Early T-cell features in blast crisis of Ph1-positive chronic myeloid leukaemia.

A patient with Ph1-positive chronic myeloid leukaemia (CML) presented in extramedullary blast crisis. Whereas the peripheral blood and bone marrow features were consistent with the chronic phase of CML, study of the enlarged lymph nodes demonstrated massive replacement by Ph1-positive blast cells of lymphoblastic morphology. Such blast cells showed diffuse acid phosphatase positivity, were positive for TdT, and had an enzyme pattern (adenosin-deaminase, purine-nucleosidephosphorilase and lactate-dehydrogenase) typical of immature T-cells. To further characterize the phenotype of the blast cells, they were analyzed for surface markers using a panel of monoclonal antibodies selected to identify differentiation antigens of T cells, B cells and myeloid cells. The results of the latter analysis were consistent with an early T-cell origin of the blast cells, since they were positive for TdT, CRIS1 (T1), E rosettes and OKT10, and were negative for OKT3, Leu3 and OKT8. These features demonstrate that T-cell markers may also be expressed in blast crisis of CML and provide evidence that T-cells may share a common stem cell with myeloid and B-cells in CML.

Combined study of lymphocyte phosphoglucomutase (PGM) and adenylate kinase (AK) isoenzymes in the early characterization of bone marrow engraftment.

A starch-gel electrophoretic study of 2 peripheral blood lymphocyte enzymes, phosphoglucomutase (PGM) and adenylate kinase (AK), was performed in 25 donor-recipient allogenic bone marrow transplantation (BMT) pairs. A different isoenzymatic pattern between donor and recipient for PGM, AK or both, was observed in 11 of these 25 pairs. From these 11 cases, an identical donor isoenzyme pattern was observed in 10 cases between 24 and 85 days (mean 41.8, SD 22.08) after BM infusion confirming engraftment, whereas in the remaining 1 the demonstration could not be substantiated due to early death. The present study demonstrates that, in addition to other cell markers, the simultaneous study of PGM and AK isoenzymes allows confirmation of BM engraftment in about 45% of cases, much earlier than is possible by means of red cell antigen analysis. Moreover, it is a simple, relatively inexpensive and rapid laboratory tool for a better characterization of the established chimera.

Characteristics and prognostic value of pleural effusions in non-Hodgkin's lymphomas.

The characteristics of 26 pleural effusions associated with non-Hodgkin's Lymphomas (NHL) have been analyzed (13% of the 200 NHL reviewed). Twenty-one of them were exudates and 5 chylous. Cytological examinations were positive in 18/21 of exudate effusions and so were 11 of the 15 pleural biopsies performed. Three of the chylous effusions were associated with chylous ascites, the pleural biopsy being positive in one case. In only 4 cases mediastinal adenopathies were found (3 exudates and one chylous). In half of the cases, the effusion disappeared as a response to chemotherapy. Intrapleural treatment with doxorubicin was performed in 6 patients with exudates, but the pleural effusion disappeared in only one case. The survival was shorter (estimated median survival, 6 months) in those patients in whom the pleural effusion did not disappear under systemic chemotherapy than in those in whom a response was achieved (estimated median survival greater than 40 months; p less than 0.01).
