Antithyroid antibodies as an early marker for thyroid disease induced by amiodarone.

Changes in thyroid function may occur during treatment with amiodarone. A double blind prospective trial of amiodarone and placebo was performed in 37 patients in the subacute phase of myocardial infarction. Though none of the patients assigned to receive placebo developed any antibody, six of 13 patients treated with amiodarone developed antithyroid microsomal antibodies. There was no difference in triiodothyronine and thyroxine concentrations between the two groups, but a significant difference in concentrations of thyroid stimulating hormone was noted on day 30 (p less than 0.05). Six months after the withdrawal of amiodarone autoantibodies could not be detected and concentrations of thyroid stimulating hormone were normal. These antibodies have not previously been reported to develop during short term treatment with amiodarone. They could have an important role in the detection of early thyroid changes in patients given amiodarone.

Acidification in the medullary collecting duct following ureteral obstruction.

A defect in urine acidification has been described in obstructive uropathy. Since the collecting tubule from the inner stripe of the outer medulla (OMCTi) is the major site for distal acidification, isolated OMCTi nephron segments from control rabbits and from rabbits after 24 hr of unilateral (UUO) or bilateral (BUO) ureteral obstruction were studied. Tubules were perfused (4 nliter/min) and bathed with an artificial solution resembling rabbit serum ultrafiltrate, and 3H inulin was incorporated in the perfusate as a volume marker. Water absorption (Jv) was -0.03 +/- 0.03 nliter X mm-1 X min-1 in control tubules, and was significantly (P less than 0.05) negative in UUO (-0.48 +/- 0.12 nliter X mm-1 X min-1) and BUO (-0.29 +/- 0.07 nliter/mm-1 X min-1) tubules, as a result of an inulin leak. Bicarbonate absorption (JHCO3) in control tubules was 11.61 +/- 1.21 pmole X mm-1 X min-1 and was significantly lower in UUO tubules (7.59 +/- 1.09 pmole X mm-1 X min-1, P less than 0.05). JHCO3 in BUO tubules although lower than control (7.96 +/- 2.75 pmole X mm-1 X min-1) did not achieve statistical significance because of a high degree of heterogeneity among tubules. To determine whether the acidification disorder was due to a gradient or capacity defect, the ability of the tubules to lower HCO3- concentration (delta HCO3) at low rate of perfusion (1 nliter/min) was examined. No difference in delta HCO3 was found among the three groups being 8.98 +/- 0.54, 9.95 +/- 1.76, and 8.93 +/- 2.19 mmole in control, UUO and BUO tubules respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Rheumatic symptoms and signs in subacute infective endocarditis.

Two hundred patients (90 men and 110 women), mean age 36.2 years, admitted to a Department of Medicine, between 1961 and 1979 with subacute infective endocarditis (SIE) were studied. Rheumatic heart valve disease was the predisposing lesion in 94% and the mitral valve was involved in about 80% of the cases. Diagnosis of SIE was confirmed by blood cultures in 66 patients and at autopsy on another 57 patients. The rheumatic symptoms and signs (RSS) considered were: arthralgia, rachialgia, arthritis, myalgia and Osler's nodes. RSS showed up in 50 (25%) of the subjects and arthralgias were the most frequent (12.5%). There were no significant differences between the 50 RSS patients and the other patients, regarding bacteriological and other laboratory data, including immunoglobulins. RSS patients were significantly younger (P less than 0.001). Out of the 200 patients, 71 (35.5%) died. Eight (16%) of these were in RSS subgroup of 50 patients. Presence of RSS did not worsen prognosis in our patients. Septic bone metastasis was not identified.

[Critical study of the treatment of fractures of the mandible. Apropos of 1 year of activity at the Clinic of Maxillo-Facial Surgery in Lyon].

The authors report findings on a series of 148 injured patients, seen in one year in the Clinic of Maxillo-Facial Surgery in Lyon. The minimum criterion of inclusion in the series was a hairline fracture. After undertaking both aetiological and anatomo-pathological studies they set out what they consider appropriate management of each type of fracture. This is followed by a detailed analysis of the outcome observed over a retrospective period of 1 to 2 years. They stress in conclusion the importance of adapting the treatment to the requirements of each case. Also they note the continued problem of how best to manage fractures of the condyles.

[Reevaluation of the electrocardiographic criteria of left anterior hemiblock associated with lower infarction].

UNLABELLED: A vectocardiographic (VCG) study of 90 consecutive cases of acute inferior myocardial infarction (IMI) was carried out by Frank's method. The overall incidence of intraventricular block was 40% and that of left anterior hemiblock (LAH) 20% (18 cases). The ECG features of the 18 cases of LAH-acute IMI and of the 55 cases of isolated acute IMI were compared with those of 100 cases of confirmed chronic IMI comprising 50 cases of isolated infarction-LAH (on VCG) and with 50 cases of isolated LAH (clinical and VCG diagnosis). The incidence of the six following electrocardiographic criteria were studied: 1) terminal negative forces in Lead II and positive forces in AVR with left axis deviation 2) W-shaped appearances in Leads II and AVF, 3) initial notching in Leads II, III and AVF, 4) abnormal growth of the R wave from Lead II to Lead III, 5) combinations of the four preceding criteria taken two by two, 6) the association of 3 of the 4 preceding criteria (from 1 to 4). RESULTS: A) Acute IMI: 1) isolated infarction: criteria 1 - 7%; 2 - 2%; 3 - 5%; 4 - 2%; 5 - 4%; 6 - 0%; 2) IMI - LAH: 33%, 56%, 61%, 17%, 50%, 6%, respectively. B) Chronic IMI: 1) isolated infarction: criteria 1 - 4%; 2 - 8%; 3 - 6%; 4 - 10%; 5 - 8%; 6 - 0%; 2) IMI - LAH: 72%, 22%, 60%, 74%, 42%, 44%, respectively. C) Isolated LAH: criteria 1 - 80%; 2 - 0%; 3 - 4%; 4 - 20%; 5 - 20%; 6 - 0%. CONCLUSIONS: Some of the ECG criteria studied were relatively sensitive and were suggestive of the association IMI - LAH, but VCG was much more sensitive and specific. Therefore, the incidence of LAH associated with acute IMI is probably underestimated by ECG alone.

[Several cases of emergency repair of loss of skin substance in the child (author's transl)].

Traumatic loss of skin substance in the child is uncommon. There are three main causes: dog bites, scraping lesions and bullet wounds. In all cases, great attention is paid to initial and early treatment of the wounds (exploration, wound care) under general anaesthesia. Provided the operator is competent, dog bites may be completely repaired from the outset. Lesions due to scraping or bullet wounds should be treated more cautiously with emergency treatment not necessarily seeking to achieve closure at any price.
