[Estrogens following menopause. Methods of hormone replacement].

Even today hormone replacement therapy concerns still only a rather small number of post-menopausal women: 19% of women aged 50-75 on the basis of prescriptions. The compliance of oestrogen substitution reflects even more a general reticence against the use of hormones within the public opinion and among physicians. Thus, hard work for widespread information remains to be achieved, based on scientifically well established data. The purpose of this presentation is to describe benefits and drawbacks of the modes of administration of oestrogens and of oestro-progestative associations and to consider the new developments in this matter.

[Clinical evolution of LH-RH reactivity in gynecology (author's transl)].

Impaired LH response to LH-RH has been found in 2/4 cases of primary amenorrhoea, in 2/8 cases of psychogenic secondary amenorrhoea, in 2/4 cases of secondary amenorrhoea associated to a pituitary tumour and in 1/5 cases of iatrogenic secondary amenorrhoea post hormonal contraception. Therefore, this test appears not to differentiate these pathologic conditions.

Bone mineral content of women with uterine fibromyomas.

OBJECTIVE: To determine if the bone mineral content (BMC) of women suffering from fibroids is different from that of the reference population. DESIGN: Cross-sectional retrospective study. PARTICIPANTS: Fifty-nine women suffering from fibroids, compared to the reference population. OUTCOME MEASURES: BMC was measured at the vertebral L2-L4 site (BMC-1) and at the midradius site (BMC-r). RESULTS: The patients with fibroids exhibited no significant difference in BMC-r). RESULTS: The patients with fibroids exhibited no significant difference in BMC-r, but had a significantly higher (P < or = .001) BMC-1 than the reference population. CONCLUSION: Since BMC-1 is mainly constituted by trabecular bone and BMC-r by cortical bone, and since trabecular bone is more sensitive to estrogens than cortical bone, it is suggested that a hyper-estrogenic state co-existing with fibroids may induce a slight protective factor against osteoporosis of the vertebral skeleton.

[Prolactinemia in primary hypothyroïdism : basal and dynamic studies--signification of galactorrhea associated with myxedema (author's transl)].

PRL and TSH were measured by radioimmuno-assay in 14 persons wth myxoedema aged 16 to 65 years. Three of them had galactorrhea. In 13 patients the circadian rhythm of PRL was studied, and in 8 patients PRL was measured after TRH administration. All these patients have a high value of serum TSH, with a mean value +/- SEM of 29,4 (24,3 - 35,6) mU/ml. Values of serum PRL were comprised in normal ranges with a mean value +/- SEM of 126(196 - 238) mU/ml. The circadian rhythm of PRL is not altered, since mean values of serum PRL are more elevated at 4 am than 8 am with highly statistical significance. After THR administration the PRL response is normal or exaggerated.

Amenorrhoea, sterility and hyperprolactinaemia. Importance of complex movement tomographic x-ray study and follow-up of the sella turcica.

In a 24 y.o. woman complaining of primary amenorrhoea and infertility, hyperprolactinaemia and clearly enlarged sella turcica on standard x-rays in 1975 led to the diagnosis of a pituitary prolactin-producing adenoma, later confirmed surgically. Galactorrhoea never occurred spontaneously and could not be provoked at physical examination. In the course of a previous investigation in 1967, the standard x-ray of the sella turcica, although showing already a minor duplication of the anterior wall of the sella, had been misinterpreted as being normal. It is clear from the present observation that repeated, for example at yearly intervals, radiological examinations and prolactin determinations (not available before 1971) would allow an early diagnosis. It is furthermore stressed that a tomographic radiological examination using complex movement (spiral or hypocycloidal) should be mandatory in any case of amenorrhoea with hyperprolactinaemia in order to assess or not the possible existence of a prolactin-producing pituitary adenoma. Indeed, dynamic studies of anterior pituitary secretions cannot allow a differential diagnosis between tumoural and functional hyperprolactinaemia.
