[The direction and motivation for an evolution in methods of antiulcer surgery].

A 23-year retroactive analysis of a heterogeneous series of observations with unfavourable tardy results after antiulcer surgeries showed that the most failures requiring a second surgery appear after large resections with gastrojejunal or gastroduodenal anastomosis and after vagotomies associated with gastric drainage. The most favourable tardy results followed the vagotomy associated with limited gastric resection (hemigastrectomy). This kind of intervention prevents the appearance of the ulcerous relapses and lowers significantly the incidence of the other type of post-surgical iatrogenic complications. The optimal protection against the ulcerous relapse given by vagotomy associated with hemigastrectomy permits a tactical adaptation of the intervention to the lesional and physiopathological characteristics of each case.

[Synthesis and physicochemical and biological characteristics of phthalazine derivatives].

Further studies in view of obtaining new stable phtalasiniuilides present the synthesis and physico-chemical and biological characterization of certain derivatives obtained through the action of p-phenyl-phenacyl and p-nitro-phenacyl bromides upon R-phtalasine. The structure of the new products was confirmed by physicochemical and spectral analyses. The preliminary biological tests showed a moderate antimicrobial activity of certain studies compounds.

[Pancreatic fistulas].

Based on 17 observations the authors review the major etiopathogenic, evolutive, clinical and therapeutic aspects of pancreatic fistulae. In 14 cases the cure was achieved by medical treatment while surgery was resorted to in only three cases. The medical measures are discussed, as well as the indications for surgical treatment.

[Complications of surgery of cancer of the colon].

In the paper a systematic analysis is carried out of postoperative complications of colon surgery, 118 patients had complications, out of a total of 400 interventions on the colon. The complications are studied in relation with the time of their occurence and the localization of the neoplastic process, as well as from the point of view of the influence exerted by age and the "background" of the patients. A review is also made of the therapeutic attitudes required by the various complications, of the results obtained and finally a detailed analysis is presented of the causes that led to death of operated patients.

[Treatment of supramalleolar open fractures by means of centro-medullary osteosynthesis of the fibula].

The study is concerned with 11 cases of open supra-malleolar fractures involving both bones. The tibial fracture was of the comminutive type, with a large number of small fragments allowing an exteriorization of the proximal end or telescoping of the ends with consecutive loss of the axis. Due to a difficult synthesis of the tibial bone synthesis of the peroneum with a thick rod or with a brooch was performed, recovering the length of the lever and allowing to de-telescope the tibial focus, to recover the normal length and a corresponding axis. Since in all the treated cases consolidation was achieved over a duration of 5 months, the advantage is stressed, of the procedure which, solving indirectly the fracture, protects the tibial focus, already poorly irrigated and susceptible to pseudarthrosis in thi;s type of location.
