[Clinical trial with barbexaclone in epilepsies].

A open clinic assay with barbexaclone which is an association of phenobarvital with light stimulant of the central nervous system (tau-l-ciclohexil-2-methil-aminopropane) for the treatment of epileptic disorders is reported. The authors have studied 40 patients aged between 0-12 years of age, of both sexes, all of them having convulsive seizures of the Grand Mal type associated or not to other epileptic manifestations. After analysing the different aspects of the group they consider satisfactory the results obtained: very good or good in 82,5% of the cases.

The L-dopa sparing effect of G 31,406 in the treatment of parkinson's disease.

The AA. study 60 cases of Parkinsonism treated with levodopa associated to G31,406 (dibenzazepnic derivative), to amantadine and to a placebo. Drug-associations: 1) L-dopa + G 31.406, of anticataleptic effect, probably because of its anticholinergic activity and adrenergic central and/or dopaminergic stimulating action; 2) L-Dopa + chl. amantadine; 3) L-dopa + placebo. The patients were previously under exclusive use of L-dopa. The assay verifies: a) if there is any benefit with these associations and in what proportions; b) incidence and intensity of side effects; c) comparative study. The assay also intends to evaluate the effect of the G 31.406 on the depression which usually occur in Parkinsonians.

Induced pneumoperitoneum in CT evaluation of peritoneal carcinomatosis.

BACKGROUND: Imaging of peritoneal carcinomatosis is a well-known problem even for technologies as recent as computed tomography (CT). The purpose of this study was to evaluate whether CT performed after induced pneumoperitoneum (CT-PP) could have a higher sensitivity in the detection of peritoneal implants over conventional CT. METHODS: Five patients with known ovarian malignancies underwent standard CT and CT-PP. Exploratory laparotomy was performed with a maximum interval of 7 days from the last imaging procedure. Results were prospectively compared with surgical findings on a compartment to compartment basis. RESULTS: CT-PP was well-tolerated with no serious adverse reactions registered. The anterior and visceral peritoneum, the paracolic gutters and subphrenic areas were particularly well depicted but not the pelvis which was poorly evaluated in all cases. CT-PP detected all the three cases where peritoneal carcinomatosis was present even when metastatic nodules were smaller than 2 mm; it also showed intraabdominal adhesions in two patients, an important finding that precludes the use of intraperitoneal chemotherapy. CONCLUSIONS: With CT-PP there seems to be a reduction in the threshold of detectability of peritoneal implants. The direct demonstration of intraperitoneal adhesions is an important secondary finding. Disadvantages of CT-PP are (1) it is a time-consuming method and (2) it does not evaluate all the peritoneal recesses potentially involved in peritoneal carcinomatosis.

Monte Carlo studies for irradiation process planning at the Portuguese gamma irradiation facility.

The paper describes a Monte Carlo study for planning the irradiation of test samples for microbiological validation of distinct products in the Portuguese Gamma Irradiation Facility. Three different irradiation geometries have been used. Simulated and experimental results are compared and good agreement is observed. It is shown that Monte Carlo simulation improves process understanding, predicts absorbed dose distributions and calculates dose uniformity in different products. Based on these results, irradiation planning of the product can be performed.

Cytologic diagnosis of vaginal metastasis from renal cell carcinoma. A case report.

BACKGROUND: Metastasis of renal cell carcinoma to the vagina is rare, although it may be the first evidence of the existence of the primary tumor. CASE: A metastatic deposit of renal cell carcinoma in the vagina was diagnosed by cytology as clear cell adenocarcinoma, which was confirmed by biopsy. Radiographic and ultrasound examinations confirmed the renal site of origin, which was corroborated by immunohistochemistry of the biopsy specimen. CONCLUSION: When a cytologic diagnosis of vaginal clear cell adenocarcinoma is made, metastasis of renal cell carcinoma should be considered in the differential diagnosis.

Birth after electroejaculation coupled to intracytoplasmic sperm injection in a gun-shot spinal cord-injured man.

We describe the case of a man who, after a gun-shot wound, has become paraplegic. As a consequence of his spinal cord injury he developed infertility due to incomplete erection and anejaculation. After several unsuccessful penile vibratory stimulation attempts, it was possible to achieve ejaculation by means of transrectal electrostimulation. However, the total sperm motility count, the percentage of normal sperm morphology, and the vitality and hypo-osmotic swelling test scores were rather low, and in particular viability was strikingly decreased during sperm preparation. These parameters further decreased following electroejaculate, forcing us to use intracytoplasmic sperm injection instead of conventional in vitro fertilization for the treatment cycle. A normal pregnancy was achieved which resulted in birth of a healthy girl on 1st January 1997.

[Factors of recurrence of intraepithelial lesions of the uterine cervix].

MATERIAL AND METHODS: A retrospective study (1994-1996) of 282 cases of intraepithelial lesions treated in the Gynecology Services of Coimbra University Hospitals and subsequent follow-up (9-45 months). The following parameters were evaluated as recurrence factors: the patient's age, lesion degree, associated HPV infection, treatment type, safety margin in the ablative treatments and interval free of illness. RESULTS: Of the 282 cases, 72.4% (N = 204) corresponded to HSIL and 27.6% (N = 78) to LSIL. The patients' mean was 36.3 +/- 9.1 [19-67] years. The most commonly used treatment types were the LLETZ (large loop excision of the transformation zone) (76.9%) followed by CO2 LASER vaporization (16%) and finally cold-knife conization (7.1%). The safety margins were determined in 184 cases and, on average, were of 3.2 +/- 2.4 [1-10] mm. Forty-four recurrences occurred (15.6%) of which 75% corresponded to HSIL lesions (N = 33) and 25% (N = 11) to LSIL. From the total number of recurrences the association to HPV infection was found in 24 of the cases (54.5%) and 75% of these (N = 19) corresponded to HSIL. The average age in the recurrence group was 38.2 +/- 8.7 [21-53] years and 35.9 +/- 9.2 [19-67] years in the group without recurrences (p > 0.05). In the LSIL group treated with CO2 LASER (N = 32) the recurrence rate was 15.6% versus 13.3% in the group treated with LLETZ (N = 45). In the HSIL that were treated with CO2 LASER vaporization (N = 13) the recurrence rate was 23%; in those treated with LLETZ (N = 172) there were 16.2% recurrences and in the group submitted to cold-knife conization (N = 19) that rate was 10.5%. CONCLUSIONS: The total rate of recurrence was 15.6% with the greatest incidence of lesions associated to HPV (p > 0.05). Regarding the particular case of the HSIL, a great number of recurrences with the destructive treatments (CO2 LASER) was observed when compared to the ablative treatments (cold-knife conization or LLETZ).

Molecular characterization of Brazilian infectious bursal disease viruses.

A reverse transcriptase-polymerase chain reaction (RT-PCR) procedure was used to amplify a VP2 gene fragment (248 bp) from infectious bursal disease virus (IBDV). The procedure allowed the detection of known IBDV strains from the United States, along with field isolates and commercial vaccines produced in Brazil. Amplified VP2 fragments were further characterized by restriction fragment length polymorphism (RFLP) analysis. From 55 Brazilian commercial flocks, 48 field samples were IBDV positive by RT-TCR. Vaccine RFLP patterns were found in 12 flocks, a pattern compatible with classic IBDV in one flock, four new patterns in 31 flocks, and a pattern compatible with very virulent (vv) IBDV in four flocks. Sequence analysis showed that the vvIBDV RFLP patterns were closely related to the vvIBDVs described in Europe and Asia. Phylogenetic analysis of the four new RFLP patterns showed that they were closely related to but distinct from other classic, variant, and vvIBDVs, suggesting a high prevalence of different IBDV strains in Brazilian commercial flocks.
