Stenosis of the renal graft artery is a relatively common complication in renal transplantation. Therapy consists of either transluminal angioplasty or surgical correction. Nevertheless some untreated cases show good spontaneous evolution. We describe a well-documented case of graft artery stenosis causing severe hypertension with declining renal function which experienced spontaneous resolution.

We have studied 32 patients with idiopathic nephrotic syndrome aged from 3 to 59 years. The clinical course of 20 patients with IgM mesangial deposits was compared with that of 12 patients without glomerular immune deposits. The presence of IgM deposits seems to be unrelated to any particular clinical onset, histological pattern on light microscopy, therapy response, or clinical course in our relatively short follow-up. The conclusion from this study is that IgM mesangial deposition is not a marker for response to therapy in patients with idiopathic nephrotic syndrome.
Incidence of intradialytic symptomatic hypotension was significantly reduced by lowering dialysate temperature from 37 degrees C to 35 degrees C. This improvement seems not to be mediated by temperature-induced changes in membrane biocompatibility, since leukocytes, platelets and complement activation were similar in both situations.
A 40-year-old woman on chronic hemodialysis had been diagnosed as having lepromatous leprosy at the age of 17 and treated for 15 years with sulfones. She remained clinically free of leprosy during 19 months of hemodialysis and then underwent successful renal transplantation. Fourteen months after surgery, recurrence of leprosy was observed. In spite of immunosuppression, the skin lesions healed with sulfone treatment. Renal transplantation is a useful treatment in patients with leprosy and chronic renal failure.
Coagulation studies were performed in 47 hemodialysis patients in order to assess abnormalities predisposing to thrombosis. A very high incidence of Lupus Anticoagulant was detected, a finding not previously described. This work is a preliminary report of this association and of its possible clinical implications.

Over a four year period, 34 patients on chronic haemodialysis with non A-non B hepatitis were studied. Although clinical symptoms were mild or absent in most patients, biochemical evolution seemed to be serious as more than 70 per cent became chronic or showed a relapsing pattern. Blood transfusions play an important role in the spread of this disease; on the other hand in our experience patient to patient transmission was less relevant.

Renal function was evaluated before and after intravenous urography (IVU) in 124 randomly selected patients. In cases with renal insufficiency (RI) (serum creatinine level, greater than or equal to 2 mg/dL), the incidence of renal impairment was higher (11 of 20 patients, 55%) than in the group without RI (16 of 104 patients, 15%). In the latter group, high blood pressure (BP) was associated with a higher frequency of renal impairment (28.6% vs 10.5%). Advanced age, mild proteinuria, and a single functioning kidney were not risk factors. The IVU preparation contributed to renal function impairment in ten cases, while in the other 17 cases, the iodinated contrast material was the only factor apparently involved. Renal function returned to its previous level in a mean period of 12 days. One patient suffered progressive and irreversible renal failure, and two others had a slight, persistent impairment of renal function. It was concluded that the incidence of renal function impairment is high, but cases are usually mild and reversible. The most important predisposing factors are RI and high BP.
