[Removal of amateur and dirt tattooing with the Q-switched ruby laser].

The Q-switched ruby laser with a wavelength of 694 nm and a pulse duration of 25 ns causes selective damage to skin pigment. This laser has produced good to excellent results in the treatment of amateur professional and traumatical tattoos, and also of benign pigmented lesions such as lentigines, ephelides, café-au-lait spots and Becker nevi. We report on three patients with amateur and traumatic tattoos, which were removed by Q-switched ruby laser treatment without leaving scars.

Contact allergy to imidazole antimycotics.

Between 1977 and 1986, 9 patients with contact allergy to the active ingredient of imidazole antimycotics were found at the Department of Dermatology, University of Heidelberg. The number of positive reactions decreased in the following order: miconazole (6), clotrimazole (3), econazole (3), isconazole (3), and oxiconazole (1). When 5 patients were tested with a series of imidazoles in different concentrations and vehicles (petrolatum, ethyl methyl ketone, ethanol), petrolatum turned out to be the least effective one. The active ingredient at 1% in ethanol seems to be the most suitable choice for routine patch testing. Bifonazole may be the therapeutic alternative for patients sensitive to miconazole or clotrimazole, since no cross reactivity was observed.

[Home treatment of hyperhidrosis of the hands and feet tap water iontophoresis].

Ten patients with severe palmo-plantar hyperhidrosis have been treated successfully at home with tap water iontophoresis for a period not less than 6 months. The patients were asked to evaluate the treatment subjectively; 8 considered the results to have been good to very good. The same results were obtained in an objective evaluation by gravimetry, hygrometry and colorimetry. The full effects were seen after an average of 15 treatments of 20-30 min each. Through the new possibility of treating patients at home, therapy for palmo-plantar hyperhidrosis, which to date was only possible under medical supervision, has been simplified.

[Contact allergies to antifungal agents].

Contact allergies to modern antimycotic agents are comparatively rare considering the high prescription rates. They may be caused by components of either the vehicle or the active ingredient. According to both the literature and our own investigations, the most important sensitizers of the vehicle are cetylstearyl alcohol and propylene glycol; as to the active substances, miconazole and nystatin are the most frequent sensitizers of the imidazole and non-imidazole group, respectively. Cross-reactions of imidazole derivatives mainly occur between miconazole and econazole. For patch testing with imidazoles, we recommend a 1% alcoholic solution of the active ingredient.

[Effect of chamomile on wound healing--a clinical double-blind study].

In a double-blind trial, the therapeutic efficacy of chamomile extract was tested on 14 patients. As objective parameters served the epithelial and drying effect on weeping wound area after dermabrasion of tattoos. The period of the healing and drying process was judged by the doctor. The decrease of the weeping wound area as well as the drying tendency was statistically significant.

[Contact allergy to bufexamac].

Thirteen cases of contact allergy to bufexamac have been seen since 1976 at the Department of Dermatology of the University of Heidelberg. Most of the patients were women (69%). Nine patients suffered from chronic eczema and four had stasis dermatitis. Four patients had used bufexamac for less than 2 weeks. Based on the case histories, about half of the patients were suspected of having contact allergy to bufexamac. The clinical picture was consistent with that of allergic contact dermatitis; a generalized eruption was observed in two cases. Eight patients had additional sensitizations; three patients were allergic to benzyl alcohol, which is used as a preservative in the bufexamac cream available commercially. Contact allergy to bufexamac seems to be rare; however, it should be considered as a cause of sensitization in patients with chronic eczema, even after short-term application.

Hair removal in 40 hirsute women with an intense laser-like light source.

Until recently, previously applied methods to remove hair have ultimately proven ineffective or resulted in the formation of scars and small wounds. Different methods for removing hair in a more or less permanent way have been used: electrolysis, thermolysis and the blend method. In this study we describe the removal of hair without side-effects by means of non-laser incoherent emitted light, produced by the ILS flashlamp. In a multicenter study we treated 40 women with a median age of 38.6 years with hirsute hair growth of different hair colours on the upper lip and chin. In general 76.7% of the hair was removed within 6 treatments, with an average fluence of 38.7 J/cm2 and a mean wavelength of 585 nm per patient. A correlation was found between the percentage reduction of hairs and the number of treatments and between hair removal and needle epilation before treatment. Furthermore, a correlation was seen between hair reduction and wavelengths of 570 nm and 550 nm. No association was found between hair removal and clinical data of the patients, nor between hair reduction and technical data of the device. This study presents a new alternative for hair removal.

Free-floating thrombus in the femoral vein--a challenge in phlebologic diagnostics.

Although thrombophlebitis may be a complication of an incompetent great saphenous vein, it usually has a benign outcome. The risk of embolisation is present especially when ascending progression of superficial venous thrombosis extends to the femoral vein. The proximal extension of the thrombus often precedes clinically visible symptoms. Use of colour duplex sonography is superior to phlebography in assessing the saphenofemoral junction area and thrombus propagation in these cases. Our case report details the difficulties encountered in the diagnosis and therapy of a patient with a free-floating thrombus at the saphenofemoral junction.

[Contact allergy to clotrimazole and azidamfenicol].

A 19 year old man developed an acute contact dermatitis of the anogenital region after the application of an antimycotic cream containing clotrimazole and azida col. Patch testing revealed sensitization to both active ingredients.
