INTRODUCTION: The risk of developing parenterally transmitted infections in health-care personnel has become a challenge because of high costs, laboral incapacity and mortality, and social stigmatization. OBJECTIVES: To inform the incidence of occupational exposure in our institution, to determine the type of personnel affected and the circumstances of the injuries, and to assess the serologic follow-up of these employees. METHODS: The occupational injuries spontaneously reported to our infection control program from June 1987 to December 1993, were reviewed. Personnel categories, type of accident, instrument and circumstances of the injury, as well as serologic follow-up for hepatitis B virus (HBV), hepatitis C virus (HCV) or human immunodeficiency virus (HIV) infections, baseline and every three months during the first year were evaluated. RESULTS: In 6.5 years 260 injuries were registered in 240 workers. Two cases were registered in the second semester of 1987, 23 in 1988, 25 in 1989, 23 in 1990, 36 in 1991, 90 in 1992, and 61 in 1993. Housekeeping and maintenance staff were mainly affected (32%), nursing staff in second place (27%), followed by senior medical students (20%). Eighty-eight percent were sharp injuries (20% considered deep injuries). Most of the injuries occurred after instrumental utilization: sharp devices disposed without the use of adequate containers (18%), inappropriate handling of sharp containers (14%) and recapping needles (11%); 36% of the injuries occurred during specific procedures. One HBV seroconversion occurred in 10 surface antigen exposures, one in nine exposures with an HCV contaminated source, and none in 54 accidents with HIV contamination. CONCLUSIONS: The increase in the annual incidence of occupational exposures is due most probably to more awareness to report the injuries. Eighty-eight percent were sharp injuries and at least 43% could have been prevented. Our main educational and preventive efforts should be directed to housekeeping staff, nursing staff, and medical students of our institution.
Nosocomial bacteremias are a important cause of mobility and mortality for hospitalized patients which, in addition, generate additional and elevated costs to the institutions. Its frequency has increased in the last year in relation to a greater number of patients, many of them immunosuppressed, which are submitted to invasive procedures. In this study, we analyzed the primary nosocomial bacteremias which occurred in our institution, carrying out a comparative case and control study in order to find the main risk factors which condition their occurrence. In a period of 42 months, 83 episodes of nosocomial primary bacteremias were detected. These represented an incidence of 8 per 1000 of patients which left the hospital and with a clear rising incidence in the last six months. In the Intensive Care Unit (ICU) a greater rate was found per sector (21.3 x 1000). The predominating agents were S. epidermidis, enterobacteremia, S. aureus and enteroccocci. Of the 15 possible risk factors analyzed by multiple regression, those significant were over two weeks of hospital stay and the use of parenteral feeding. These results make it imperative to reinforce control measures in the preparation of parental feeding solutions as in patients with prolonged hospital stay, especially in the ICU.
UNLABELLED: Communication of results a study the effect of open and closed-heart surgery in the immune system of infants and children. Data collected 24 hrs before anesthesia and surgery and five days after surgery. Operating room and pediatric intensive care of Hospital Central de Petróleos Mexicanos (PEMEX) in the South, Mexico City. Children undergoing surgery for correction of congenital heart disease (age 16 months to 14 years). A total of 16 patients. MEASUREMENTS AND RESULTS: increased neutrophil counts with luymphopenia in both groups (p < 0.05), serum levels of the complement components C3 and C4 were higher after surgery, serum immunoglobulin IgG, IgA and IgM were higher after surgery, serum immunoglobulin IgG, IgA and IgM were decreased form preoperative levels (p < 0.01). Two patients had infection in the surgical wound. The effect of open and closed-heart surgery produced transitory immunodeficiency with recuperation of his immune systems and 5th day after surgery.
The authors have studied some usual parameters of the evaluation of the thyroid gland function, after secondary irradiation during tele-cobaltherapy treatment in 20 patients with larynx carcinoma. The parameters studied have been iodine uptake, Hamolsky index, T4, FT4 PB127I and cholesterol. The values, statistically treated, enable the conclusion that normal adult thyroid gland has a very low clinical and analytical radiosensibility.
