Cardiac and extracardiac flows in discordant twins.

OBJECTIVE: Our purpose was to evaluate Doppler-detectable differences in the fetal circulation of discordant twins with a growth defect resulting from either placental insufficiency or twin-to-twin transfusion syndrome. STUDY DESIGN: Serial weekly Doppler recordings were performed for at least 3 weeks preceding delivery in 15 pairs of dichorionic twins (group A) in which the smaller twin had fetal distress (i.e., antepartum fetal heart rate late decelerations) and in 10 pairs of diamniotic and monochorionic twins (group B) in which the diagnosis of twin-to-twin transfusion syndrome was confirmed postnatally. Doppler recordings were obtained from umbilical artery, descending aorta, and middle cerebral artery, and the pulsatility index values were calculated. Furthermore, peak velocity from cardiac outflow tract and the percent of reverse flow in the inferior vena cava were calculated. For all these index values the intertwin differences (delta value) were calculated by subtracting the values obtained in the larger twin with those of the smaller twin. RESULTS: In group A significant changes of delta values were evidenced for all the parameters tested. In particular, delta values of pulsatility index from the umbilical artery and descending aorta progressively increased approaching the occurrence of late decelerations, whereas the delta value for the middle cerebral artery reached a nadir 2 weeks before delivery. Similarly, delta values of peak velocity from outflow tracts significantly decreased, whereas those of the percent reverse flow in the inferior vena cava increased during the time considered. In group B fetuses no significant intertwin differences in pulsatility index values were evidenced in the vessels investigated, resulting in absence of modifications in delta values during the time interval considered. Moreover, significant changes were found in delta values of both the peak velocity from the outflow tract and the percent of reverse flow in the inferior vena cava. However, these changes were limited to the last recording, where the former delta value increased and the latter decreased. CONCLUSIONS: Serial Doppler recordings may show hemodynamic changes in the fetal circulation of discordant twins. Different trends occur according to the underlying pathophysiologic mechanisms of the growth defect. The knowledge of these temporal changes may be useful in the management of such fetuses.

Hysteroscopic Appearance of Benign and Malignant Endometrial Polyps. What are the Differences?

The objective of this study is to evaluate the differences between benign and malignant endometrial polyps at hysteroscopy. In our collected experience of 1793 diagnostic hysteroscopies performed from December 1989 to December 1993, we found 3 malignant polyps in 151 cases of hysteroscopically diagnosed endometrial polyps. The differences in appearance of the malignant polyps are mainly represented by surface irregularities such as necrosis, vascular irregularities and whitish thickened areas. We conclude that when these patterns are present it is better to perform a targeted biopsy to confirm the hysteroscopic diagnosis. This approach allows selection of patients that can be treated by hysteroscopic polypectomy from those who require more radical treatment.

Hysteroscopic Evaluation in Patients with Thickened Endometrium at Vaginal Ultrasound Examination

The purpose of this study was to evaluate the hysteroscopic appearance of the endometrium in cases with abnormal thickening (>8mm) evaluated by vaginal ultrasound. We considered postmenopausal patients who were asymptomatic and those with abnormal uterine bleeding (AUB). Hysteroscopy with endometrial biopsy revealed cancer, hyperplasia, polyps, and myoma in patients with AUB. In the asymptomatic group with increased endometrial thickening, hysteroscopy revealed hyperplasia, polyps and myoma. Our results show that vaginal ultrasound is a reliable method for evaluation of the endometrium in postmenopausal women. Hysteroscopic examination is required to assess the endometrial pathology, and to determine which patients will require biopsy or surgical intervention.

Hysteroscopic Findings of Placental Site Nodule (PSN). A Case Report

The hysteroscopic findings of placental site nodule, an unusual and cytologically alarming lesion, is reported. A 29-year-old woman, para 2, presented with a 3 month history of intermenstrual bleeding. The last spontaneous delivery occurred 12 months previously. Pelvic examination was negative. Ultrasound examination revealed increased thickness and irregularity of the endometrium (16-18 mm). Diagnostic hysteroscopy performed the same day revealed the endometrial surface to be irregular due to a focal nodular lesion with increased regular vascularization in the fundal area. The nodule was about 2 cm in diameter, had an irregular surface and a variegated appearance, and was white-red in color with hemorrhagic and necrotic areas. A targeted biopsy of the lesion was performed. Histologic examination revealed a "placental site nodule." A subsequent D&C confirmed the diagnosis. Fifteen days after surgery, hPL and hCG serum levels were evaluated and were negative. This is the first hysteroscopic report of a placental site nodule. We conclude that hysteroscopic evaluation of abnormal uterine bleeding is very important, because it allows the examination of the uterine cavity and lesions, making possible a guided biopsy. Hysteroscopy is also useful during the follow-up of this type of pathology.

The use of topical anesthesia in diagnostic hysteroscopy and endometrial biopsy.

STUDY OBJECTIVE: To determine whether the pain and discomfort of routine hysteroscopy with endometrial biopsy to diagnose infertility and endometrial pathology can be minimized by topical application of mepivacaine. DESIGN: Prospective, randomized, double-blind study. SETTING: The Department of Obstetrics and Gynecology at a teaching hospital in Rome, Italy. PATIENTS: Eighteen women undergoing diagnostic hysteroscopy. INTERVENTIONS: Hysteroscopy and endometrial biopsy were performed after transcervical injection of 5 ml 2% mepivacaine or 5 ml saline solution into the uterine cavity. MEASUREMENTS AND MAIN RESULTS: Difficulty introducing the hysteroscope was rated by the operator on a scale of 1 to 3. An observer scored visible signs of each woman's distress using a three-point scale. The patients reported their pain 15, 30, 60, 120 minutes after the procedure on a visual analog scale. Mepivacaine was more effective than placebo according to all measurements. CONCLUSIONS: Topical mepivacaine reduced the pain experienced during and after hysteroscopy and endometrial biopsy.

Vaginal micropapillary lesions are not related to human papillomavirus infection: in situ hybridization and polymerase chain reaction detection techniques.

The objective of this study was to assess the human papillomavirus DNA presence in vaginal papillary lesions, with particular regard to micropapillomatosis to better define their clinical significance. Prospective study: the study population was composed of 62 women who were recruited consecutively from the Colposcopy Centre of the Ancona University, Department of Obstetrics and Gynecology, on the grounds of vaginal papillomatosis or/and typical acuminata warts. Biopsies for routine histology, and for human papillomavirus (HPV) DNA detection by means of in situ hybridization and polymerase chain reaction (PCR) were taken from the papillary lesions and from 24 healthy women, who were selected as controls. Macroscopically, vaginal micropapillomatosis was ascertained in 51 cases (82.3%), while in 11 cases (17.7%) the colposcopic diagnosis was condyloma acuminatum. During in situ hybridization, HPV DNA positivity was observed in 8 (9.4%) out of 85 samples of squamous papillae and in 11 (64.7%) out of 17 samples of condylomata; in control specimens, HPV DNA was detected in 2 (8.3%) out of 24 bioptic samples. The correspondence between in situ hybridization and PCR was 96.1%, with 17.4% more diagnosis obtained by PCR. Vaginal micropapillomatosis may be regarded as a variation in the normal anatomy of the lower genital tract without any significant relationship with HPV infection, and as a lesion easily distinguishable from condylomata acuminata by clinical examination alone.

Effects of Braxton-Hicks contractions on fetal heart rate variations in normal and growth-retarded fetuses.

The objective of this study was to evaluate whether Braxton-Hicks contractions induce changes in fetal heart rate variation in normal and growth-retarded fetuses. 110 uncomplicated singleton pregnancies were cross-sectionally studied as well as 16 pregnancies complicated by fetal growth retardation secondary to uteroplacental insufficiency. Fetal heart rate variability was analyzed by a commercially available computerized system (2CTG Hewlett Packard, Italy) 10 min before and 10 min after the Braxton-Hicks contraction. All the included fetal heart rate tracings fulfilled the following criteria: (1) presence of a single Braxton-Hicks contraction in the 20 min considered; (2) absence of fetal heart rate decelerations after the contraction, and (3) stable fetal heart rate behavioral pattern in the period analyzed. 82 tracings of normal fetuses were analyzed during an active fetal heart rate pattern (type B) and the remaining 28 during a quiet pattern (type A). In both patterns no significant differences in delta value, long-term irregularity, short-term variability and interval index were found before and after the contraction. All the tracings of growth-retarded fetuses were analyzed during the fetal heart rate pattern A. Short-term variation and interval index significantly decreased during the first 5 min after the contraction while no significant differences were found in the other indices investigated. The decrease in these indices was significantly more marked in those fetuses developing fetal distress within 7 days. In conclusion, Braxton-Hicks contractions induce a significant decrease of short-term variation and interval index only in growth-retarded fetuses. This can be useful in the early identification of fetal compromise in such fetuses.

Interferon alpha 2b treatment of cervical intraepithelial neoplasia grade 2: modulation of natural killer cell.

The objective was to evaluate natural cytotoxicity of peripheral blood during interferon treatment in cervical intraepithelial neoplasia grade 2 (CIN2), as index of interferon activity. Twenty-one patients with CIN2, histologically proven, were treated with interferon alpha 2b (Intron A, Sheering-Plough Corp.), in a dose of 3,000,000 units three times per week for 8 weeks self-administered by intramuscular injection. The rate of remission to the interferon treatment was 38.1%. Eight healthy patients had a significant increase in natural killer activity during and after the treatment (p < 0.001), whereas the 13 nonresponder patients remained with low values of natural killer activity. By analyzing the basal natural killer activity before the treatment, the patients with clinicopathologic remission had a significantly higher mean value than nonresponder patients (p = 0.007). Notwithstanding the specimen exiguity, the individual basal natural killer activity seems to be a predictive parameter of interferon treatment response in patients with CIN2.

Analysis of factors influencing ventricular filling patterns in fetuses of type I diabetic mothers.

In an effort to determine the factors influencing the abnormal ventricular filling patterns of fetuses of type I diabetic mothers, Doppler flow velocity waveforms were recorded from fetal atrioventricular valves in 37 pregnancies complicated by type I diabetes immediately before an elective cesarean section. The ratio between the peak velocities during early passive ventricular filling and active atrial filling was calculated at the level of both atrioventricular valves and related to different factors including ventricular chamber wall thickness, heart rate, umbilical vein hematocrit and time to peak velocities values obtained at the outflow tract. Multiple stepwise regression demonstrated that the interventricular wall thickness, heart rate and hematocrit values significantly and independently affected the ratios between early and active ventricular filling from mitral and tricuspid valves. As a consequence all these factors should be taken into account in the interpretation of atrioventricular Doppler indices.

The relationship of clinical-pathologic status and adjuvant treatment with natural killer cell activity in stage I and II endometrial carcinoma.

OBJECTIVE: The aim of our study was to evaluate the basal immune reactivity in patients with locally advanced endometrial carcinoma, and the immune modulating effect of adjuvant treatment in cancer population randomized to endocrine or radiation therapy. MATERIAL AND METHODS: Forty-three patients with FIGO stage I and II endometrial carcinoma, treated with primary radical surgery, were randomly selected to receive endocrine (medroxy-progesterone acetate 30 mg weekly and Tamoxifen 300 mg weekly, given consecutively) or radiation adjuvant treatment (external beam photon treatment of the whole pelvis, with an average total dose of 3680 cGy rad). The immune assay included the evaluation of natural killer cell activity by target cell retention of the fluorescent dye carboxyfluoresceyn diacetate, using sensitive cell line K 562. The immunological monitoring was performed before surgery, and then before, during, and after adjuvant treatment. Nine patients, who refused any adjuvant treatment, were recruited as controls. RESULTS: Patients, matched for age and demographic characteristics, with locally advanced endometrial carcinoma had significantly lower mean values of natural killer activity than in healthy controls; the decrease of natural cytotoxicity was significantly related to the depth of myometrial invasion. The adjuvant treatment was associated with a significant immune modulation: patients in endocrine therapeutic regimen showed an increase of natural killer activity, while patients in radiation therapy had a reduction; in the control group there was no significant modification of natural cytotoxicity during negative follow up. CONCLUSIONS: Natural killer cell activity of peripheral blood is significantly reduced in local advanced endometrial carcinoma patients. The natural cytotoxicity evaluation, as a function of therapeutic modality, may be useful in establishing a relationship between adjuvant treatment and immune status in endometrial carcinoma.

Serum CA-125 concentration in endometriosis patients: role of pelvic and peritoneal irritation.

Serum CA-125 concentrations were investigated preoperatively in 91 consecutive women undergoing laparoscopy for infertility, pelvic pain and/or annexial cysts. The presence and extent of endometriosis were carefully assessed, including the American Fertility Society stage of disease, and implant and adhesion scores. Postoperative CA-125 measurements were obtained in 32 of 53 endometriosis patients and evaluated with respect to clinical evolution of the disease. Serum levels of CA-125 were significantly increased in patients with endometriosis (46.5 +/- 39.5 vs. 13.5 +/- 7.3 U/ml in controls, p < 0.001) and correlated with the severity of disease. A positive correlation (r = 0.7, p < 0.001) was observed between adhesion score and CA-125 levels, while the relationship with implant score was not significant (r = 0.3, p = 0.07). CA-125 level was also significantly increased in women with peritoneal endometriosis (70.7 +/- 47.3 vs. 33.5 +/- 25.6 U/ml for those with ovarian endometriosis), and in these patients the post-operative CA-125 level was significantly related to the clinical evolution of the disease, being higher in patients whose disease recurred compared to those with negative follow-up, irrespective of the adhesion score. We conclude that in endometriosis patients, serum CA-125 level is directly related to the adhesion score and peritoneal involvement, suggesting a central role of pelvic and peritoneal irritation in the increased level of this serum marker.

Calcium, phosphorus and magnesium intakes correlate with bone mineral content in postmenopausal women.

Qualitative and quantitative differences in the dietary habits of postmenopausal women were studied to assess their influence on bone health and osteoporosis. A total of 194 postmenopausal women were studied with forearm DEXA densitometry. 70 were osteoporotic and 124 served as controls. Women had been menopausal for 5-7 years, and had never been treated with hormone replacement or drug therapy. A 3-day dietary recall was completed on Sunday, Monday and Tuesday after the examination: the results were processed by computer and daily calcium, phosphorus and magnesium intakes were related to bone mineral content (BMC). Data were compared with Student's t-test and significance was assessed at p < 0.05. Regression analysis was performed to correlate BMC and intake levels. The dietary intake of calcium, phosphorus and magnesium was significantly reduced in osteoporotic women and correlated with BMC. Calcium and magnesium intakes were lower than the recommended daily allowance even in normal women. The results suggest that nutritional factors are relevant to bone health in postmenopausal women, and dietary supplementation may be indicated for the prophylaxis of osteoporosis. Adequate nutritional recommendations and supplements should be given before the menopause, and dietary evaluation should be mandatory in treating postmenopausal osteoporosis.

Evaluation of pulsatility index nomograms based on fetal biometry in small for gestational age fetuses.

Reference limits for the PI from the umbilical, middle cerebral, and renal arteries were constructed using BPD, AC, FL, and transverse cerebellar diameter as independent variables and their efficacy tested in a population of SGA fetuses. Therefore, 153 normal fetuses and 90 SGA fetuses with established dates between 20 and 40 weeks of gestation were considered. Normal fetuses showed a linear negative relationship between the PI from all the vessels investigated and all the biometric parameters considered. Although the BPD related better with the PI from the umbilical artery (r = 0.646) and the renal artery (r = 0.765) and the transverse cerebellar diameter related better with middle cerebral artery PI values (r = 0.510), no evident differences in fitting were found among the variables tested. In SGA fetuses the nomograms on BPD, AC, and FL significantly underestimated PI values in all the vessels studied when compared to the nomograms based on gestational age, while a similar ability in identifying abnormal PI values was found for nomograms based on gestational age and transverse cerebellar diameter. These newly developed nomograms based on transverse cerebellar diameter may prove useful in the evaluation of Doppler indices of fetuses with uncertain gestational age.

Nifedipine reduces pressor responsiveness to angiotensin II in pregnant women.

OBJECTIVE: We assessed the action of nifedipine on vascular reactivity to angiotensin II (AII) in pregnant women at risk for hypertension. METHODS: We studied eleven pregnant women (28-32 weeks' gestation) who had shown a 20 mmHg increase in basal diastolic blood pressure at AII infusion rates < 10 ng/kg/min (Effective pressor dose, EPD), and were therefore considered at high risk for the subsequent development of pregnancy-induced hypertension, according to Gant. After the AII infusion was completed, we allowed the patients 4 hours of rest to avoid interactions with the first test, then administered 10 mg nifedipine, and after 30 minutes repeated the test. RESULTS: In all the 11 women the EPD after nifedipine administration had significantly reverted to normal (paired t-test: p < 0.03). CONCLUSIONS: The efficacy of nifedipine in reducing the pressor response to AII suggests the involvement of intracellular free calcium in the vascular response to pressor agents in pregnancy, and supports the use of this drug in the treatment of pregnancy-induced hypertension.

Na,K-ATPase from placenta of women with pregnancy-induced hypertension exhibits an increased affinity for cardiac glycosides.

Microsomes were prepared from placentas of normotensive women and of patients suffering from pregnancy-induced hypertension (PIH). Activity of Na,K-ATPase (estimated as ATP hydrolysis) from the hypertensive tissue was lower than from tissue of normotensive women, even if the number of Na,K-ATPase molecules (monitored by anthroyl ouabain binding) was actually greater in the hypertensive tissue. The affinity of Na,K-ATPase for anthroyl ouabain was about four times higher in plasma membranes of hypertensives, indicating some structural change in the Na,K-ATPase or in its vicinity. Assuming the presence of an endogenous digitalis-like factor, the results suggest a simple way of explaining not only the lower Na,K-ATPase activity in the placental membranes of hypertensives but also the different extent of enzyme inhibition in different tissues of PIH patients.
