Clinical and urodynamic effects of anterior colporrhaphy and vaginal hysterectomy for prolapse with and without incontinence.

The clinical and urodynamic effects of anterior colporrhaphy and vaginal hysterectomy were studied in 73 patients, of whom 29 had incontinence due to urethral sphincter incompetence. Pre-and post-operative urodynamic assessment was made and follow-up continued for 2 years. Symptoms of urge incontinence, stress incontinence and prolapse were significantly reduced following surgery. Urodynamic data showed no significant change. The incidence of detrusor instability and voiding difficulties was not increased.

The effects of urinary incontinence in women.

Urinary incontinence has usually been assessed in the laboratory and no previous attempt has been made to measure the degree and extent of restrictions which are imposed on the individual. A scale was developed to measure the effect of being incontinent. This scale was derived from a questionnaire administered to women attending a urodynamic clinic. In these patients, all of whom had established urinary incontinence, the major problems encountered were in social and mental well-being, including sexual difficulties and embarrassment. The degree of incontinence did not co-relate with the degree of disability.

The causes and nursing management of constipation.

Constipation is a common symptom in the Western world with up to 10% of the population experiencing some difficulty with bowel evacuation. The consequences are seldom serious or life-threatening, but constipation is undoubtedly the cause of much chronic discomfort and misery. Most nurses will encounter constipated patients on a regular basis. It is therefore important for nurses to understand the causes of constipation so that an appropriate history can be taken and further investigations suggested if necessary.
