A factor VII concentrate for therapeutic use.

A concentrate of factor VII suitable for therapeutic use has been prepared from human plasma by a method forming part of a comprehensive scheme of large-scale plasma fractionation. Factor VIII was separated as cryoprecipitate and factors II, IX and X were adsorbed on DEAE-cellulose. Most of the factor VII remained in the supernatant. By batch adsorption on DEAE-Sepharose, followed by elution on a chromatographic column, factor VII was concentrated about 25-fold, and purified about 50-fold compared with original plasma, without the need for further dialysis or concentration steps. Data are presented from 10 batches, each from 80-120 kg plasma. Following doses of factor VII to six congenitally deficient patients, the mean rise in plasma factor VII was 95-100% of theoretical; the half-disappearance time was about 4 h. The treatment of four patients with acquired deficiency of factor VII is also described. No untoward side effects were observed.

Two populations of factor VIII-related antigen in a family with von Willebrand's disease.

The factor VIII-related antigen in a patient with von Willebrand's disease showed increased anodal mobility on two-dimensional immunoelectrophoresis. However, the antigen present in several of this patient's relatives showed two distinct peaks, one in the normal position and one similar to that of the patient. Very little of the patient's antigen could be recovered from ethanol precipitates or cryoprecipitates of whole plasma. Only the slower component could be recovered from the precipitates prepared from plasma of these family members showing two populations of antigen, the faster moving antigen remaining in the supernatant. This antigen of increased mobility corresponds very closely to that remaining in the supernatant of normal plasma, following similar fractionation procedures. It is suggested that the factor VIII-related antigen in some variant forms of von Willebrand's disease is a normal component of plasma, present in increased amounts.

Home treatment in haemophilia: clinical, social and economic advantages.

Twenty-eight severely affected haemophiliacs were observed for 3 months under treatment as hospital out-patients and for the subsequent 9 months while treating themselves at home. Delay in receiving treatment and financial costs were both clearly reduced by home treatment, the patients recovered from individual bleeds more quickly and reported a greater sense of personal freedom and independence. The amount of treatment required did not materially change and no untoward effects were noted; the use of analgesics tended to be less.

Changes in blood coagulation during total hip replacement.

In seventeen patients with osteoarthrosis of the hip undergoing total hip replacement clotting factors II, V, and VIII were measured in blood taken from the operated limb and the general systemic circulation during surgery. Concentrations of these clotting factors were consistently significantly higher in the operated limb than in the rest of the circulation. This result indicates that measures to reduce venostasis in the operated leg during surgery may help to prevent deep-vein thrombosis.

Hemophilia A↗

A comparison of coagulation factor replacement with and without prednisolone in the treatment of haematuria in haemophilia and Christmas disease.

A double-blind controlled study was carried out to investigate the effectiveness of treatment with factor VIII or factor IX concentrate and a reducing dose of prednisolone in contolling haematuria in patients with haemophilia and Christmas disease. 41 episodes of haematuria were studied in 30 different patients. No appreciable benefit was observed in the treated, as compared with the control group and this is at variance with the results of the few studies reported elsewhere.

Clinical Trials as Topic↗

The sporadic case of haemophilia A.

Plasma samples from 41 mothers of haemophilic sons in "simplex families" (families with only one affected member) have been tested for factor VIII and factor-VIII-related antigen. The results suggest that 39 of these women are likely to be carriers of haemophilia.
