Xanthogranulomatous ureteritis.

A case of xanthogranulomatous ureteritis is described and the numerous similarities with xanthogranulomatous pyelonephritis in their clinicopathological presentation are discussed. Of the xanthogranulomatous pyelonephritis symptoms our patient manifested a long history of urinary tract infection, weight loss, urinary obstruction, pyuria and gram-negative bacteriuria. The ureteral lesion was a yellowish nodule, granulomatous and was highlighted histologically by foamy histiocytes with periodic acid, Schiff positive cytoplasmic granules. The lesion mimicked an obstructing neoplasm and a specific granuloma.

Detection of dengue virus from mosquito cell cultures inoculated with human serum in the presence of actinomycin D.

We report the use of cultures of mosquito cells (TRA-284) to detect dengue virus in serum from cases of dengue fever in the state of Puebla, México. Using the conventional procedure 56 of 171 samples (32.7%) were positive. The negative sera (67.3%) were passaged 'blind' in mosquito cell cultures but no virus was detected. However, when these sera were incubated in the presence of actinomycin D (an inhibitor of deoxyribonucleic acid transcription) 20 of the 115 samples (17.4%) became positive. This procedure increased the virus detection rate from 32.7% to 44.4%. Serotypes 1 and 4 were identified for the first time in the state of Puebla, where the transmission of dengue virus is increasing. The addition of actinomycin D to mosquito cell cultures may improve the detection of dengue virus and could be a useful tool for virological surveillance in endemic countries.

[Scintigraphy with thallium chloride in a case of clear cell sarcoma of the tendons and aponeurosis].

Clear cell sarcoma of the soft parts (CCSSP) is a rare and highly malignant tumor. This is a case report of a 31 years old woman who presented with a tumor in the internal face of left thigh. The tumor biopsy was suggestive of a possible malignant tumor of the peripheral nerve sheath (malignant schwannoma). Biochemical analyses, computed tomography and magnetic resonance were performed and reported a sarcoma of soft parts (CCSSP) without abnormal inguinal lymph nodes. With this diagnostic suspicion, the patient was sent to the Nuclear Medicine Service of our center where a 201Thallium scintigraphy study was performed. In this study, it showed the primary tumor together with a hot spot in the homolateral inguinal region, suggestive of the presence of a metastastic lymph node. This finding would change the surgical attitude in the patient, including inguinal lymphadenectomy. The post-surgical histological study confirmed a clear cell sarcoma of soft parts (CCSSP) with a metastatic inguinal lymph node.

Sonographic, cytogenetic and DNA analysis in four 69,XXX fetuses diagnosed in the second trimester.

OBJECTIVE: To describe the ultrasound findings and its relationship with the cytogenetic study and the origin of the extra haploid chromosome set in four 69,XXX cases. METHODS: Four pregnant women were referred because of abnormal 2nd trimester ultrasound. Karytoypes, FISH and DNA analysis were performed. RESULTS: All cases presented asymmetrical intrauterine growth retardation, marked oligohydramnios and placental alterations and showed a 69,XXX karyotype. In three cases, DNA analysis allowed to establish the origin of the extra haploid chromosome set. CONCLUSIONS: At least three fetuses had a maternal extra haploid chromosome set. Thus, it has been possible to establish the main ultrasonographic markers and to observe the survival of the fetus until the second trimester when they have a maternal origin.

Chromosomal mosaicism for isochromosome 11q confined to CVS direct preparations.

OBJECTIVE: To analyse the discrepancy between the karyotype in direct preparations of chorionic villus sampling (CVS) and the fetal karyotype and its possible fetal phenotypic repercussion. METHODS: The karyotype was obtained from direct and cultured preparations of CVS. FISH was performed in direct CVS preparations and in four different areas of term placenta. RESULTS: Karyotype and FISH analysis in CVS revealed a 46,XX/47,XX,+i(11q) cell line. Cultured CVS preparations showed a 46,XX karyotype. Cytogenetic studies in term placenta did not reveal the abnormal cell line. Molecular studies did not detect uniparental disomy for chromosome 11 in the fetus. CONCLUSION: The fetus, at birth, had no phenotypic abnormalities. IUGR was not present during gestation, in accordance with the low proportion of aneuploid cells in term placenta, and UPD for chromosome 11 was not observed.

In vitro killing kinetics and postantibiotic effect of josamycin and other macrolides on several bacteria.

The antimicrobial activity [minimal inhibitory concentration (MIC) and killing kinetics] and postantibiotic effect (PAE) of different concentrations (MIC and 10 x MIC) of josamycin, erythromycin, midecamycin and azithromycin on Streptococcus pneumoniae, Staphylococcus aureus and Escherichia coli were studied in vitro. The MIC and killing kinetics were determined by standard methods. The PAE was measured after 1 h exposure to the drugs, which were removed by diluting the culture 1,000-fold. All tested macrolides had their lowest MIC against S. pneumoniae, and the most active against S. aureus was josamycin. In killing kinetics, using 10 x MIC, the macrolides exhibited a bactericidal effect against S. pneumoniae and E. coli, with reductions of 3 log10 in colony-forming units per milliliter with respect to the initial inoculum, while a bactericidal effect was not observed for S. aureus. The PAEs were concentration dependent, the greatest PAEs being obtained with S. pneumoniae. Although lower, the PAEs induced on E. coli were significant when high concentrations of the drug were tested. The assayed macrolides showed both more activity and PAEs on S. pneumoniae.

[Evaluation of cefoxitin and cefotaxime screening plates for the detection of methicillin-resistance in Staphylococcus aureus].

Detection of methicillin-resistant Staphylococcus aureus (MRSA) isolates represents a serious problem to low and media level microbiology labs. In this work cefoxitin (FOX) and cefotaxime (CTX) screen plates (AS) (8-16 microg/ml) with and without 4% of NaCl were evaluated to detect MRSA. Sensitivity, specificity, positive and negative predictive values were determined. The AS oxacillin and the agglutination test MRSA-Screen Latex for the detection of PLP2a were used as reference methods for the evaluation of the different studied screening plates. The 100% (94 strains) PLP2a positive were detected as MRSA with FOX (8 microg/ml), and CTX (8 microg/ml with 4% NaCl) AS. The advantage of FOX AS (8 microg/ml) is that it does not need the addition of NaCl, and CTX AS (8 microg/ml with 4% NaCl) is that cefotaxime is an antimicrobial easily accessible in our country.

[Vesicovaginal fistulas: correction using lyophilized dura mater].

Thirty three cases of surgically corrected vesicovaginal fistulae, from a total of 73 patients with iatrogenic or pathological urinary tract conditions examined and treated in our service, are presented in this paper. Both the etiology and different surgical techniques that can be used are reviewed, contributing the novelty use of freeze dried dura mater as the material to interpose for the correction of the fistulae by means of abdominal surgical techniques. The material was used in 6 patients, with recovery being obtained in all cases and achieving the fistulae closure with a single surgical procedure.

[Effect of in vitro infection with dengue virus (DEN-2) on various cellular immune response functions in the mouse].

When Dengue type 2 virus (DEN-2) is put in contact with spleen cells from DBA/2 mice that had been stimulated with Concanavalin A, it was found a decrease in the incorporation of (3H) Thymidine. Furthermore it was observed that the number of Antibody forming cells (Plaque forming cells) against SRBC was decreased, when lymphocytes from DBA/2 mice spleen in culture, had been stimulated with sheep red blood cells (SRBC) in vitro and then infected with DEN-2 virus and the interleukin-1 (IL-1) biosynthesis was quantified in the thymocyte system it was shown that macrophages produced high levels of IL-1 compared with non-infected cells, and that this increased levels could be similar to that produced when macrophages are stimulated with lipopolysaccharide form E. Coli (LPS). The above mentioned results suggest that DEN-2 virus is able of altering some functions of the immune response concerning T and B lymphocytes. Furthermore, the infection of P388D1 cells induce in the first 24 hours an over production of IL-1 that could be the reason why in the natural infection in humans, patients run a fever in the beginning of the viremia caused by DEN-2 virus related with the property of IL-1 reported as endogenous pyrogen.

[Biology of infection caused by the dengue virus].

In this review the most important aspects of the biology of the disease caused by the dengue virus are described: the characteristics of the disease, the epidemiology, the vector, the virus (proteins and genome), the diagnostic techniques and the vaccines.
