Current role of intravenous cholangiography.

One hundred consecutive intravenous cholangiograms were reviewed. In terms of bile duct and gallbladder visualization, 20 were of excellent quality, 40 were of adequate quality, and 40 were of poor quality or did not visualize. In retrospect, 12 of the studies were originally misinterpreted. Among the 29 patients who were eventually proven to have biliary tract disease, intravenous cholangiography was helpful in making the diagnosis in only 17 (59 percent). In 20 patients the study was performed to establish or rule out a diagnosis of acute cholecystitis; the test was helpful in only 9 (45 percent). It is concluded that intravenous cholangiography frequently results in marginal visualization that is subject to interpretive error. With the current variety of available tests that are able to visualize the biliary tree with a high degree of resolution, intravenous cholangiography has become a diagnostic anachronism.

Amebic colitis: correlation of proctoscopy before treatment and barium enema after treatment.

Twenty-seven patients with severe acute amebic dysentery were studied at the International Centre for Diarrhoeal Diseases Research Hospital in Dacca, Bangladesh. Patients were divided into two groups according to their pretreatment proctoscopic mucosal pattern. Those with mild lobular patterns (five patients) had a milder illness; posttreatment air-contrast barium enema examinations were normal except for subtle mucosal abnormalities in two (40%). In contrast, patients with necrotic ulceration proctoscopic patterns (22 patients) had a more severe illness; 19 (86%) had ulcerations, haustral abnormalities, and/or strictures demonstrated on posttreatment barium enema examinations. Persistent strictures were found in eight of 10 patients with necrotic ulceration patterns who had sequential barium enema examinations up to 9 months after treatment. These observations indicate that despite successful therapy with metronidazole, there is a high incidence of persisting colonic abnormalities in patients with the necrotic ulceration pattern. Since proctoscopy has a useful predictive value, it should be performed in all patients with severe amebic dysentery.

A new, treatable source of recurrent meningitis: basioccipital meningocele.

A 19-month-old boy suffered eight episodes of bacterial meningitis. During the ninth episode a meningocele of the basioccipital clivus communicating with the nasopharynx was discovered. Identification of the organism causing the episodes of meningitis was not helpful in pointing to the site of this congenital anatomic defect. Surgical closure of the defect has prevented further recurrences.

Diatrizoate distribution in dogs as a function of administration rate and time following intravenous injection.

In this study, we determined diatrizoate concentrations in plasma and the extravascular space in dogs following an injection of the same dose of diatrizoate under three different conditions: (A) subsequent to a rapid bolus injection of diatrizoate: (B) subsequent to a bolus injection followed by an intravenous drip infusion; and (C) subsequent to a slow intravenous drip infusion. The data indicate that the highest plasma level occurred immediately after bolus injection (A). Significantly more diatrizoate was present in plasma 10 to 25 min from the start of an intravenous drip infusion given over 12.5 min (C) than when the same dose was given as a rapid bolus (A). The slow infusion method also resulted in significantly higher diatrizoate plasma levels from 15 to 25 min when compared to the bolus plus drip method (B). The differences in the calculated extravascular concentrations were insignificant among the three groups once all of the contrast material had been infused, i.e., after 12.5 min. The results are discussed in terms of their practical application to cranial computed tomography.

Noninvasive evaluation of ventricular function in cystic fibrosis.

The cardiac function of 21 patients with cystic fibrosis was studied using radionuclides and M-mode echocardiography. The patients (mean age 13.2 years, range 4 to 27) had a wide range of clinical and pulmonary function abnormalities (mean Shwachman-Kulczycki score 62.1). Decreased right ventricular ejection fraction was found in 13 of 18 patients (72%); an additional four patients had abnormal septal motion on ECHO. Left ventricular ejection fraction was abnormal at rest in four patients (19%); an additional four patients had borderline low LVEF. The LV pre-ejection period to ejection time ratio increased significantly (i.e., worsening LV performance) with deterioration of S-K score, chest radiograph score, and forced expiratory volume in 1 second. Three of eight patients with normal LVEF at rest had an abnormal response to supine bicycle exercise: LVEF fell in two patients and was unchanged in one. Thus evidence of LV dysfunction was observed in seven of 21 (33%) of the patients; four at rest and in three only during exercise.

Management of natal teeth.

1. Obtain dental radiographs whenever possible before removing natal teeth in order to better advise the parents of complications. 2. Leave natal teeth in the mouth as long as possible in order to decrease the likelihood of removing permanent tooth buds with the natal tooth. 3. Obtain the parents' consent and rule out neonatal hypoprothrombinemia before removing natal teeth. 4. Recommend postoperative evaluation by a pedodontist in order to obtain diagnostic radiographs and necessary treatment.

Custom finger guard-mouth prop as an alternative to conventional mouth props.

Conventional mouth props are an essential part of a dentist's armamentarium and are irreplaceable in certain dental situations. These props, however, may provide too harsh an approach or be injurious in other situations. The custom-made finger prop is a valuable alternative to the other mouth props when treating the young or handicapped.

Localized tumor-like lesions in ulcerative colitis and Crohn's disease of the colon.

Ulcerative colitis and Crohn's disease of the colon may manifest roentgenographically as localized tumor-like lesions of the colon. Such lesions are often of inflammatory origin but are likely to be mistaken for polypoid carcinoma or infiltrating submucosal malignancy. Four patients with localized inflammatory lesions of the colon that mimicked a neoplastic process are reported. The clinical features and the radiological-pathological correlation of such lesions are presented.

Utilization of elecrical conductivity as an alternative method of assessing marginal leakage of pit and fissure sealants.

The electrical conductivity readings of extracted human teeth uere taken: prior to placement of pit and fissure sealants; immediately after placement of the pit and fissure sealants; 2 weeks after immersion of the specimens in distilled water at 37 degrees C; and after 7200 strokes on a toothbrushing machine. The specimens were then immersed in dye for 24 h, sectioned and evaluated for evidence of dye penetration. The electrical conductivity readings were then compared to dye penetration results to determine if a correlation existed. A positive correlation existed between the electrical conductivity readings and dye penetration. Threshold values of electrical conductivity existed (which varied for each sealant) - readings above these threshold values indicated sealant failure or leakage. The possibility of using electrical conductivity as a diagnostic tool is definitely worthy of further research - particularly its potential as an in vivo method of assessing leakage.
