Possibility of targeting treatment for ischemic heart disease with liposome (II).

Studies on the isolated rat heart perfusion model have proved that perfusion with high Ca2+ (4.5 mmol/L), high K+ (8.7 mmol/L) or free radical generating system (FRGS) significantly increases myocardial uptake of liposomes. Intravenous injection of liposomes covalently combined with antibody of rat myocardial cells obviously elevates the target action of liposomes to myocardium. Liposome-carried SOD for treatment of rat myocardial ischemia-reperfusion injury is much more effective than simple SOD. The results evidence that the liposome as drug carrier for treatment of ischemic heart diseases shows a broad prospect for its clinical use.

Effect of endothelin, angiotensin II and ANP on proliferation of vascular smooth muscle cells and cardiomyocytes.

By means of cell culture, 3H-thymidine (3H-TdR) incorporation and c-fos oncogene dot plotting technique, it was found that endothelin (ET) and angiotensin (ANG II) could promote proliferation and DNA synthesis of cultured vascular smooth muscle cells (VSMCs) and myocardial cells, and stimulate expression of c-fos oncogene of VSMCs. However, atrial natriuretic peptide (ANP) antagonizes the above effects of ET and ANG II.

["Walkman music" during epidural anesthesia].

The sedating effect and influence of walkman music on 120 patients, that were randomly divided into a music and non-music group, who had received epidural anesthesia were investigated. It was found that significantly fewer patients in the music group felt anxious during surgery (P = 0.049). Meanwhile, heart rate and mean arterial pressure of the non-music group remained at a higher level, resulting in higher heart loading. Furthermore, patients listening to music had a significantly smaller need of sedatives (P = 0.001). With the purpose of reducing the consumption of sedatives, and offering a better anesthetic environment, we recommend the use of music during regional anesthesia.

Plasma endothelin levels in cirrhotic patients and their correlation with atrial natriuretic peptide.

Plasma levels of endothelin (ET) were determined by radioimmunoassay in 11 healthy volunteers and 34 patients with liver cirrhosis. The results revealed that mean ET plasma level was significantly lower in 34 cirrhotic patients than in normal subjects (40.21 +/- 4.38 ng/L vs 84.55 +/- 7.88 ng/L, P < 0.01). Plasma ET was 32.17 +/- 3.98 ng/L in 23 cirrhosis patients with ascites compared with 57.0 +/- 9.0 ng/L in patients without ascites (P < 0.01). Moreover, it is apparent that plasma levels of ET decreased with the increase of ascites volume, and increased after the disappearance of ascites. But plasma levels of atrial natriuretic peptide (ANP) in cirrhotic patients were markedly higher than those in the controls (830.12 +/- 72.18 ng/L vs 439.45 +/- 50.18 ng/L, P < 0.01). In patients with ascites, ET plasma levels were inversely correlated to ANP. The results suggest that deficiency of circulating ET might be one of the important factors involved in sodium and water retention in cirrhosis.

[The hemodynamic and ventilatory effects between Trendelenburg and reverse Trendelenburg position during laparoscopy with CO2-insufflation].

Thirty ASA class I female patients underwent gynecologic pelvioscopy and laparoscopic cholecystectomy were included in this study. All of them received general anesthesia and certain volume of CO2 pneumoperitoneum. We evaluated the effect of positioning (Trendelenburg as group I and reverse Trendelenburg as group II) on hemodynamic parameters and ventilation. The hemodynamic parameters, ventilation, and arterial blood gas were determined at the following stages: (1) after 10 minutes of normoventilation and before CO2 pneumoperitoneum (as control), (2) 5 minutes after CO2 pneumoperitoneum and positioning, (3) 25 minutes after positioning, (4) 50 minutes after positioning, (5) 10 minutes after resumption of supine position and decompression of abdomen. Arterial blood pressure, heart rate, minute volume, end-tidal CO2, oxygen saturation and blood gas analysis were taken during normoventilation. We found that there was no statistical difference between either group in respect to hemodynamic parameters. For the ventilation, both groups had appropriate SpO2 during the period of CO2 pneumoperitoneum, but PaCO2 was significantly increased in group I 25 min after CO2 pneumoperitoneum and Trendelenburg positioning. Nonetheless, end-tidal CO2 was not significantly increased. pH value and PaCO2 did respectively decrease and increase significantly in group I 50 min after Trendelenburg position. However, no change was found in end-tidal CO2. All data were not significantly different after resumption of supine position and decompression of abdomen. Conclusively, except that pH and PaCO2 had changed after a period of Trendelenburg positioning with CO2 pneumoperitoneum, the patients who underwent laparoscopic surgery with normoventilation have no hemodynamic and respiratory change.(ABSTRACT TRUNCATED AT 250 WORDS)

[Expression of left ventricular ANF-gene in rat with cardiac hypertrophy].

Plasma concentration of ANF and expression of left ventricular ANF-gene in rats with cardiac hypertrophy induced by abdominal aortic partial ligation were analyzed by RIA and Northern blot respectively. Results showed that plasma concentration of ANF and level of left ventricular ANF-mRNA in cardiac hypertrophic rats increased markedly, indicating that the cardiac load may induce transcription and expression of left ventricular ANF-gene. This effect could be potentiated by intracellular calcium modulator taurine and inhibited by vasodilator hydralazine.

[Changes of calcium transport capacity of myocardium and myocardial mitochondria during sepsis].

On the isolated perfused heart model of septic rats, the present study showed that: (1) Calcium content and 45Ca-influx of myocardium increased 190%, 208% (P < 0.01) and that of mitochondria elevated 332%, 178% (P < 0.01) respectively with no change of myocardial 45Ca-release during sepsis. (2) 10(-8) mol/L calcitonin gene-related peptide (CGRP) or 10(-7) mol/L atriopeptin (ANP) added into the Krebs-Henseleit solution could effectively reduce 45Ca-influx to myocardium and mitochondria with no effect on myocardial 45Ca-release. (3) The calcium uptake reserve of mitochondria evaluated in vitro showed that the maximal calcium uptake and uptake velocity of mitochondria during sepsis were reduced 34.6%, 33.3% (P < 0.01) respectively. The data suggested that the net increase of myocardial Ca2+ content resulted from increase of 45Ca-influx with no change of 45Ca-efflux and the reduction of mitochondrial Ca2+ buffering capacity during sepsis were key events in the pathogenesis of intracellular Ca(2+)-overload. CGRP and ANP could effectively alleviate Ca(2+)-overload of myocardium and mitochondria. This may have some cellular protection action during sepsis.

Possibility of targeting treatment for ischemic heart disease with liposome (I).

This paper reports the basic research on the possibility of using targeting treatment for ischemic heart disease with liposome as drug carrier. Studies have been performed on isolated rat cardiomyocytes, or isolated perfused rat and rabbit hearts. Results show that cardiomyocytes may interact with liposome through fusion, endocytosis, adsorption and molecular exchange of phospholipid. Forms of cellular uptake of liposome depend chiefly on the physicochemical properties of liposomes. Anoxia changes the pattern of liposome uptake by cardiomyocytes and increases uptake of liposomes. Uptake of liposomes, especially of positively charged liposomes by ischemic myocardium is significantly increased. The quantity of increase of liposome uptake is in the following order: ischemia-reperfusion area > peripheral area of the infarct > non-ischemic area > infarcted area. The above results indicate that liposome as drug carrier might promote the delivery of drug into ischemic myocardium and cardiomyocytes.

Sexual and aggressive motives in sexually aggressive college males.

The relative contributions to sexual aggression of general sexual and aggressive motives and their respective inhibitory factors were compared. One hundred forty-three university males responded to self-report measures of sexual and aggressive drives, sex and hostility guilt, social desirability response bias, and history of coercive sexuality. With the effects of social desirability controlled, the only predictor of sexual aggression was Sex Guilt, which predicted level of force used but not intimacy of the sexual act obtained.

Effect of bicarbonate on intracellular potential of rabbit ciliary epithelium.

Extracellular HCO3- hyperpolarizes the intracellular potential and makes the aqueous medium negative with respect to the stromal surface of the rabbit ciliary epithelial syncytium. The bases for these observations have been unclear. We have been studying the bicarbonate-induced hyperpolarization (BIH) with sustained intracellular recordings for periods as long as 1-2 hrs. The BIH was observed [6.0 +/- 0.4 mV (mean +/- SE, N = 22)] even when the external pH was clamped constant by appropriately changing the CO2 tension. External HCO3- was required since aeration with CO2 at low external pH did not replicate the BIH. DIDS [4,4'-diisothiocyano-2,2'-disulfonic acid] did not abolish the effect. The hyperpolarization is unlikely to reflect the pH dependence of K+ channels alone, since the effect was not reduced by either 2 mM Ba2+ alone or 2 mM Ba2+ together with 50-100 microM quinidine. The BIH depends directly or indirectly on external Na+, since the sign of the polarization response was reversed either by replacing Na+ with N-methyl-D-glucamine or by blocking the Na+,K(+)-exchange pump with 50-100 microM ouabain. Replacement of external Cl- with NO3- or application of the Cl(-)channel blocker NPPB [5-nitro-2-(3-phenylpropylamino)-benzoate] depolarized the membrane and reversed the sign of the BIH. The response of the ciliary epithelium to HCO3- is complex and may arise from several mechanisms. We suggest that one important element is an anion channel whose conductance is reduced by bicarbonate and whose reversal potential is indirectly dependent on the operations of the Na+,K(+)-pump and a Cl(-)-linked symport.

[Effect of endothelin on isolated myocytes from rat heart].

The mechanism of endothelin (ET) action on isolated myocytes of rat heart was investigated. Administration of 10(-9)-10(-7) mol/L ET caused dose-dependently severe contracture of myocytes, increase in cytosolic lactate dehydrogenase leakage and increase of cellular total calcium. ET also enhanced 45Ca2+ influx in myocytes, that could be antagonized by calcium channel blocker verapamil. Incubation of Fura-2 -preloaded myocytes with ET would induce a significant increase of cytosolic free Ca2+, and this effect of ET could also be partly antagonized by verapamil. These results suggest that ET enhances Ca2+ influx in myocytes mainly through voltage-dependent calcium channel, by which ET exerts its biological action.

[Effect of NO-like relaxing factor (NO-LRF) in rat tourniquet shock].

On tourniquet shock (ToS) rat model, it was found that the reactivity of isolated perfused aortic ring to noradrenaline was decreased, while the cGMP content of the aortic tissue was increased. These ToS-induced changes could be potentiated or attenuated respectively by perfusion with NO-precursor, L-arginine, or NO-synthesis inhibitor L-NNA independent of the presence of vascular endothelium. Guanylate cyclase inhibitor, methylene blue, could also attenuate the aortic reactivity. All these results suggest that the aortic musculature can produce a NO-LRF factor capable of lowering the vascular reactivity of the ToS animals. That L-arginine can ameliorate while L-NNA can exacerbate ToS, suggest that NO-LRF do play an adaptive role in the protective mechanism of the organism during ToS.

[Role of endothelin in the pathogenesis of rat experimental hypertension produced by aorta narrowing and saline uptake].

Endothelin is a bioactive polypeptide released from vascular endothelium, which has a strong action promoting vascular contraction, proliferation and hypertrophy of vascular smooth muscle cells. The present investigation was performed on the hypertensive rat produced by narrowing abdominal aorta and drinking saline. It was observed that the level of plasma endothelin in the hypertensive rats was doubled (9.70 +/- 0.68 vs. sham group 4.11 +/- 0.33 pg/ml, P < 0.01), and administration of specific endothelin-antiserum into hypertensive rats significantly attenuated the increase in the blood pressure (18.97 + 1.32 vs. 27.33 + 0.09 kPa in untreated hypertensive rats, P < 0.01), and dramatically ameliorated malfunction resulting from myocardial hypertrophy. The results suggest that endothelin is an important factor in the pathogenesis of hypertension, and that inhibition of endothelin action may be a new effective way in prevention and therapy of hypertension.
