The clinical course of a case of hepatoblastoma in an infant following the use of an oral contraceptive during the Ist month of pregnancy is reviewed. The following points are worthy of considerations: age of onset, clinical silence, rapid terminal course, AFP negativity.
Rat thymus and thymocytes possess an NAD-glycohydrolase activity lower than spleen and spleen lymphocytes. In all cases, the highest activity (50%) is bound to the nuclei, while that in the soluble fraction is higher (in percent) in the lymphocytes than in the whole organ. Considering that thymus and thymocytes are characterized by a soluble NAD-ase activity higher than in spleen and in spleen lymphocytes, and that lymphocytes in general have a greater soluble than microsomal activity, we interpret these findings are evidence that the soluble NAD-ase is due essentially to T lymphocytes, of which it represents a typical feature.
Nicotinamide injected in a high dose (100 mg/100 g body weight) into rats induces several variations both of enzymatic activities and of metabolites levels. We have tested the effect of NAm on liver cAMP content; it increases reaching a peak at the second hour after treatment. Liver adenylate cyclase and 3',5'-cyclic AMP phosphodiesterase activities have been tested; the former increases reaching a peak after 1 hour, while the latter decreases consistently one hour after treatment. This variation of cAMP phosphodiesterase activity could be related to the change of the pyridine coenzymes redox state observed after NAm injection. The increase of the liver cAMP concentration could explain the mobilization of glucose after treatment with NAm, and the alteration of several enzymatic activities, as serine dehydratase, observed after treatment.
Research relating blood pressure levels of 6-14 years-old age schoolchildren has been carried into the whele school-population of Chivasso (Turin) town. Three kinds of armbands of different widths and lengths were used according to age. In conformity with Long's suggestions, the correction factors were used for the age-groups from 6 to 7 years and 8 to 11. On the grounds of the levels taken the quantiles were calculated. The graphs show the shape of the 5th, 25th, 50th, 75th, and 95th percentiles, related to the systolis and diastolic pressures and to the age of the child. The results showed a continous increase in the shape of the levels, which did not depend on the sex variant. A spike in the 9th year can be related to the fact that the correction factor can be less than it should be. The decrease in the 11th year can be put down to the fact that the correction for the armband has not been carried out from the 12th year onwards. 6.52% of schoolchildren have systolic and/or diastolic blood pressure greater than 95 degrees C.

Lung transplantation today is considered an effective option for patients with severe idiopathic pulmonary fibrosis. The standard medical treatment for this disease consists of high-dose steroids alone or combined with other immunosuppressive drugs. Unfortunately, pretransplantation administration of steroids may jeopardize the healing of the airway anastomosis and cause other complications; therefore it is considered a relative contraindication to lung transplantation. For this reason we try to reduce the dose of prednisone to 15 to 20 mg/day or less before the transplantation, but this creates many difficulties and is sometimes impossible in severely ill patients. Therefore we used cyclosporine (4 to 7 mg/kg/day) in 10 patients who were receiving high-dose prednisone (> or = 50 mg/day) therapy, but who were otherwise suitable candidates for lung transplantation. In seven cases prednisone could be tapered to 20 mg/day or less, allowing acceptance in our program. These patients had a CRP score of 60 or more before entering our trial and remained stable at this level after conversion to cyclosporine. The 6-minute walk test showed a mild improvement in five cases (71.5%). Three patients underwent single lung transplantation; two patients are on our waiting list after 3.5 and 4 months of treatment with cyclosporine and prednisone (10 mg/day), and two patients died while awaiting a suitable organ 6 and 7.5 months after starting cyclosporine therapy. Combined administration of cyclosporine and prednisone may extend the waiting time while receiving low-dose steroids and allow more patients with idiopathic pulmonary fibrosis to qualify for lung transplantation while reducing the risk of steroid-induced complications.
