Relation of neutrophil alkaline phosphatase activity to Fc IgG receptor development in human blood and bone marrow.

A possible correlation between Fc-IgG receptor expression and neutrophil alkaline phosphatase (NAP) activity was investigated in relation to maturation of granulocytes in human peripheral blood and bone marrow. NAP activity was studied in bone marrow from patients with normal peripheral blood NAP scores (20-100) and compared with those with high NAP scores. Results indicate that NAP activity is confined to segmented neutrophils (SN) in marrow and peripheral blood except when peripheral NAP activity is abnormally high. There is a normal increase of approximately 60% in mean NAP scores of peripheral blood compared to marrow SN. Granulocyte Fc-IgG receptor activity, known to increase with maturation, was studied in relation to NAP activity. A combined assay using a rosetting technique with ox-erythrocyte (oxE) antibody-IgG and subsequent NAP cytochemistry was used. Receptor expression was found to be virtually complete in normal granulocytic maturation before the appearance of detectable NAP activity. This was supported by findings in chronic myeloid leukaemia where no cases of low Fc receptor activity were found despite NAP scores of less than 5. The significance of these findings is discussed in the light of current concepts of the control of NAP activity.

Erythroid Fc-IgG and complement receptor expression: a study of normoblastic and megaloblastic human bone marrows.

Using a standardized rosetting technique with IgG-coated ox erythrocytes, avid IgG (Fc) receptors were demonstrated on red cell precursors. The proportion of receptor-positive cells in normal marrows was highest in early precursors and appeared to be lost with maturation. In megaloblastosis, the absolute percentage of early precursors increase, but there is an even greater increase in the proportion of receptor-positive cells. It is proposed that this reflects the degree of maturation arrest. The specificity of the receptor was confirmed by inhibition studies with aggregated human IgG. In contrast, the expression of the C3b "immune adherence" receptor, assessed by IgM-C3b-coated ox erythrocytes, was seen to increase with erythroid maturation. Early megaloblasts, especially in severe megaloblastosis, showed a marked decrease in C3b receptor activity, again in proportion to the level of maturation arrest. The significance and possible function of these receptors is discussed.

Maturation-linked expression of C3b and C3b' receptors on developing human bone marrow and peripheral blood granulocytes.

The sequential expression of granulocyte membrane receptors for C3b, C3b' and C3d have been investigated in normal human bone marrow and peripheral blood. In particular the relationship between receptor expression and morphological characteristics of granulocyte maturation has been assessed. The result indicate that C3b receptor development precedes that of the Fc-IgG receptor and is present on all neutrophil precursors with the exception of the agranular myeloblast. Furthermore, there is, in contrast to Fc-IgG receptors, no difference (P greater than 0.35) in C3b receptor expression between marrow and peripheral blood segmented neutrophils. An apparent subpopulation of granulocytes also appear to have receptors for C3b' whereas receptors for C3d were not detected at all during granulocyte maturation. The application of these findings to the normal in vivo functions of granulocytes and to the study of abnormal granulocyte populations in myeloproliferative disorders is discussed.

Maturation-linked expression of the Fc (IgG) receptor on developing human bone marrow and peripheral blood granulocytes.

Expression of Fc(IgG) receptors, as demonstrated by IgG-coated ox cells and heat-aggregated IgG, was assessed on normal human peripheral blood and bone marrow neutrophils and their precursors. The results indicate that Fc receptor development is confined to post-mitotic granulocytes, being primarily associated with neutrophil segmentation. Fc receptors were not detected on myeloblasts or promyelocytes. A mean value of 79% and 99% Fc positive segmented neutrophils were found in bone marrow and peripheral blood respectively. This highly significant difference (P less than 0.001) appears to represent a morphologically indistinguishable terminal maturation stage. Inhibition studies with IgG show that the neutrophil Fc receptor is inhibited by aggregated but not, in contrast to the corresponding monocyte receptor, by monomeric (7S) IgG. Applications of these findings to the in vivo function of the neutrophil in health and pathology are discussed.

Puerperal laparoscopic sterilization.

Two hundred and ninety-seven consecutive puerperal laparoscopic sterilization cases during a five-year period are reviewed. There were six failures to complete the laparoscopic sterilization. A single case of anesthetic cardiac arrest constituted the only major complication. There has been no mortality. Two subsequent pregnancies occurred in this series, and their follow-up operative findings are reviewed.

Phenotypic analysis of an established cell line derived from a patient with Hodgkin's disease (HD).

This report describes the phenotypic analysis of a cell line obtained from a female patient with the nodular sclerosing subtype of Hodgkin's disease (HD). The cell line has a neoplastic karyotype and is stable in culture in the absence of feeder layers or growth factors. Phenotypic analysis of this cell line shows that it cannot easily be characterized as either a lymphocyte, macrophage or granulocyte but resembles in its characteristics certain HD lines already described in the literature. The cell line carries the antigen defined by the Ki-1 monoclonal antibody, shows myeloid markers on a proportion of cells and has cytoplasmic UCH-T1.

Diagnostic features and survival in typical and prolymphocytoid variants of chronic lymphocytic leukemia.

Diagnostic features were evaluated with regards to survival in 203 cases of typical and prolymphocytoid chronic lymphocytic leukemia. Excluding 27 (13 per cent) patients with second malignancies, survival analyses indicated that the prognostic factors with greatest significance were age at presentation (less than 65 years and 65 years or more: p = 0.0004), proportions of prolymphocytoid cells (less than 10 per cent and 10 per cent or more: p less than 0.0001 age corrected) and surface immunoglobulin (SIg) density (weak and more than weak: p = 0.001 age corrected). In contrast, sex, absolute numbers of prolymphocytoid cells, SIg light chain type and FMC7 expression were not prognostically significant. These observations suggest that the recognition and delineation of prolymphocytoid CLL variants by morphological and immunophenotypic criteria, although important in diagnostic terms, may have less relevance with respect to prognosis and patient management.

Physicians' attitudes toward preventive care services: a seven-year prospective cohort study.

We report results of a seven-year prospective cohort study of physicians' attitudes about and intentions to provide 27 preventive care services in their future practices. Respondents in the cohort were surveyed three times: first, during orientation to medical school; second, during their third year of medical school; and finally, following completion of their third-year of residency training. The majority of preventive care services were viewed as more important to clinical practice in primary care than in non-primary care specialties. Positive attitudes toward preventive care services generally persisted among both primary and non-primary care physicians. Respondents expressed only fair to medium levels of confidence in the ability of physicians in their specialty areas to provide any of the preventive services examined. Respondents reported low levels of confidence in the ability of primary care physicians to provide nutritional counseling, though they ranked it as important. Respondents were fairly or moderately confident in the ability of primary care physicians to provide counseling about smoking cessation, health, AIDS education, and substance abuse. Participants ranked smoking cessation counseling, health counseling, AIDS education, cancer detection education, and substance abuse counseling and education as very important. In general, physicians were less likely to plan on providing preventive services than they were to expect their residency programs to prepare most or all to provide the services. Findings document the need to prepare physicians better to provide preventive services.

Withdrawal after narcotic therapy: a survey of neonatal and pediatric clinicians.

Pharmacists at the 1995 American College of Clinical Pharmacy Pediatric Practice and Research Network meeting volunteered to act as coordinators at their sites and survey pediatric and neonatal nurses, pharmacists, and physicians regarding dependency in neonatal and pediatric patients after therapeutic administration of narcotics. Thirteen (60%) of 21 coordinators returned 244 surveys. Primary symptoms of withdrawal reported by clinicians were agitation (100%), irritability (100%), inconsolability (100%), crying (99%), tremors (98%), high heart rate (98%), fidgets (98%), high blood pressure (97%), less sleep (96%), and sweating (94%). Most clinicians considered narcotic withdrawal to be a problem (74%) that should be treated (87%). A dependency scale is being developed and will include symptoms reported by more than 75% of respondents.
