Comparison of transvaginal ultrasound, hysteroscopy, and dilatation and curettage in the diagnosis of abnormal vaginal bleeding and intrauterine pathology in perimenopausal and postmenopausal women.

STUDY OBJECTIVE: To compare transvaginal ultrasound, hysteroscopy, and dilation and curettage (D&C) in the evaluation of women with perimenopausal and postmenopausal bleeding. DESIGN: Descriptive study (Canadian Task Force classification II-1). SETTING: Seven outpatient clinics. PATIENTS: One thousand two hundred eighty-six women. INTERVENTION: Transvaginal ultrasound, hysteroscopy, and D&C. MEASUREMENTS AND MAIN RESULTS: Of our patient population, 29 (2.26%) had a histologic diagnosis of endometrial carcinoma; in 2 of them (7.14%) endometrial thickness was 5 mm or less. In 10 women (34.5%), endometrial carcinoma was missed by hysteroscopy (sensitivity 65.52%, specificity 99.92%). Complication rate of D&C was 1.4%. CONCLUSION: In women with perimenopausal and postmenopausal bleeding neither transvaginal ultrasound nor hysteroscopy as a single diagnostic tool is suitable to rule out endometrial cancer.

[Aspects of peritoneal fascia closure after surgical laparoscopy].

Following laparoscopic surgery using trocar ports of 10 mm and more Richter's hernia and incarceration of bowel are reported. To prevent this wound closure of all layers is a must. Newly developed techniques are described. Fascia closure is accompanied by serious pain. Pain management is achieved by local anesthesia.

Gynecologic laparoscopy and hysteroscopy in a day clinic: trends and perspectives.

We have increased the performance of minimally invasive gynecologic surgery from about 15% to 20% of operations a year, especially hysteroscopies and laparoscopies, many of which are performed in a free-standing ambulatory unit. In 1992 our team performed 8057 outpatient operations, significantly more than in 1991. As the techniques are developed, the number of indications for these operations expands. By maintaining high standards, even with major endoscopic procedures, we have a very low complication rate.

[Ovarian stimulation by a new therapeutic principle for the treatment of complications of the climacteric. Results of a double-blind crossover study with a defined spleen dialysate].

In a placebo-controlled double-blind cross-over study (test period 8 weeks) the spleen dialysate Solcosplen is tested for effectiveness and tolerance in 40 women, i.e. 14 pre- resp. 26 postmenopausal women in two consecutive periods (test periods I and II). The treatment begins in the first week of each test period with 2 ampoules i.m. t.i.d. and is to be continued for the following 7 weeks with 2 dragees b.i.d. Besides the incidence of clinical symptoms there intensity is summarized in a graded form to the Kupperman index. For further objectifying the therapeutic results vaginal-cytological examinations and radioimmunological analyses of estradiol (E2), LH, FSH and DHEA-S are carried out. The courses of therapy are frequented on a balanced basis. The progress of clinical symptoms shows in respect of incidence of appearance and intensity measured in Kupperman index in Phase I as well as in the cross-over statistically clear reciprocity, which demonstrates - with homogeneous prefindings - significantly differing results of treatment in favour of the dialysate. In the intraindividual comparison of judging the effectiveness by the physician and patient yields significant preferences for the verum. Side-effects were not observed. The proven effectiveness of the spleen dialysate within this study is explained by the stimulation of the ovarian residual function during menopause. It is discussed and compared to the presently applied monotherapeutic E2-substitution.

[A 70-year-old controversial discussion: spleen and gonad function in medical literature].

The development of a protein-free spleen-dialysate has led to a new discussion arguing about the old question as to whether there is a link between the spleen and the endocrine system. Experimental animal-trials demonstrate that the therapy with spleen dialysate is regarded superior to an adequate dose of gonadotropins. The present contradictory results in the field of spleen research cannot yet offer a valid model of the mechanisms how it works. The therapy with spleen-dialysate in various fields of indications demonstrates first remarkable results without any serious side-effects.
