Preventing drug interactions in the home: a five-step approach for client teaching.

An important area of pharmacologic concern, particularly in the home setting, is that of drug interactions. The 1997 action by the Food and Drug Administration (FDA) to remove fenfluramine and dexfenfluramine, causing the weight-control phen-fen scare, is a high-profile example of an issue that occurs every day with little fanfare. In drug administration and supervision, there are many factors of concern. This article illustrates five basic factors to consider to prevent drug interactions in the home setting.

An Internet primer. Part I.

The Internet provides electronic communications and access to information around the globe. In only one decade of public use this format has emerged as the method of rapid access to information, and projections are for continued growth. The WWW consists of over 40,000,000 servers containing billions of pages of information on virtually every conceivable subject. This doesn't include news servers, FTP servers, bulletin boards, and other online information sources. The home healthcare nurse, now and in the future, needs knowledge about and access to this system, to tap into the most current government recommendations about medications and treatment protocols, to access health statistics, consumer information, and additional educational, professional, and client-centered articles and opportunities. We hope this new Home Healthcare Nurse feature, "Staying Connected," will develop into a source of ongoing, up-to-date information to assist you in accessing and utilizing this valuable resource. Next month, we'll provide some specific directions for navigating the information superhighway.

Benchmarking in home health care: a collaborative approach.

Benchmarking outcome data and constructing "best practices" to meet patient care needs are emerging trends and new mandates on the horizon in home care. The Arizona Association for Home Care Continuous Quality Improvement Forum embarked on a journey to develop standards to define and monitor urinary tract infections in the home. A collaborative process among six home care agencies allowed for comparison of urinary tract infection rates and a beginning step toward establishing benchmarking standards in home care.

Searching the Web.

The Web is an extraordinarily dynamic medium that is growing in size and complexity at a furious pace. Web pages already number in the billions and roughly double each year. The contents of the Web are largely unstructured, which may make finding specific information a considerable challenge. However, once you develop some skill at navigating this realm, you'll reap considerable rewards. Surfing the Net can become habit forming, and as your proficiency increases, the challenge may well be knowing when to stop.

Rehabilitation nursing: a curriculum for home care nurses.

Elderly patients look to nurses for the majority of their home care, including rehabilitative services. This model curriculum in geriatric rehabilitation nursing incorporates both clinical skills and case management and provides hands-on agency experience to enhance the learning process.

Family satisfaction with nursing care is difficult to evaluate because there are many factors which influence it, especially in critical care areas. These authors present a model for evaluating the primary factors which affect family satisfaction with critical care nursing.

Family satisfaction techniques: meeting family expectations.

With the current focus on outcomes of care and promoting the image of nursing there is significant attention on how families perceive nursing care. These authors provided a patient satisfaction model in a previous article in DCCN (September-October, page 272). Here they describe nursing techniques which help to increase family satisfaction by meeting the families expectations or helping them to adjust their expectations.

Tuberculosis: a new threat from an old nemesis.

Mycobacterium tuberculosis (TB), which only a few years ago was thought to be under control and soon to be eliminated, has resurfaced as a real threat in the home and community setting. This article discusses TB symptoms, therapies (including a discussion of the drug-resistant forms of TB), prevention and control strategies, and the role of the home healthcare nurse in the surveillance for and management of TB.

Pneumonia is as significant concern for caregivers of the elderly, often because of the obscure manifestation of the disease and reports of increasing mortality. Key diagnostic criteria, prevention measures, and management strategies are reviewed for the home healthcare nurse.

Patient satisfaction in home healthcare: what are the essentials?

Addressing patient satisfaction evaluations in home healthcare is a multifaceted approach and has reached new importance due to the move to outcome based care within the quality-of-care arena and new or pending governmental or accreditation standards (Figure 2). Home healthcare agencies must be responsive to patient needs and provide supporting documentation for care that meets patient demands and expectations for care. Identifying what nurses and staff do for patients is no longer enough. The results of interventions are the driving force in home healthcare today. Obtaining positive results requires incorporating the key components of patient satisfaction: the agency, the staff, and patient service mandates that focus on consumer rights and responsibilities. Providing meaningful and useful evaluation results will assist home care managers with the required interventions to assure satisfaction of patients with home healthcare services.

Using plug-ins and Internet browser extensions.

Plug-ins and helper applications are programs that add additional capability to computer Web browsers. These browser extenders allow us to access the most current and most dynamic information on the Web: material that can assist us in fulfilling our professional role now and in the future. We can--and most of us will need to--add browser extensions to our desktop computers both at the time a Web site advises us that such an extension is needed, or we can load the most common browser extensions in advance (see "Essential Plug-Ins"). These steps may seem daunting, but are only inconveniences to our use of the Web as a vital resource and should not be seen as barriers. Happy hunting!

Lyme disease: an emerging infection with home healthcare implications.

The 11,424 cases of Lyme disease reported in 1994 represented a nearly 40% increase from the previous year. The dramatic increase of Lyme disease in the United States points to the need for home healthcare nurses to review their knowledge of the clinical manifestations, treatment, and prevention measures for this emerging infection.

The tuberculin skin test. Administration and interpretation.

With the incidence of tuberculosis on the rise, home healthcare nurses are challenged to maintain skills in administering healthcare and interpreting tuberculosis (TB) tests. In this article, the most widely used method of TB testing, the Mantoux PPD test, is presented. The procedures for administering the test, the methods of evaluating the results, and consideration of the variables that affect proper administration and interpretation in the home setting are discussed.
