Fibronectin in healing rabbit corneal wounds.

Fibronectin, a glycoprotein present in plasma and extracellular matrix, is believed to be involved in cell-cell and cell-matrix interactions. Using immunofluorescence techniques, we studied the time course of appearance and distribution of fibronectin in healing rabbit corneal epithelial wounds and compared fibronectin to other selected proteins in the cornea. In the normal cornea, fibronectin was detected only in Descemet's membrane and not in the epithelial basement membrane. Shortly after wounding, fibronectin deposited on the denuded corneal surface and was a continuous prominent layer by 8 hours. The epithelium had begun to migrate over the deposited fibronectin by 22 hours and by 52 hours had completely covered the denuded surface. Fibrinogen/fibrin was also detected on the initial bare wound surface. Once the wound was reepithelialized, the subepithelial fibronectin and fibrin layer then progressively disappeared, so that by 2 weeks only a small amount was detected. Fibronectin also appeared in the deep stroma of corneas after wounding in elongated patches, a pattern suggestive of keratocyte association. Fibronectin had an inverse relationship to bullous pemphigoid antigen which was used as a marker for the lamina lucida of the epithelial basement membrane. The bullous pemphigoid antigen, which was found in the normal corneal epithelial basement membrane, was removed with the epithelium and reappeared during the wound healing when fibronectin was diminishing. IgG and albumin did not localize on the wound surface, and the diffuse staining seen in the stroma did not change during healing. These findings are compatible with the hypothesis that fibronectin and fibrin play a role in epithelial migration and temporary adhesion to the surface during corneal wound healing, at a time when the normal anchoring mechanism is lost.

Tissue adhesive arrests stromal melting in the human cornea.

The direct early application of cyanoacrylate adhesive to a prepared ulcer bed and adjacent basement membrane, followed by placement of a bandage lens, gave good results in ten patients with corneal ulceration. The patients had ulceration with keratitis sicca, herpes keratitis, and other surface diseases. Progressive melting was arrested in all cases, and concurrent adjunctive therapies were used as indicated. The technique is quick and simple.

Selective culture of epithelioid cells from a human squamous carcinoma using a monoclonal antibody to kill fibroblasts.

Monoclonal antibody LICR LON/FIB86 is cytotoxic for human fibroblasts from at least some sources, with rabbit complement. Explants of human squamous carcinomas of the head and neck were put into culture and fibroblasts and epithelioid cells began to grow out. The cultures were treated regularly with antibody plus complement: the fibroblasts were killed consistently, as they emerged, leaving sheets of epithelioid cells. We conclude that this is a practical method for controlling human fibroblast growth in culture.

Randomized clinical trial of topically administered cromolyn sodium for vernal keratoconjunctivitis.

We performed a controlled, randomized, double-masked, double-crossover clinical trial topically administered cromolyn sodium (4%) in 11 patientas with atopy and vernal keratoconjunctivitis. Statistically significant differences were noted in conjunctival injection, superifcial punctate keratitis, mucous production, and itching when eyes treated with cromolyn were compared to those treated with a placebo. The drug was well tolerated by all patients.

Immunosuppressive therapy for external ocular inflammatory disease.

Fourteen patients with progressive ocular inflammation and destructive lesions, unresponsive to conventional therapy, were treated with systemic immunosuppression. Ten patients had connective tissue/vasculitis diseases; two, cicatricial pemphigoid; and two, bilateral Mooren's corneal ulcers. Control of underlying systemic disease by immunosuppression resulted invariably in concomitant control of ocular inflammation and destruction. Encouraging results were obtained in the patients with cicatricial pemphigoid and those with bilateral Mooren's ulcers.

Mucus-secreting 'alveolar-cell' tumour of the lung: a histochemical comparison of tumours arising within and outside the lung.

Five cases of mucus-secreting 'alveolar cell carcinoma' are described, two of primary pulmonary origin and three metastatic from breast or pancreas. Mucin histochemistry demonstrated qualitative and quantitative differences between the mucopolysaccharides produced by these two groups of tumours. Hyaluronic acid production was restricted to the primary lung tumours. Greater quantities of acid sulphomucins were found in the two tumours arising in the lung and more neutral mucins in the three tumours metastatic from extra pulmonary primaries. It is proposed that mucus-secreting alveolar cell carcinomas represent an unusual, and saprophytic relationship between the metastatic cells of well-differentiated adenocarcinomas and the lung. As the malignancy of the tumour cells increases, so the ability to produce specific mucins decreases and, simultaneously, the tumours cease to maintain their alveolar pattern.

Insect contamination of a cornea stored in M-K medium.

We report the unusual case of contamination with an insect of corneal donor tissue stored in M-K medium. Some possible explanations of this occurrence are proposed, and suggestions are made to help prevent its recurrence.

Schirmer test after topical anesthesia and the tear meniscus height in normal eyes.

The height of the inferior tear meniscus was measured in 86 normal eyes. A value greater than 0.1 mm was obtained in 93% of the eyes that were studied. However, attempts to correlate meniscus height with subsequent Schirmer test results showed that these measurements varied randomly. Schirmer tests that were conducted on 265 eyes without instillation of 0.5% proparacaine hydrochloride and on 466 eyes with proparacaine showed that topical anesthesia reduced mean test values by 40%. When proparacaine was used prior to testing, no decrease in tear production with advancing age was demonstrated, nor were results significantly different in males and females. Schirmer test values, ranging from 0 to 3 mm, were obtained in 15% of the normal volunteers when the test was performed after instillation of topical anesthesia and after blotting of the tear lake from the inferior cul-de-sac.

Intraocular penetration of miconazole in rabbits.

Data from this in vivo albino rabbit study suggest that miconazole nitrate may penetrate the ocular compartments better than either natamycin or amphotericin B after intravenous, subconjunctival, or topical administration. The concentrations of miconazole in cornea and in aqueous humor after either topical or subconjunctival administration were very high, and a further threefold increase in the levels was seen if the corneal epithelium had been removed prior to drug therapy. Miconazole was found in the vitreous in some animals after subconjunctival injections of the drug. Intravenous administration produced high concentrations of miconazole in the aqueous humor, which rapidly fell over eight hours. No signs of toxicity or adverse reactions were found in these short-term experiments. Miconazole may be a useful addition in our methods of treating keratomycosis and oculomycosis.

Lack of blood group antigen A on human corneal endothelium.

Using direct immunofluorescence and mixed agglutination, we did not find blood group antigen A on human corneal endothelium from patients of blood type A or AB. The antigen was easily detected on the epithelium of these patients. We believe that the ABO blood group antigens are probably absent on human corneal endothelium, therefore they cannot play a direct role in specific corneal endothelial rejection.

The immunopathology of Mooren's ulcer.

In a 42-year-old man with bilateral severe Mooren's ulcers, results of immunologic studies were normal, with the exception of an in vitro blastogenic response of the patient's lymphocytes to corneal stroma. The histologic studies revealed vast numbers of vacuolated neutrophils with intracytoplasmic phagosomes in and about the ulcerating cornea. Although the progression of ulceration was initially unaltered by surgical and pharmacologic trials, systemic immunosuppression with methotrexate may have prevented ulceration of the lamellar tectonic graft.

The brown bowel syndrome: a possible smooth muscle mitochondrial myopathy?

Two cases of lipofuscinosis of the gastrointestinal tract are described, and a mitochondrial origin of the pigment is proposed. The mechanism of formation of lipofuscin is discussed, with particular reference to the maintenance of structurally and functionally intact mitochondrial membranes. The central role of vitamin E is considered, and its biochemical significance to mitochondrial metabolism is emphasized. Comparisons are drawn between the abnormalities demonstrated within the smooth muscle cells of the two cases described, and the muscle cells observed in cases of skeletal muscle myopathies of mitochondrial origin. It is proposed that lipofuscinosis of the gastrointestinal tract, otherwise known as the 'brown bowel syndrome', may be regarded as a smooth muscle myopathy of mitochondrial origin.

Ascorbate therapy in impaired neutrophil and monocyte chemotaxis. With atopy, hyperimmunoglobulinemia E, and recurrent infection.

A Candida albicans corneal ulcer developed in a 24-year-old man with a history of eczema, asthma, and multiple bacterial infections since childhood. The infection responded well to oral flucytosine (12 g/day for 15 days) and topical amphotericin B. Positive laboratory findings included eosinophilla, hyperimmunoglobulinemia E, and impaired neutrophil and monocyte spontaneous migration and chemotactic responses. Ascorbic acid corrected the monocyte defect in vitro and in vivo, but had no effect on neutrophil function.

The mode of mitochondrial degeneration in gliomas.

Morphologically abnormal mitochondria from human glial tumours are described. For each tumour both the appearances of the mitochondria, and the subsequent mode of degeneration and formation of osmiophilic pigment is characteristic. The significance of these observations is discussed, and it is suggested that the mode of degeneration observed reflects directly a fundamental abnormality in composition compared to normal mitochondria.
