that TxA2 plays an important role in the development of airway responsiveness after ozone exposure in dogs, and ozone-induced airway hyperresponsiveness may not be associated with the hyperproduction of TxA2 but with the relative increase of TxA2 due to the decrease of PGI2.

[The role of 5-lipoxygenase products in the development of airway responsiveness induced by platelet activating factor inhalation in dogs].

To determine whether 5-lipoxygenase products are involved in the development of airway responsiveness and in the infiltration of inflammatory cells into the airway after platelet activating factor (PAF) inhalation, we studied the effect of a selective 5-lipoxygenase inhibitor, AA-861 on PAF-induced airway hyperresponsiveness and on the increase of neutrophil and eosinophil counts in bronchoalveolar lavage fluid (BALF) after PAF inhalation in seven dogs. Airway responsiveness to inhaled methacholine was determined by modified Astograph (7Hz oscillation method). PAF (1000 mu/ml) was delivered as an aerosol, generated from a Devilbiss 646 nebulizer for ten minutes. Airway responsiveness to inhaled methacholine increased significantly 3 hr after PAF inhalation (p less than 0.01). After PAF inhalation, neutrophil and eosinophil counts in BALF increased significantly (p less than 0.01), and the levels of thromboxane (Tx)B2 in BALF also increased (p less than 0.05). Pretreated AA-861 significantly inhibited the increase of airway responsiveness after PAF inhalation (p less than 0.01). The increase of neutrophil and eosinophil counts in BALF after PAF inhalation was also inhibited significantly by pretreated AA-861 (p less than 0.01). The levels of TxB2 in BALF did not change after PAF inhalation following pretreatment with AA-861. These results suggest that 5-lipoxygenase products play important roles in the increase of airway responsiveness and in the infiltration of inflammatory cells into the airway after PAF inhalation in dogs. TxA2 released from inflammatory cells may be involved in the increase of airway responsiveness induced by PAF inhalation.

Sebaceous epithelioma in the external auditory meatus: a case report.

We report a rare case of sebaceous epithelioma(SE) in the external auditory meatus of an 84-year-old woman. The tumor, measuring 1.5 cm in diameter, was hemispherical, yellowish-brown, elastic and hard. It was removed surgically without complications and there has been no recurrence two years postoperatively. The histopathologic findings were characteristic of a sebaceous epithelioma. The controversy over the origin of this tumor is discussed.

[The role of thromboxane A2 in the development of airway responsiveness after platelet activating factor inhalation in dogs].

To determine whether thromboxane A2 (TxA2) is involved in airway hyperresponsiveness after platelet activating factor (PAF) inhalation, we studied the effect of a specific TxA2 receptor antagonist, AA-2414 on the development of airway responsiveness induced by PAF inhalation in six dogs. Airway resistance and airway responsiveness to inhaled methacholine were determined by modified Astograph (7 Hz oscillation method). PAF inhalation (1000 micrograms/ml, ten minutes) caused a significant increase of airway resistance (p less than 0.01), and the increase of airway resistance was not inhibited by pretreated AA-2414. Airway responsiveness to inhaled methacholine increased significantly 3 hr after PAF inhalation (p less than 0.01). Pretreated AA-2414 inhibited the increase of airway responsiveness significantly (p less than 0.01), but the inhibition was partial. After PAF inhalation, total cell counts, neutrophil counts, eosinophil counts and the levels of TxB2 in bronchoalveolar lavage fluid increased significantly (p less than 0.05), and these increase were not affected by pretreated AA-2414. These results suggest that TxA2 is not involved in the bronchoconstriction induced by PAF inhalation, but TxA2 plays a partial role in the development of airway responsiveness after PAF inhalation in dogs.
