An effective heat strain index (EHSI) using a hand calculator with memory is suggested for on-site evaluation of prevailing hot ambient conditions. The inputs to the programmed calculator include dry-bulb, wet-bulb and globe temperatures, and estimates of metabolism and air movements. The index is based on a program for calculation of the total heat balance and on the efficiency of sweating. The display of information on the ambient conditions EHSI display is one of following: no strain; low strain; high strain; or time limits of exposure.

Adenolymphoma (Warthin's tumor) is a well studied benign tumor of the salivary gland. Malignant transformation of such a tumor is rare and not well documented in the literature. The light microscopic and ultrastructural features of an undifferentiated carcinoma arising in an adenolymphoma in the parotid gland of a middle aged male are described, and the relevant literature is reviewed. Similarities between the benign adenolymphoma and the undifferentiated malignant tumor, such as the presence of interstitial lymphoplasmacytic cell infiltrates, dark and light epithelial cells, similar cytoplasmic organelles, and nuclear morphology, suggest a malignant transformation of a previously existing benign adenolymphoma.
A battery of cognitive tests was administered to groups of younger (34--49) and older (50--59) alcoholics and nonalcoholic control subjects. Regardless of age, alcoholics were found to be impaired on essentially all measures of learning and memory. The relevance of these findings to the premature-aging hypothesis is discussed.
A battery of challening tests was used to assess learning ability and short-term memory in groups of detoxified chronic alcoholics with and without complaints of memory impairment, alcoholic Korsakoff patients, and nonalcoholic controls. While alcoholics without memory complaints did not differ from controls on standardized clinical memory tests, their performance was significantly impaired on our more demanding experimental tests. In contrast, the performance of alcoholics reporting memory complaints was impaired, relative to the other alcoholics, on both clinical and experimental memory tests, overlapping that of the alcoholig Korsakoff patients. These results are consonant with Ryback's continuum-of-impairment hypothesis.
Alcoholics abstinent at least 4 weeks took longer to learn a list of noun pairs than did nonalcoholic controls, yet were able to remember as well as the controls when provided with mnemonics.

Any patient with elevated blood pressure on at least two occasions should have a hypertensive workup; however, extensive evaluation for a correctable cause of hypertension is seldom necessary unless findings in the history, physical examination, or basic laboratory tests are abnormal. Diuretics are the cornerstone of therapy. If blood pressure does not respond adequately to these agents, a step-care approach with more potent antihypertensives is indicated.

Twenty-five cases of extraluminal and 47 of intraluminal inferior vena caval (IVC) occlusion for management of pulmonary embolism were reviewed. A comparison of results obtained with both methods suggests that the IVC umbrella filter provides the safer, more reliable means of IVC interruption in patients whose condition fulfills the criteria for caval occlusion. The simplicity of this technique and the fact that it can be performed under local anesthesia justify its consideration as the procedure of choice for IVC occlusion and permit its use in severely ill patients. On the basis of our positive findings, we now recommend that extraluminal occlusion be reserved for patients in whom insertion of the IVC umbrella is technically impossible.
