The Ocutech Vision Enhancing System (VES): utilization and preference study.
An NIH-sponsored user and prescriber preference trial was conducted comparing a new low vision telescope system, the Ocutech Vision Enhancing System (VES) to two conventional Keplarian spectacle telescope systems in a controlled cross-over clinical study performed at four independent low vision clinics. Fifty-five visually impaired individuals, 18-81 years of age, with both previous and no telescope experience, were followed for 8-16 weeks. The experimental and control devices were compared on a series of subject-selected visual tasks using a standardized clinical protocol. Subjects were trained in the use of the systems with standardized clinical methods. The data shows a statistically significant preference of the new design by both patient and clinician with the overriding factors relating to appearance, weight, adjustability and the fit of the frame.

Chemoimmunotherapy for maintenance in acute myeloblastic leukemia.
Of 30 adult patients with acute myeloblastic leukemia, 14 achieved complete remission. Eight of these were given chemoimmunotherapy for maintenance. The immunotherapy consisted of intradermal pooled allogeneic leukemic cells (snap-frozen irradiated) and BCG vaccine given by Heaf gun, given twice in 4 weeks. The chemotherapy was given for 1 week in 4 weeks. The median duration of remission in these eight patients was 115+ weeks and the median duration of survival was 147+ weeks. The other six patients who were given chemotherapy only for maintenance had a median duration of remission of 15 weeks and a median survival of 52 weeks. The two groups cannot be compared properly, however, as allocation of patients was not random, and the chemotherapy differed significantly.

Measurement of clozapine and norclozapine in plasma/serum by high performance liquid chromatography with ultraviolet detection.
A simple method for the measurement of clozapine and its N-desmethyl metabolite in human plasma or serum by high performance liquid chromatography is described. An internal standard (aqueous nortriptyline, 4 mg/L) (50 microL) and Tris buffer (2 mol/L, pH 10.6) (100 microL) are added to plasma/serum (200 microL) and the analytes and internal standard extracted into methyl tert-butyl ether (200 microL). The extracts are analysed on a 150 mm column containing Spherisorb S5SCX using methanol containing ammonium perchlorate (35 mmol/L, pH 6.7) as eluent at a flow rate of 1.5 mL/min. Detection is by ultraviolet absorption (215 nm). The limit of detection is better than 0.05 mg/L for both analytes and the intra-assay precision (CV) for clozapine and norclozapine was 5.3 and 7.3% at 0.5 mg/L and 2.6 and 2.8% at 1.5 mg/L, respectively. The method can be applied to the measurement of these compounds in plasma after acute overdosage, for the assessment of compliance in patients apparently refractory to therapy and to identify interactions between clozapine and other neuroleptic and antidepressant drugs which may effect toxicity.

British and American health policy: recent lessons from one system to the other.
Arguably, US and British health-care policies have shown some convergence in recent years, despite vast differences in the two health-care systems. In part, this reflects the incentives common to all systems which must be mobilized to improve efficiency and effectiveness; in part, this reflects the (two-way) trans-Atlantic 'trendiness' of ideas. This article seeks to examine the main context of policy affecting the two health-care systems, the means now used to achieve the systems' goals, and the validity of claims of convergence. The focus is conceptual, although a significant aim is to assess recent policies in a comparative framework.
